Tesla’s Full Self-Driving (FSD) system has repeatedly demonstrated unsafe behavior in my 2021 Model Y (VIN [XXX] , ~60,184 miles). The issues involve Adaptive Cruise Control, Lane Keeping Assistance, and Forward Collision Warning. FSD frequently follows vehicles too closely, leaving insufficient distance. When I disengage, I am left with no safe option: braking hard risks being rear-ended and triggers a hard-braking event, while lifting off the accelerator leaves me dangerously close, still flagged as unsafe following. FSD often waits until less than a mile before highway exits to attempt lane changes, causing last-second darting, missed exits, or unsafe maneuvers. It also performs excessive and unnecessary lane changes that add risk without improving efficiency. Merges are frequently unsafe, forcing me to brake hard to let others pass or accelerate aggressively to avoid being cut off. Immediately after an over-the-air (OTA) update on Aug 8, 2025, my vehicle began showing camera connection errors and disabled multiple safety/ADAS features. Over the following days, I repeatedly received alerts that AEB was unavailable. The vehicle remained drivable, but AEB and related safety functions were inactive. Chronology: Aug 8, 2025: OTA software update to 2025.26.6 completed. Right after completion, the touchscreen displayed camera connection errors and disabled AEB / Autopilot / cruise control. Aug 11, 2025: While driving, I again received an “AEB unavailable” alert. Similar alerts appeared intermittently throughout the week. There was no water intrusion, impact, or external damage. Cameras were clean and unobstructed. I scheduled the earliest available Tesla Service appointment and reported the AEB failure as a safety issue. The front collision warning is too sensitive and reacts unnecessarily to many events that are not a risk for collision. In addition, the sound of the warning is too loud causing the driver to become unnecessarily startled and may cause possible loss of control of the vehicle.

Summoned my car in parking lot. Vehicle showed on app that it planned to go straight then turn left but instead cut to a left and thus colliding with the car that was parked next to me. I have the footage if needed. Car should have stopped instead, or followed planned route. I was staring at the car the whole time and it looked that it was going straight, even after colliding, it looked like it was aimed to go forward. The car created a log of what happened if needed.
Tesla released a feature called Obstacle Aware Acceleration. The Owner's Manual states: "Obstacle-Aware Acceleration is designed to reduce the impact of a collision by reducing motor torque and in some cases applying the brakes, if Model Y detects an object in its driving path. The touchscreen displays a visual warning and sounds a chime when the brakes are automatically applied. For example, Model Y, while parked in front of a closed garage door with Drive engaged, detects that you have pressed hard on the accelerator pedal. Although Model Y still accelerates and hits the garage door, the reduced torque may result in less damage. Obstacle-Aware Acceleration is designed to operate only when all of these conditions are simultaneously met: Drive or Reverse is engaged. Model Y is stopped or traveling less than 10 mph (16 km/h). Tesla FSD consistently ignores temporary lane closures, and will cheerfully try to drive over the barrier poles. The locale is the San Diego Pure Water Project construction zones. They set up lane closures that change every week or so. They use a line of red poles along the middle of the road to block off a lane and direct both lanes of traffic into one lane. Sometimes there is also a large CLOSED sign in the middle of the closed lane. Tesla never sees the red poles and often does not see the CLOSED sign. Vehicle not acting safely, speeding up to pass stopped school bus with stop sign out and yellow lights flashing. This is a generic VIN as it applies to the current software on all FSD cars.
I was driving to work this morning, February 7th, 2024 driving at freeway speeds. My Tesla touchscreen dropped out and went completely dark. The reset process is to press and hold for about 30 seconds and another 30 seconds for the full restart. I attempted this twice and proceeded to pull off the highway. During this period, I could not see how fast I was driving, whether my brake lights, turn signals, or headlights worked at this time to safely transit through traffic to get off the road. I put my hazard lights on, but again did not know if these worked or not. Computer will suddenly error out and cruise control, AEB, forward collision warnings, and lane keep assist are not available. A warning "cruise control unavailable" shows if you attempt to engage autopilot or FSD. No forward collision warnings, no auto slowing down. Not docked for close following. No visualizations made. Not resolved with soft or hard reboot. Sometimes will randomly will allow to re-engage, other times I have to park the car and let it go to sleep without charging or sentry mode for things to reset. I've driven many, multiple times including for 1.5-2hour segments with a frozen visualization and no AEB or cruise control or FSD. Speed independent, weather independent. Time of day independent.
