All Forward Collision Avoidance Automatic Emergency Braking Complaints
All the ADAS systems have stopped working. I get warnings and beeping as I drive my vehicle. Numerous people are telling me that it is the $2,000 front sensing camera that needs to be replaced. This is a huge safety concern and it is plaguing the 3rd generation Honda Pilots.
While driving, multiple safety systems suddenly failed at the same time including Adaptive Cruise Control, Lane Keeping Assist, Collision Mitigation Braking System, Road Departure Mitigation, and Auto High Beam. All warning lights appeared simultaneously. The vehicle became unsafe to drive relying only on manual control. This appears to be related to the front-facing camera or sensor malfunction. Many other Honda Pilot owners have reported the same issue.
The front camera driving assist malfunctioned, causing all driving safety mitigation to be stopped. This is not an issue due to the driver, it is an issue with the camera either malfunctioning from heat from sun exposure or being faulty due to manufacturing issues. I reported to Auto National Honda Valencia & they said it would cost approximately $2200 to fix. Dash light did come on and are continuing to go on until the faulty camera is replaced.
After only 76K miles the ADAS stopped working. Honda Sense safety features no longer work. Honda dealership diagnosed the problem and said the ADAS camera needed to be replaced. The car is in excellent condition. It was never in an accident, windshield in perfect condition. Up to date with its scheduled maintenance.
Front camera stopped working all of a sudden and didn't help with lane assistance.
My car "lurches" when increasing speed sometimes acting like it does not want to go at all. I am constantly on edge that I am going to get rear ended or hit. Especially when my children are with me. Occasionally the brake light will light up and my car will slow down and then accelerate again. Which also makes me worry about getting hit. Both of these things have happened multiple times and we've done our research. It's a major issue
Vehicle adaptive cruise control and emergency braking completely stopped working.
There was an internal failure of the multi-purpose camera located behind the windshield rear-view mirror. It was inspected by Russ Darrow Honda and the failure code diagnosed. The camera needed to be replaced. Service included a video from the dealer and the defective camera was so hot he could barely hold it while wearing gloves. This caused the Lane-Keeping Assist, Adaptive Cruise Control, Auto High Beam, Collision Mitigation and Road Departure Mitigation Systems to fail, and the dummy lights on the dash to warn of the system failures. There was no accident or injury resulting yet, but none of the advanced safety features were operable. We have since learned that this camera is a common failure in Honda vehicles, and we feel there needs to be a recall. This critical and expensive safety feature should not be failing this frequently, or in vehicles this new.
The front winshield camera stop warning and now Im getting the following warnings: lane departure warning, Adaptive Cruise Control Automatic Emergency Braking Blind Spot Warning Forward Collision Warning Lane Departure Warning Lane Keeping Assistance And now the car is showing: Emission System Warning

This vehicle continues to have issues related to a problem that was issued for the fuel pump earlier this year. My fuel pump was replaced, but the vehicle continues to do the same thing. It stalls and jerks while I am driving and does not start sometimes after getting gas. When I am trying to accelerate the vehicle does not go at full power. One time I was going out into traffic and had to reverse because I knew that I would not make it and get hit. In addition to this, the vehicle has an issue with the braking system. Other years have been recalled for what they call ghost breaking, and although this one does this, it has not been recalled. I will be driving and the car will all of a sudden just brake on its own as if there is a vehicle/object in front of me....
To Whom It May Concern, I am submitting this complaint through the National Highway Traffic Safety Administration regarding a serious and unresolved safety-related defect on my 2019 Honda Pilot (VIN: [XXX] ]). The vehicle’s front camera system failed, disabling essential safety features including Lane Keeping Assist, Adaptive Cruise Control, and Collision Mitigation Braking. These systems are part of Honda’s Advanced Driver Assistance System (ADAS) and are integral to safe vehicle operation. Their failure constitutes a critical safety defect, not a routine maintenance issue. Misrepresentation by American Honda I was initially assigned to a case manager named Christian at American Honda. During our recorded phone conversations, Christian falsely claimed that the discount I received from the dealership was provided by American Honda, even though the dealership is an independent franchisee and American Honda has no control or direct relationship over the dealership. The ACC, LKAS, Collision Mitigation, High beam, lane departure warnings light errors are on. This happened 2 years ago and Honda wanted me to pay $1,200 to replace all sensors of this car. Of course I just left it. It's not worth the hassel. However, for a car bought brand new in 2019 to have these systems fail in 2023, and for this to be a main component advertised for consumers to buy vehicles, it is not good. We pay thousands of dollars for a car because it has these safety features only for it to fail within 4 years and then expect the customer to pay over $1,000 to fix, is simply not right.
