Over the past 2 weeks there have been multiple (5+ times in one day) instances of phantom breaking both using adaptive cruise control and autopilot. This often happens when approaching a bridge but also in areas where bridges are not present. Sometimes the sun may be towards the front of the car, sometimes not. Luckily no collisions, but the sudden breaking is a safety hazard and prevents the use of the car’s advanced features that are meant to reduce driver fatigue.
Phantom Braking while driving interstate 15 on May 8, 2023 at 11:15 AM. No other cars were within a half mile in front or behind. Car went from 70mph with hard braking until I took control to regain speed. Had there been other vehicles behind me, it could have resulted in a rear end collision. Reported to Tesla today, May 15th, but told there is no fix. I have not used the Autopilot feature much at all, since most driving is short city trips. I do not intend to use this feature for safety concerns.
For the FSD beta, it does not currently parse and respect road signage such as “No Turn on Red” and “Detour”. This constitutes a material safety risk and also a moving violation.
I experienced multiple instances of ghost braking while driving at highway speeds, one that resuled in a near collision with the vehicle behind me.
I was driving down the highway. The weather was clear. Sunny. The road was dry. Middle of the afternoon. I was using the Adaptive Cruise Control. I was driving the speed limit, 70 MPH after driving for a couple of hours, the car had about three different times when it felt like the brake was tapped, the car slowed a bit and then regained speed. I stopped at my destination for about 4 hours and then returned home. Same roads conditions, sunny, dry road, etc. Same road, 70 MPH, the car suddenly hit the brakes HARD. Slammed might be too strong of a word because it's not like the car left skid marks on the road, but the brakes hit very, very hard. The car slowed to 25 or 30 MPH. Fortunately there were no cars behind me or me and my wife could had been killed right then.
Last weekend, during a 600-mile road trip, I experienced at least 20 instances of phantom braking when using the traffic-aware cruise control. A rapid deceleration of the car for no apparent reason is dangerous and needs to be fixed. I have complained to Tesla on a couple of occasions and there now seems to be more instances of the braking issue occurring since my last highway trip.
Phantom braking of Tesla: While using the AutoPilot feature to assist driving, on April 13, 2023 between 10 am and Noon while traveling on US Highway 2 on a clear sunny day on a 4 lane highway with excellent visibility and no traffic, the Tesla suddenly and unexpectedly slammed on the brakes for no apparent reason. I placed a service request to Tesla the next day and the texting Tesla service tech said he need a timestamp within 5 minutes of the incident. Because I could not define the time within 5 minutes of when it happened, there was no further information available. We finished the texting episode by cancelling the service appointment and I will not use the AutoPilot feature on the Tesla until Tesla says it is safe. Vehicles continues to make random unwanted and dangerous sudden braking at highways speeds while the cruise control is engaged.
While utilizing the ADAS features including the adaptive cruise control, the vehicle unexpectedly applies its brakes while driving at highway speeds.
Sudden deceleration and braking while at interstate speeds and crossing over a bridge. I was using cruise control only, overcast day, dry pavement with moderate traffic. Cruise control strongly applied the brakes from interstate speeds while crossing over an overpass bridge. There was no danger or traffic ahead of me that should have triggered the emergency braking. This needs to be investigated, it will get someone killed.
We recently traveled about 300 miles on a roadtrip in this Tesla and while using the adaptive cruise control, the car would randomly slam on the brakes for no apparent reason. There was nothing in the road or any hazards at all. Also, it was not just a one time thing. It happened multiple times before I decided to not use the cruise control because it to dangerous.
This is my second NHTSA report for this car, there was no reply to my first report. Since the manufacturer, without warning or owner consent, disabled radar systems for automated cruise control, the car is now exhibiting a new dangerous behavior when the camera detects the presence of emergency vehicle flashing lights and suddenly, and dangerously applies an emergency brake to slow the car, without taking into consideration the location of the emergency flashing lights relative to my car, or the presence of other cars around me that might be affected by the sudden, un-announced braking of my car. In many cases, the police/emergency lights are on the opposite side and direction of the freeway, 8+ lanes away from my car, and I am under no legal obligation to brake, nor would it be safe to do so. ON MARCH 18, 2023 WE WERE DRIVING ON HIGHWAY 80 FROM Winnemucca NV TO FERNLEY NV. At 3pm and about 3:10pm the car suddenly decelerated from ~82mph to ~50mph. The road ahead was clear of obsticles. Luckly there were no cars following close behind us, if there had been the unnecessary and unexpected deceleration could have been dangerous.