I was driving the speed limit with the adaptive cruise control engaged on I-95 in Maine with no one in front of to the sides of me. All of a sudden without warning my car brakes on the middle of the highway. I regained control right after and did not use the adaptive cruise control for the rest of the drive.
On February 19, 2023, I was driving my 2021 Tesla Model Y on I-5 from LA to Sacramento on a clear afternoon. I was using adaptive cruise control (ACC) set at 78 mph, but not using auto-pilot, in the fast lane with a car following me. My car suddenly braked aggressively without anything in the roadway ahead of me. I then applied the foot brake and veered off onto the gravel left shoulder to avoid having the car behind me hit me. I saw in the rear-view mirror the following car swerve to the right to get around me, entering the slow lane. I believe that when I stepped on the brake, the ACC turned off and the car's automatic braking ceased. I then got back into the fast lane as there were no other cars approaching and resumed my drive. On a long trip on I-70 across Colorado and Kansas I experienced 4 instances of "phantom" braking while using cruise control. I had set the speed at 78 and the car aggressively braked itself as if there was an object in the road when there was no object or reason to brake. In each instance if a car had been following closely they likely would have crashed into me. It also causes panic to the driver when the car aggressively brakes. I am contacting Tesla service today to report this issue and hope that it will be addressed but there has not been any inspection at this point. There was no waring or issues with the car prior to this and it has never happened to me before. It only occurred when using cruise control as I do not have full self driving and was also not using lane assist.

I was driving on highway when the car in front of me brake hard. Tesla failed to detect imminent crash and did not provide warning AND emergency braking. The vehicle has been recently serviced in tesla service center. There were no other warning lamps / messages / symptoms of the problem prior to the failure.

UNKNOWN This incident happened on 10/16/2022 while on I-95 while the vehicle was engaged in the Auto-Pilot mode on the highway (We do not use this autopilot mode unless we are o the highway for long distance drives and that is very infrequent). The vehicle suddenly swerved around and lost control while on auto pilot. As always my hands were on the wheel and I could undo the auto pilot and take control of the vehicle. Fortunately there were no other vehicles around us and there was no damage! We have filed another claim previously with the NHTSA - 11455695
My wife and I recently drove cross country in our Tesla Y. Occasionally, we had the forward collision warning go off for no reason. More importantly, when we had the adaptive cruise control on and particularly when the autopilot system on, we experienced episodes of sudden braking when there were no other vehicles or other objects in the vicinity. This happened, on average, once or twice every 30 minutes. It got to the point where we had to keep the cruise control off because it seemed to put us in potentially dangerous situations. Vehicle was driving with auto steer enabled at 65 mph on freeway and braked unexpectedly and unnecessarily 3 times within a few minutes. Each time it occurred, I canceled auto steer and re-engaged. At no time were any obstructions or objects visible in front of the vehicle, and at no time did any warning lights or messages appear, the car simply braked without notice. Safety was at risk as cars behind me were traveling closely and were not expecting my car to brake suddenly, although I responded quickly enough to prevent further braking and accelerated out of danger. I have researched "phantom braking" and it seems this incident is exactly what was described in those reports. The car has not been inspected since the incident. I am hesitant to use auto steer since I do not trust that the cameras are adequately detecting what's in front of the vehicle.
Tesla Autopilot (Adaptive Cruise Control) rarely recognizes parked vehicles or obstructions beside the driving lane (right or left side) based on the display console driving environment display. Bicyclist, pedestrians, and other large roadside obstructions are also rarely recognized. Might explain the recent Tesla collisions with stopped emergency vehicles while in Autopilot mode. Don’t know if Teslas recent elimination of radar sensing made this problem worst or not. This failure to recognize nearby obstructions happens daily.