After receiving a CMBS error message I tried to drive home and the vehicle automatically engaged brakes while merging with no other vehicle near by and I was almost rear ended.
Sensor lights turned on and frequent events occurred causing lights to come on. Took to the dealer and informed me code u3000-49 multi purpose camera unit wave radar internal electronic failure upon researching several other people have the same issue and has not been resolved.
During normal operation of the vehicle, the dashboard information display on the gauge cluster lit up displaying the following error messages: Adaptive Cruise Control Problem, Collision Mitigation System Problem, Lane Keeping Assist Problem, Auto High-Beam Problem, Road Departure Mitigation System Problem. Additionally there is a fixed "dash light" that is showing "BRAKE SYSTEM" illuminated in amber color. After research I have found that Honda issued a TSB for other vehicles with their sensing system for the same issue and referenced the high-mounted mono camera on the windshield. This issue extends to the 2019 Pilot Elite that I own and significantly reduces the safety features of this vehicle.

Multiple warning lights and alarms sounding after starting the engine: Collision Mitigation System Problem. Road Departure Mitigation System Problem. Adaptive Cruise Control Problem. Lane Keeping Assist Problem. Auto High Beam Braking System Error. When these warning lights are on, the vehicle continues to drive and brake as if these features were not installed. HOWEVER, since the "Brake System Error" light is now always illuminated, there is no longer an on dash warning to indicate an actual problem with the braking system. This began in August of 2024 as a single episode. It escalated to intermittent occurrences. It now happens every time. Malfunctioning braking and lane minding systems pose obvious danger to passengers. Problem was noted by Virginia State Vehicle Inspection station.
The front multipurpose camera that controls all safety features has failed. This is a major safety hazard because almost every safety feature is non functioning. These include automatic emergency brake, blind spot warning, forward collision warning, lane departure warning and all included in the ADAS.
Our Honda had a failure while driving on the highway. The camera behind the windshield apparently failed all at once and this caused all of the safety features to stop functioning and we almost had a wreck trying to get across the busy lanes of traffic because when this camera failed it caused the service brakes light on the dash to come on and stay on as well as the adaptive cruise control, the lane keep assist, the emergency braking, the blind spot warning all to stop functioning immediately without warning. We had the vehicle towed to the Honda dealer and had to pay to have the $1300 camera behind the windshield replaced on a vehicle that’s barely 5 years old. This camera that is so ingrained into all of the safety features that are the main reason we bought this vehicle should not be failing. Honda refused to assist...
The cars smart car controls stopped working. ACC, Collision Mitigation System, LKAS, Road Departure Mitigation, and Auto High Beam Problem warning lights are on.
When we are driving the vehicle brake light lights up on dash and car breaks for a second or two. There are no cars around and no railroad tracks. It just does it a random times. We aren’t speeding. Just driving down road.
Vehicle would brake suddenly. Then shortly thereafter, the collision mitigation system error appeared on the dash. Honda dealership says the camera no longer works and the entire system needs to be replaced. The fact that the camera stopped working spontaneously is concerning.
Vehicle brakes or collision warning comes on when roads are curved and traffic in oncoming lanes.
The forward collision warning and automatic braking system are terrible and have been terrible since we purchased our Pilot new from a dealer. The warning lights and beep go off frequently when driving on highways and surface streets, in fact it’s nearly impossible to have an incident-free trip longer than 20-30 minutes. The automatic braking system has engaged many times, causing unexpected and sudden slowing of the vehicle, which has the potential to cause confusion, loss of control, or even a vehicular accident. Making matters worse, one cannot simply disengage the system permanently; it must be turned off each time the engine is started and requires holding a button down for several seconds.
The front camera has gone out on the Honda pilot, but inconsistently works. It will turn on while driving and turn off while the adaptive cruise control is on. The vehicle does not have a way to bypass the camera, so then there is consistently lights and alerts on. With not being able to bypass the camera features they will automatically come on or off while driving while the camera is not working properly. This vehicle was purchased in October of 2019, and has had camera issues since January 2024. The vehicle is not even 5 years old and has consistently had problems. The fuel injectors went out at 3 years, which I would not expect to go out, and from discussion with dealership there was an extended warranty for the same year Honda Odyessy, which was not extended for the Honda Pilot, but both vehicles have the same fuel...
