I was operating my 2023 Tesla Model Y Long Range AWD with Full Self-Driving (FSD) engaged at low speed in a residential neighborhood. The vehicle took an incorrect turn into a neighboring driveway instead of continuing on the roadway. I attempted to correct the vehicle’s path. At that point, the vehicle unexpectedly accelerated. I experienced a sudden loss of control and was unable to stop the vehicle before it struck my garage door and entered the garage, causing structural damage to the garage wall. Notably, automatic emergency braking did not activate, and no effective front collision warning intervened, despite an imminent collision with a fixed structure. On Sunday evening (12/21) at approximately 6:30 pm, I enabled Self‑Driving while my car was parked in my garage with the garage door open. The vehicle was positioned facing forward. Immediately after activating Self‑Driving, the car began reversing rapidly in an apparent attempt to exit the garage. During this unexpected movement, the passenger‑side mirror struck the garage door trim and cracked. I quickly applied the brakes and disengaged Self‑Driving to stop the vehicle. I was driving 2023 Tesla Model -Y (VIN [XXX] ) with the latest FSD software with FSD and navigation on. As I approached a stoplight it was unclear whether the car would make the appropriate turn at the intersection, so I pushed up on the right stalk to disengage FSD and inadvertently touched the brake pedal before FSD had disengaged in response to the stalk movement. Touching the brake apparently disengaged FSD, so when I pushed up on the stalk it put the car in reverse. There was no visual or audible warning (or at least none that I saw or heard), and when I stepped on the accelerator the car went in reverse. Fortunately, I did not hit the car behind me. Currently on the newest Tesla Full Self Driving software v14.1.4, it is phantom breaking as well as phantom swearing out of nowhere. It is swerving and using the breaks so hard it actually affects the steering wheel and self disengages it's self (without the big-loud safety alert to immediately take over control), it just does the soft disengage audio notification, like if you wanted to cancel FSD yourself. The serious problem is that the car does not stop like it should, instead it just continues it seems like it just neutrally rolls on its own drive and twice in the last week it would swerve directly into oncoming traffic. Suddenly my car took a spin, i lost control of the steering. We noticed that in the morning, and parked the car inside the garage, and kept the door and windows open so it could dry quickly. Control arm bolts failure. As my wife was driving from a stop light making a right turn at below 10 mph she heard a loud bang. Both of the bolts connecting the control arm had fallen out onto the ground causing the wheel to fail and push back into the wheel arch. The problem has been reported to the dealer but from research and conversations with them this is fairly common. Should that have happened on a freeway it could have been catastrophic.

I solved it myself and when I got home, I called Roadside service for help re-programing the key card that failed. The person told me to call Tesla on Monday about the inability of my other drivers to unlock my car. I did that yesterday, and I told him I thought it was something wrong with my car, and suspected I needed to take it in for service. He said no, it wasn't the car but that I needed Premium Connectivity ($10/mo) in order for my drivers to be able to unlock my car. My car is equipped with autopilot feature which I am unable to use due to safety score system designed by Tesla which is used for calculating insurance premiums. This safety score system doesn’t count the miles driven on autopilot and only counts the miles driven without autopilot to decide safety score which impacts insurance premium. This policy forces consumers to drive car without autopilot to increase number of miles driven to compensate for any mistakes made during driving without autopilot. This not only compromises safety of the vehicle and passengers but also deprives me to use features of my car that I paid for. Since autopilot is a supervised driving and requires driver active engagement the miles driven using autopilot should also be counted to decide safety score. Heat pump tends to fail when temperatures reach out temperatures below 0ºF. This is a safety risk as at these temperatures, a failure to the vehicle heat pump could lead to death. There was one Zoox ahead of me and one behind in the left lane and I tried to merge in between with blinkers on. The Zoox car behind sped up to close the gap to block me out. After the stop light turned green zi tried to merge again before my lane ended and the driver did the same thing by closing the cap and almost hitting the Zoox ahead of him. Tesla Model Y 2023 running on Enhanced AutoPilot (software up to date as of Nov. 19, 2024) on a highway in the rush hour when traffic is moving at the speed of 5 - 30 mph, failed to identify the trailer in the front and sped up. I had to take over control, apply brakes to avoid the rear-ending the trailer in front of me. Tesla was speeding up as if the road was open. In the computer display it does not show any traffic in front of the car. I was driving down I90 with no one in front of me and a clear road. Unexpectedly, the right side passenger airbags exploded and struck me in the face and shoulder. The car immediately braked and I saw a semi truck pass me on the right with pieces of what appeared to be tire debris flying in the air. I pulled over and my car become inoperable. I inspected the outside of the vehicle and there was no damage or collision at all yet the passenger side airbags went off.

Regenerative braking is a huge factor when it comes down to hydroplaning. Hydroplaning and braking do not go well together. Generally in a normal gasoline vehicle, when in a hydroplaning situation where you feel the vehicle is beginning to slide, you would let go of the throttle (gas pedal), and correct the steering. Upon doing that, regenerative braking (which comes from letting go of the gas pedal), caused the vehicle to completely go sideways and no chance for me to correct the steering. This situation could have been avoided if the vehicle did not automatically brake on it's own when I let go of the throttle, in this situation.

In May 2023, after I took my car from the Tesla service center in Santa Ana, I realized there was an issue of shaking and shredding from the motor, and it got worse when I pressed the brakes. That happened when I drove the car down a steep road from [XXX] down to [XXX] in the city of Laguna Beach; Then I drove the car to a different road from [XXX] , then down from [XXX] to [XXX] and [XXX] , where it is down steep as well. Then the same issue showed up, and I couldn't keep pressing break because the shaking got worse, so I let the car roll with shaking till it reached the straight road and stopped.