The driver infotainment system locks up randomly. Which makes all safety features, turn signal indicators, backup camera, reverse mode (mirrors drop), speedometer, collision sensors, etc NON-FUNCTIONAL. Until either the system crashes or finally unfreezes. This also removes one-pedal driving as well when frozen. Version is 2022.16.3

I've had it in Tesla's service center several times for inspection. They always say the technicians can't fix the phantom braking problem, that only the engineers can, and that they are working on it. It's been one year and there hasn't been a software update to remedy the dangerous flaw. In August 2021, I was promised by Tesla that a software update would be issued soon to remedy the problem. It still hasn't. I'm very exasperated. I understand Tesla was supposed to give you information about this issue on June 20, 2022. I am anxious to hear about Tesla's response. Furthermore, I sincerely hope the Tesla has told you they are fixing the problem. Has it? Please let me know of updates and proposed fixes to Tesla's phantom braking.
There are 4 issues to report for our Tesla Model Y: 1) On the highway, our Tesla often presses on the brakes and slows down by itself when it should not be doing so. This occurs when we have adaptive cruise control on. We’ve noticed that at the time of the slowdown, the Tesla misreads the highway speed limit. For example, the Tesla will read the speed limit as 70 mph, then all of sudden without warning, the Tesla will suddenly read the speed limit at 40 mph, which then causes the Tesla to break and slow down by itself, when the speed limit is actually still 70 mph. This can happen several times on a single highway trip. Aside from the shock we receive as a driver as our vehicle slows unexpectedly, this could cause a rear end collision if a car is following us too closely from behind....

There hasn't! I have had five appointments with the Tesla service center, and they continually say "we can't fix it, only the software engineers can". The braking episodes are dangerous! I implore you to hold Tesla accountable and demand they fix the problem immediately. PLEASE!
I was traveling early morning before sunrise over the tehachapi pass towards Mojave in California. Someone had lost a camper shell from the top of a pickup truck in the lane. It was still dark outside. I saw the obstruction in the road but the car cameras did not pick it up. I pulled into the left lane to try to get around the camper shell. The lane assist pulled me right back into the camper shell which tore down the side of the car doing 10k in damage. The car never recognized impact or an accident. I have the videos from the cameras of the car. The lane assist, as far as I know, only doesn’t activate when the turn signal is used, which I did not have time for, as I had less than 200 feet between me and the camper shell.
Tesla Phantom breaking issues on my Model Y. Clear roads and then sudden deceleration in perfect weather conditions with no nearby cars
While using the autopilot (not full self driving feature) on the highway, I have experienced multiple inatances sudden braking for no reason. No vehicle has been in front of me in these instances and it has occurred most often on undivided highways and highways where there is a hill. This has made engaging the autopilot dangerous as there are cars behind me and I could have been rear-ended on multiple occasions. Additionally, while driving manually on city streets where there are stop signs, there have been multiple instances where the vehicle has given me a forward collision warning because I am slowing down to a vehicle that is ahead of me at the stop sign. This has most often happened on uphill and downhill instances where there are stop signs.
Car was on autopilot full self driving. Truck hit car and vehicle was severely side scrapped. Over 35k in damages. Autopilot did not stop or drive to the side to avoid collision. Autopilot failed Videos can be provided.
Recently while pulling into my children's school, my car has started deciding that the entrance to the parking garage is a large truck stopped diagonally in front of me and slams on the brakes scaring me and causing everything in the car to go flying. Luckily no one has been behind me while this has happened. I am always entering very slowly, maybe 5 mph at the very fastest.
We were driving roughly 80 mph on an open highway. Clear skies. No cruise control/autopilot engaged. Vehicle slammed on the brakes. Threw both occupants rapidly forward in our seats. Lucky no cars behind us.

I was stopping at the parking lot of my church St. Thomas Syro-Malabar Catholic Forane Church (508 Elizabeth Ave, Somerset, NJ 08873) to drop my daughter. I applied the break but the vehicle kept moving and hit the gas line of the church. The gasline broke and car front got damaged. There was no auto break and warning of the obstacle.