While driving the vehicle on a 2 lane highway in sunny conditions, the vehicle suddenly braked while using cruise control. There was no traffic on the road ahead, no oncoming or traffic in my lane. Car slammed on the brakes slowing from 55 to 30 in about 2 seconds. If there was a vehicle behind me this would have caused a serious accident. This is NOT an isolated incident on my vehicle. The exact scenario has been reported a multitude of times by other people. This will kill someone and needs a recall. This is the standard equipment cruise control provided on all Tesla models. This isn't self driving or any other advanced feature. This is a critical issue that needs to be resolved.
The contact owns a 2021Tesla Model Y. The contact stated while driving 65 MPH with the cruise control activated, the vehicle independently and aggressively slowed without driver input. The vehicle then accelerated independently. There was no warning light illuminated. The failure occurred approximately 10 times. The vehicle had not been taken to the dealer to be diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 65,000.
Multiple "phantom braking" events while using adaptive cruise control (no full self driving, just basic adaptive cruise). First time while going at interstate speed and crossing a bridge, vehicle saw a shadow on the road and slammed on the brakes thinking it was a vehicle. Obviously this is a startling and dangerous situation especially for vehicles behind you. This has also happened on the highway, flat terrain, roads were clear of any obstacles and it slams on the brakes thinking there is something in the road. Every time it does this, there appear to be shadows on the road (from street signs, light poles, trees, etc.). The sudden braking is of course bad, but there's also nothing in the vehicle that tells you why, no chime, it just hits the brakes. It still appears that cruise control is active, but it won't resume speed after an event like this until...
On February 19, 2023, I was driving my 2021 Tesla Model Y on I-5 from LA to Sacramento on a clear afternoon. I was using adaptive cruise control (ACC) set at 78 mph, but not using auto-pilot, in the fast lane with a car following me. My car suddenly braked aggressively without anything in the roadway ahead of me. I then applied the foot brake and veered off onto the gravel left shoulder to avoid having the car behind me hit me. I saw in the rear-view mirror the following car swerve to the right to get around me, entering the slow lane. I believe that when I stepped on the brake, the ACC turned off and the car's automatic braking ceased. I then got back into the fast lane as there were no other cars approaching and resumed my drive. Experienced phantom braking twice today on I 5. Almost crashed or got rear ended. Car was on auto pilot. This needs to be fixed
SYSTEM MALFUNCTION: Emergency braking ("phantom braking") while set to automated driving/adaptive cruise control. OUR SAFETY PUT AT RISK: On a 2,000 mi. road trip from Denver CO to Tucson, AZ and back, the vehicle slammed on the brakes at 75-80 mph speeds over two dozen times. All incidents occurred either driving south on I-25 or west on I-10 going to Tucson, or on I-10 east and I-25 north on our return. By far the most dangerous event occurred when we were driving east on I-10 in SE Arizona on our return to Denver on 29 January. Driving in the right lane at 75 mph on a perfectly clear day with excellent road conditions, the vehicle suddenly slammed on its brakes. Following closely behind us was a semi-tractor-trailer, that also slammed on its brakes, blasted its horn, and very nearly rear-ended us. The truck driver then followed us at very close...
While driving on cruise control, the car slammed on the brakes out of nowhere. It was terrifying and we were almost hit from behind. There was absolutely no obstruction or any reason for the car to slam on the brakes. We do not feel safe having our young children in the car. We have notified tesla of the issue many times and they refuse to fix it. This car is unsafe and someone, a Tesla driver or members of the public on the road with this car, is going to be killed by the phantom braking . What is the NHTSA waiting for? Please force a recall. Clearly Tesla doesn’t care.
I was driving on a large, multi-lane, divided freeway (5 south Interstate just before Anaheim, CA in Orange County, California) and using adaptive cruise control (not autopilot) to maintain my speed in the mid 70s MPH. It was around 9pm PT and there was very light traffic. The next car in front of me was at least 100 feet ahead of me and there weren't any cars on the left or right of me. All of a sudden the car started decelerating really quickly, and by the time I pressed the accelerator to counteract the deceleration, the car was at roughly 60 MPH. There wasn't any indication as to why the car decelerated so suddenly, and there wasn't anything that I could see that might have caused the cruise control to react that way. Jan 13, 2023 at 7:02AM PST. While using adaptive cruise control with lanekeeping, I was driving in second lane from the right at a constant speed. There was plenty of room in front of me- no car nearby. As I was slowly passing the car to my right, another car that was behind the car to my right began a normal lane change to its left into my lane behind me. For no reason, my car began to slow down suddenly. Video footage of cameras on my car show the moment when the front of my car pitched down because it was suddenly slowing down. Car sufferers from “phantom braking” and will aggressively brake while adaptive cruise control is engaged for no apparent reason. No “regular” cruise control is available to over ride this.