Radar and multi purpose camera simultaneously failed. Dash lights came on indicating that auto high beams, lane keep assist, forward collision, adaptive cruise control, and braking system where not working properly. I think my safety and the safety of others where compromised with these systems not working.
All electrical systems failed at once due to a malfunction in the front camera-sudden failure with no damage to the vehicle physically. Honda refuses to fix or honor the issue. A recall should be required due to the vast amounts of complaints on the same issues. This should not be a financial responsibility on the owner.
While driving on very straight and non-rolling hills the large red words BRAKE will appear on the dash, the brakes will start pulsing and slightly shaking for a few seconds when there is very minimal on-coming traffic. The above has happened several times and first occurred several years ago and at different mileages and different roads
Automatic braking system failure parallel issue that is same to the accord and other models with the emergency braking system which randomly activated and panic stops and sometimes without an traffic near to car and other times have almost caused catastrophic collisions on highway systems with divided medians causing system to activate.
When I am driving and at times the braking system tell me to Brake when I am in my line and no one is heading my way.
The Honda Sense camera failed due to an internal fault just outside the manufacturer's warranty. Dealership was needed to replace it in order to regain function at out of pocket cost. Upon the camera failing the driver is met with 5 warning lights that the following are no longer active: LKAS, ACC, Automatic Braking\Forward Collision Warning, Adaptive Headlights, and Lane Mitigation System. You have to clear each of these before you can use the main dash (including MPH). Cruise control is completely disabled. Although there is no asserted timeline for the life of the camera, research shows (and dealership sates) that this is a known issue. Reaching out to Honda's technical support team returns a somewhat blanket response stating that although campaigns exist for other issues with my VIN it is my monetary responsibility to handle this particular issue, although I was writing to the campaign department to add to...
This morning 01/30/2024, I was travelling in the right lane of two on the highway. The vehicle slowed rapidly just about to a stop. I barely had time to pull to the right shoulder. I narrowly was struck from a vehicle travelling behind. This was from one of the safety mechanisms in the Pilot SUV I think the lane departure sensor. This was very scary. I feel this is a huge safety concern and need to report it. If I didn't have a shoulder what the heck would have happened to me. I probably would have been struck from behind and who knows possibly injured or worse. The shoulder was approximately 8 ft wide thankfully. While driving on two lane roads and roads with left turn lanes, the "BRAKE" warning comes on almost every other on-coming car. Most instances it just the warning light and sound, but every once in a while the automatic brakes are applied by the system momentarily at times. This became worse on a highway while driving in the left lane and passing a truck that was in a left turn lane, the system braked immediately and without prior warning, nearly causing a rear end collision by a vehicle behind. This occurred twice on a recent trip. Went to the Honda dealer and they informed me the only thing they can do is set it to the closest setting or turn off the driving assist system.
Transmission fluid is burning up within 15,000 miles or less. When the flood is going bad the vehicle studders as you accelerate and it's not safe. Ive seen a lot of people online with the same issue. Contacted Honda of America and they also know about the issue and are not doing anything to help.
When i turned on my vehicle errors started coming up Collision Mitigation System problem, Road departure mitigation system problem, auto high beam problem, Adaptive cruise control problem and Lane keeping assist problem. Also has orange Brake System highlighted. Unable to use any of the safety features mentioned as well as cruise control. Took vehicle to Honda dealer on February 6th and was informed the camera behind the rearview mirror had gone bad. There were no warnings prior. I had driven the vehicle around and stopped to get gas and when I turned it back on all the errors begun to appear. This was the first time they appeared and they have been there since as the dealer wanted over $1800 to repair this. After looking on the internet/Facebook Honda Pilot group there have been several others that have had this same issue with their 2019 Honda Pilot.
My car was parked in my driveway, when I started the car, 4-5 warning lights started to flash in orange, ACC, LKAS, Automatic High Beam, Collision Mitigation Problem, Road Departure Mitigation Problem. When driving the vehicle, none of these safety features are working. Took the vehicle to the dealer and was told the Multi Purpose Camera had an internal failure and it was likely due to heat exposure. The vehicle only has 32,000 miles on it and it was not at any unusual heat exposure. The past 3 summers the temperature has been the same and no problems. This appears to be a Manufacturer Defect on this camera but dealer won't accept that. The same day I took my car to the dealer, on 7.12.23 for the check up, I was told 4 other customers came in with the same problem. That is Not Normal. I live in Southern California...