We've owned this car since delivered in June 2021. Took a trip from AZ to MO in July 2021. When using cruise control, the system occasionally and severely activated the brakes when passing a large vehicle on a four lane divided highway and in some cases no one else was around on the 4 lane divided highway. Auto braking only happened if on cruise control and did not occur if cruise not activated. I mentioned this to a service tech at one of the charging stations in Rolla, MO who indicated it was a known issue and they were working on it. I didn't report it to Tesla thinking it was being handled, but now I see many others reporting it and you dates indicate cars produced prior to April 2021. I'm assuming ours was produced around that time.
I was driving northbound on Highway 1 on 3-18-22 from Ventura California to Clayton California where I live. Four times through my trip the car suddenly decelerated and the brakes were applied automatically for no reason whatsoever there was no traffic in front of me there were no obstacles in front of me there were no hazards in front of me, it was open road and for whatever reason the car auto braked which immediately reduced my speed to a dangerous condition. If I had been in traffic I would’ve been rear ended!
While driving from Tucson to Denver the vehicle suddenly decelerated and/or braked no less than hundreds of times with varying rates--sometimes just a couple miles per hour and other times more than 30 miles per hour. These "phantom braking" events were generally on a divided interstate highway with clear visibility, flat, straight roads without rain/snow & oftentimes without another vehicle within 1/2 mile+. These events even happened 3 times within 3-4 minutes while I was speaking to Tesla roadside service trying to disable all sensors/interventions. More than once a vehicle behind me (at highway speeds) thought I was "brake-checking" them and would cause them to honk and deservedly give me a few finger symbols (road rage). This complaint is specific to the automatic control of the Model Y related to "PHANTOM BRAKING". This is an ongoing problem with the "AUTOPILOT" and "FULL SELF DRIVING" functions on my car, it has always had. Most of the miles on my car are freeway and highway miles. Regardless of which of the automatic functions selected on my car, "phantom braking" occurs repeatedly in a startling fashion that doesn't seem to be related to the environment the car is being driven in. The problem is consistent enough that I will not use any of the auto driving functions when I have passengers in the car with me. The problem requires constant intervention by the driver. There are 2 versions of this type of un-commanded breaking: 1. Unexpected braking with no computer warnings or alerts. It can be hard braking or, light to medium breaking, for no apparent or justifiable reason....
Sudden and dramatic application of emergency collision braking for no apparent reason. No objects in the road in the driving path of the vehicle, but the emergency braking system engages. Available for inspection upon request. Having this occur during traffic can result in being rear ended as the braking can be so severely applied. Bodily injury in the form of whiplash is also foreseeable. The problem has not been reproduced by the dealer or service center. The vehicle has not been inspected for these problems. There is a warning chime prior to almost every event. Sometimes it just slows without warning. They first occurred on our first long road trip beyond twenty miles.
This vehicle when on ADAS (aka autopilot) and adaptive cruise control I experience constant "phantom" braking where car will slow down or slam on brakes for seemingly no reason. Most noticeable on a non separated highway phantom braking occurs constantly and is very predictable when large truck is near on the oncoming lane; happens probably 80% of the time. Slamming on the brakes for no reason is alarming and dangerous in my estimation. This system is completely unusable and very unnerving. Ironically I had a 2019 Model 3 with the same features and did this very rarely so this feature is much worse than it was on older vehicles with radar.
I was driving on a 2 lane highway at about 60 mph in the middle of my lane. The adaptive cruise control was used to manage my speed. Another vehicle was travelling in the opposite direction going toward me, and in the middle of their lane. The other vehicle was an 18 wheel truck. Suddenly, with no warning, the braking system slowed my vehicle. Both myself and my wife felt our seat belts tighten quickly. There was a vehicle behind us, that nearly hit us when the brakes were applied so quickly, for no apparent reason. I had to quickly put my foot on the accelerator pedal to stop the braking, and get back to the correct speed. This has happened several times, usually when driving on a road that gently curves to the right. The sudden braking also occurred when there was no other traffic, just a new section...
These issues occur w/ no warning, occur randomly, in all conditions. Issue 1. Vehicle randomly slams on the brakes with no obstructions or vehicles in front. This occurs at highway speeds as well as neighborhood speeds. I'm worried about being rear-ended. This occurs daily. Issue 2. Vehicle thinks oncoming traffic is in same lane and attempts to emergency stop. Issue 3. Upon changing lanes, using auto lane change, vehicle serves back I to lane for no reason.