I have experienced phantom braking numerous times with this car so I plugged in an arbitrary date below. Fortunately it has generally been on rural, open roads where there was nobody close behind me, but there have been at least a couple of times where there were cars behind me and the braking came close to causing accidents. It is also terrifying when it does happen and has nearly caused me to lose control of the car more than once as I attempted to compensate for the abrupt braking. I understand that you are already investigating this issue with these cars so you know the details. Generally it happens when I am on 2-lane roads that don't have good dividers. Phantom braking
Tesla "Traffic Aware Cruise Control" occasionally unnecessarily applies brakes aggressively for no apparent reason when cruise control is in use. This presents a hazard to other driver's who may be behind the Tesla. This phenomena is well known to Tesla owners who have repeatedly posted about it in Tesla's Owners Forum. It is often referred to as "phantom braking". There is no option for the Tesla owner to disable the "Traffic Aware" aspect of Tesla's cruise control I believe Tesla could resolve this problem with a simple software update which would allow the Tesla owner to use cruise control normally without the "Traffic Aware" feature. The "Traffic Aware" feature is part of Tesla's problematic "Auto Pilot" feature. However, my Tesla is not equipped with "Auto Pilot". My Tesla is available for inspection upon request.
I had the adaptive cruise control (Autopilot) engaged on I95 and had it set to 60 MPH. Twice between a minute, the Model Y lowered the cruise speed to either 40 MPH and 25 MPH while on the interstate. No obstacles were in front of me to affect the cruise control system.
In Traffic Aware Cruise Control (aka Autopilot) car randomly changes the maximum set speed. This has caused a dangerous dramatic slowing of the car when highway driving. Car should not change speed setting without human input. I have video of this occurring.
My Model Y applied the brakes suddenly when it was in Adaptive Cruise Control while I was driving in the interstate at 75 mph. This caused the vehicle to decelerate very abruptly. There was no vehicle in front of me and no obvious debris in the road. This happened several times on a trip from California to Kansas and back. On one occasion , I had just pulled back into the right lane after passing several tractor trailer rigs going 70 mph. I had to tromp on the accelerator to avoid being rear-ended.

The contact owns a 2021 Tesla Model Y. The contact stated that while driving approximately 75 MPH with the cruise control activated, the vehicle abruptly decelerated to approximately 35 MPH. The contact indicated that during the incident no other vehicles or objects were nearby to cause the vehicle to suddenly decelerate. Additionally, there were no messages or warning lights displayed or illuminated. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure, but no assistance was offered. The failure mileage was 20,000.
We were driving 65 miles per hour using cruise control on a flat road with no cars in front or behind us for hundreds of yards on both sides of a two-way highway. Suddenly, and for no apparent reason, our Tesla applied the brakes hard as though it was trying to avoid hitting an obstacle on the road. Within a second our speed dropped to about 30mph at which point our Tesla released the brake and accelerated back up to 60 miles and hour before I shut off cruise control.
I drove from Flagstaff, AZ to Las Vegas, NV on 10/26/2022. I attempted to put my car in AutoPilot (adaptive cruise control) around a dozen times during this 260 mile trip. Every 10-25 miles the car would brake extremely hard forcing the passengers to lunge forward as if the brakes were being slammed. In every case, the car was in the same lane (no lane changes) with nothing but open road ahead. In most cases there wasn't a vehicle in front or to either side of me (only behind me). No corrective messages or indications appeared on the screen to explain this action. The only corrective action I could take was to hit the accelerator to avoid a rear end collision. When I contacted Tesla about the issue, they checked the logs during this trip and found no faults in the system to explain this behavior.
UNKNOWN This incident happened on 10/16/2022 while on I-95 while the vehicle was engaged in the Auto-Pilot mode on the highway (We do not use this autopilot mode unless we are o the highway for long distance drives and that is very infrequent). The vehicle suddenly swerved around and lost control while on auto pilot. As always my hands were on the wheel and I could undo the auto pilot and take control of the vehicle. Fortunately there were no other vehicles around us and there was no damage! We have filed another claim previously with the NHTSA - 11455695
The vehicle is subject to phantom braking. When I use the adaptive cruise control and am driving in the middle or left lane (i.e. when there is a clear lane to my right), the car will brake when there is no car beside me. Sometimes this occurs when I am passing a large truck on my right. Last weekend, I was driving 68mph on a highway during a bright sunny day. There were no cars on either side of me. The car braked for no reason. I had to step on the accelerator pedal to get back to highways speeds.