Rear ended the vehicle in front. Vehicle in front had emergency braking activate, our pilot did not activate emergency braking. It does often go off on 2 lane highways when a large vehicle is coming the other way or when trying to take a freeway exit, but apparently not when you're about to hit someone.
Vehicle automatically applies brakes in several unwanted situations: (1) driving down 2 lane road going around curve with other vehicle approaching from opposite direction (2) something small in roadway such as plastic bag or small limb (3) normal stopping when car thinks other vehicle is too close in front but driver is under complete braking control These actions are dangerous at normal highway speeds and on wet road!!!! This has happened multiple times, not just on date listed below.
AEB/FCW. On January 24, 2023 while traveling on I-15 the Control Panel flashed “Warning” and the brakes were applied. There was no vehicle in my lane. There were other vehicles in adjacent lanes. This occurred additionally on the following dates. 1/22/23 (2x). 1/17/23, 1/12/23(3x) and 12/20/22.
This is a 2019 Honda Pilot with start stop function. There is a noticeable delay in activation and getting longer with time; this is a 3 year old vehicle. During 2 other instances, the system failed and vehicle would not start on a public street. During these failures and delays, other than the obvious collision safety issue, the safety systems are unavailable via a message on the dash. When it eventually starts back up it functions normally. This is clearly the system prematurely failing or the system prematurely killing the battery (which has a normal functional life of 5 - 7 years).
The engine start/stop idle does not start up again after the car stops when at a red light or in traffic.
Several times since 2019 while driving on not straight roads(slight curves either left or right), the Automatic Emergency Braking would activate, First I would get a big bold BRAKE in Red letters on the Display. I would be looking around, no cars are coming, I'm in the correct lane, not speeding and sometimes the car would actually start applying the brake(s) when there is absolutely no reason to apply the brakes. This has happened several times while we have driven the vehicle either on a 5 to 6 hour trip and maybe has occurred a total of 5 times since we have owned the vehicle
I currently have a 2019 Honda, Pilot with approximately 70,000 miles. I have had problems with the idle stop feature since the first year of owning the vehicle. I have had many instances of a restart resulting in actuating the warning lights on my instrument panel and a few times when my vehicle did not restart. My dealer either was not able to diagnose the problem or did not know how. I even paid to have my battery replaced, which helped elevate the problem but did not fix it.I also either have a problem with the automatic braking system giving me a warning or in some instances actually applying the brakes when there is no traffic.Both of these are reoccurring and on-going problems.
2019 Honda Pilot Collision Mitigation System flashing BRAKE on screen average 3 times per day. Includes shaking of steering wheel & if cruise control on, braking. All this is happening when there is nothing blocking car. If cruise control on & need to respond to car that just pulled in front of me on highway, because the brake pedal is already suppressed by the car I can not respond properly. The only thing I have found to override the car is to push the gas pedal, which is also very dangerous!! Please help!! This is so dangerous. I have had the assisted emergency braking deploy itself with the Brake light blinking on the dash board when there is no reason for it to engage. No traffic around, just a sudden braking which pushes you forward in your seat and quickly scan all around you for impact which is not going to occur. I have heard of this happening to others but this has happened to me about 6 times and my husband has had it twice.
On several occasions the car has flashed BRAKE and then started braking when there has been no car in front of me.
Driving and the automatic braking engage with no vehicles around
While entering the express lane on I-595 E my Honda 2019 Pilot unexpectedly reduced speed applying the brakes. There were no cars in front or behind me and the yet the emergency braking system was applied. This could have caused a rear end collision. In speaking with a local Honda service dealer he had said that this had also happened to him twice. My incident occurred approx. 1 year ago. I've also read that this issue has been identified in Honda CR-V's and Accords.
When driving my Honda Pilot the Automatic Emergency Braking system has engaged multiple times (3-4 times with me, and once with my husband driving) in the middle of driving with there was no reason for the braking to occur - Meaning there were times when no cars were even in front of me. When this happens the orange BRAKE warning shows up on my dashboard advising "BRAKE" and my car jolts forward to strongly break. There has not been a recall on this issue, therefore, there has not inspection on my car specific to this issue. When this occurs it causes extreme panic on unawareness of what is going on.