Car slams on brakes at inappropriate times. Ex: 1. When a car makes a left turn across my travel lane several blocks away. 2. When a vehicle moves into my travel lane, going the same direction, several blocks in front of me. 3. For no reason I can discern. Cars behind me often honk. Could easily rear end me. Thankfully hasn't happened. You are welcome to inspect it. Don't think service center has tried to replicate the braking. No accident so not reports. Just very scary, thankfully. No warning lamps, etc. Anticipating question about dates, happens randomly, but pretty frequently. At least weekly. Speed also variable. Sometimes 35, sometimes 70, and in-between
The Tesla Model Y had a software update at 12:05 am on Feb 13, 2022, which was complete. I drove the car to local YMCA at around 8:15 am on Sunday Feb 13, 2022, for my swim exercise and drove it home at around 9:30 am. I planned to park it by backing up to my driveway so that I can charge it. I positioned my car, with the front facing my neighbor's house. My foot was on brake and my right hand was moving the Drive Stalk towards R for reverse. The car all of the sudden flew forward on its own! It was only stopped after it hit my neighbor's house, making a big hole into her house, and causing severe damages to it outside and inside (smashed furniture and falling glassware), as well as to her landscaping. When this happened, my foot was still on the brake...
While driving my Tesla Model Y with Cruise Control activated, the vehicle would inexplicably brake/decelerate rapidly without any vehicle traffic in front of the car. It would occur at its worst while changing lanes, as I approached another vehicle from behind and attempted to change lanes to overtake. I would either be mostly in the other lane, or had already successfully changed lanes, when the vehicle inexplicably braked/decelerated. It was alarming, frustrating, and infuriating as it seemed clearly unjustified. This occurred while on the interstate, which was extremely dangerous given the speed traveling at as well as the number of vehicles that typically travel on the interstate. I have not used the feature since, and have even attempted to try and disable the feature while still being able to utilize some sort of basic cruise control feature, to no avail.
I am aware that NHTSA is investigating Tesla over the emergency braking issue when using its advanced features. When I have read articles about this they always reference auto pilot or full self driving. I wanted to make sure you knew that this issue also impacted adaptive cruise control. When I brought my Model Y home in August of last year I quickly learned I could not safely use the adaptive cruise control. I live in rural NE Iowa, and mainly drive on two lane roads. If I was on a downhill slope and meeting a vehicle with any kind of height to it the car would activate emergency braking, even though the other vehicle was completely in its own lane. Autopilot suddenly breaks to a dangerous levels while driving in the highway. Sometimes this happens with incoming cars/trucks on the other line. I have to manually take over control as I am worried I might be rear ended. These events are very likely to happen at least once on a given commute. This sudden braking also happens while going over a certain bridge. This has happened to all drivers in this car.
I have had issues since i purchased this vehicle in march of 2021 where driving down the highway with Adaptive Cruise Control enabled and it will suddenly slam on the brakes at highway speeds with no other vehicles around. This sudden braking is so severe at times that everyone in the car is thrown forward and bracing for a false impact. This has caused several close calls for vehicles following me. Also the severity of the braking in winter weather causes the vehicle to lose traction and potentially place all the occupants and others on the road at risk. During my time in the FSD Beta program i was able to report all of these events, sometimes dozens of times a day. Eventually I asked to be removed from the program as it seemed worse in the program than when on a standard build version of the software.
We are familiar with managing the controls which, in my experience, operate similarly to other vehicles with similar safety and driver-assist features. Unfortunately, in regards to the effectiveness and ease of operation of the TACC, our Tesla is by far the worst of the 5 vehicles with which I'm familiar. The experience of managing TACC with oncoming traffic is frustrating, distracting and sometimes unsafe. Autosteer, because it builds on TACC's systems, is similarly deficient although the features it provides would be extremely welcome when TACC is improved. Nearly every oncoming large truck (including many pickup trucks) caused TACC to brake the car TACC assisted braking was inconsistent.