Phantom braking is common when driving on highway with advanced cruise control. On a recent trip from Tucson to San Diego I noted seven events in a 300 mile stretch. There were no cars within 1/2 mile in front of me, or any road debris during any event (I saved dash cam footage each time). There was a car behind me during one particularly violent event. I regularly commute on I-5 between San Diego and Irvine. One event per 70 mile trip is about the average. I’m almost used to it now.
While driving with cruise control set to 70-74mph, we had several incidents of phantom braking on a recent road trip. From 70mph with no obstacles, trees, structures, or other cars in sight, the car would suddenly brake itself. This happened multiple times, sometimes just a minute or so after the previous time. We were saved from being rear-ended only because the cars behind us were far enough back, but it was terrifying nevertheless. We cannot use cruise control anymore. Tesla must address this totally unacceptable safety issue.
I was going on I-95 North (Palm Beach County) on Autopilot (and I had Full Self Driving enabled in my car). It was raining, and I was at moderate speed, I would say 45-50 mph. The car started spinning uncontrollably. By miracle nodoby hit me, the card corrected after what I believe were 2 spins and continue on the same direction as traffic. I believe, with the nerves of the moment, that about 45 to 60 seconds later, I put auto pilot again, not knowing what hapened. And the car, out of nothing, started spininng again. 3 times, and I hit the opposite wall of the highway. By another miracle, nobody hit me (rush hour, I-95 north, 820am)
My wife and I recently drove cross country in our Tesla Y. Occasionally, we had the forward collision warning go off for no reason. More importantly, when we had the adaptive cruise control on and particularly when the autopilot system on, we experienced episodes of sudden braking when there were no other vehicles or other objects in the vicinity. This happened, on average, once or twice every 30 minutes. It got to the point where we had to keep the cruise control off because it seemed to put us in potentially dangerous situations. The "Traffic Aware Cruise Control" and "Auto Steer". Yes, available for inspection. During driving on a long highway trip with these features turned on, the car suddenly braked severely for no reason(no objects or other vehicles were in front of car). This caused a large truck, which was following me to also brake and nearly collided with my car's rear. Repeat experiences like this "phantom braking" occurred. Based on other reports, this is common in other Teslas. No warning alerts in car.
While driving on freeway on a clear day with no other cars in front of me the car suddenly hard brakes such that one could feel the anti locking activating. I was lucky there were no cars behind me or it would have been vary dangerous. There was a car on my right but it was holding its lane and there was no visible reason for emergency braking.
When on cruise control, the vehicle frequently brakes hard at highway speeds, jerking our bodies and forcing cars behind us to brake hard as well. Sometimes this happens when we’re beside a semi truck or a car changes lanes far in front of us, but often there’s nothing. There is no warning prior to the braking.
I started a drive from my house. I entered a destination into the NAV system, it plotted a route. I then proceeded to start my drive. When I tried to backup, the backup cameras did not display any video, I waited over 1 minute and there was still no video. I then proceeded with my drive. The car would not start the cruise control, autopilot or FSD functions. Whenever I tried to engage the systems the car would state that they are unavailable. At this time, I also noticed that the NAV system was not updating the display, it continued to display the same image that showed that I was at home in the driveway. Adaptive cruise control has failed many times. Recently had adaptive cruise control set at 75 mph on interstate. With no warning, the car instantly slowed to 55 mph. If a car would have been behind me I would have been rear-ended. Or worse, if someone behind me thought I was intentionally braking hard or "squating" they could become angry and instigate a road rage incident. The car provided zero warning before it braked. The car has not been inspected yet. Yes, the car is available for inspection.

Enroute, we experienced a number of unneeded emergency braking events while on the highways at speed (65-72 mph). While this happened about a dozen time, 5-6 of them were significant slow downs and could have caused us to be rear ended. In none of these events was there any reason for the emergency braking. One happened when another car was approaching from the rear quickly to pass. These events led us to stop using the cruise control entirely for the last 2 hours of our trip home. I have not yet contacted Tesla about this which I will be doing right after I submit this report. My Tesla model Y has been braking for no apparent reason when using the adaptive cruise control. Sometimes there may be a car coming in the opposite direction, but most times there is no other cars within miles. Several times the braking has been extremely aggressive causing me to have to hit the accelerator hard to avoid somebody behind me running into me. This has happened many times on long,straight, flat roads in the middle of the Arizona desert.
The vehicle brakes for no apparent reason without any other vehicles in near vicinity, both in adaptive cruse control and autopilot. This has improved with updates, but on a recent trip we experience six phantom breaking events on interstate travel.
Tesla's Traffic-Aware Cruise Control system has malfunctioned on my vehicle several times. It will often brake hard at high speeds despite there being no vehicle or object in front of me.
