The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake application, the vehicle did not decelerate as expected and continued to move forward. I also attempted to stop the vehicle by pressing the Park button, but this action did not stop the vehicle. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. My car is equipped with autopilot feature which I am unable to use due to safety score system designed by Tesla which is used for calculating insurance premiums. This safety score system doesn’t count the miles driven on autopilot and only counts the miles driven without autopilot to decide safety score which impacts insurance premium. This policy forces consumers to drive car without autopilot to increase number of miles driven to compensate for any mistakes made during driving without autopilot. This not only compromises safety of the vehicle and passengers but also deprives me to use features of my car that I paid for. While driving on the highway at 75mph, with "Traffic Aware Cruise Control" (aka, "Adaptive Cruise Control") enabled, a white napkin had blown into the lane ahead. This caused the car to slam on the brakes, and rapidly reduce speed to 20 mph, before I was able to re-accelerate back up to speed. I consider this a dangerous malfunction of the Adaptive Cruise Control system (and possibly the Automatic Emergency Braking system as well) on a high speed lane on a highway, which easily could have resulted in a high speed rear end collision and injury. (Fortunately, there wasn't any traffic close behind in the the lane.)

Ever since the first few miles on the car, the forward collision warning (FCW) system triggers all the time for no reason. I cannot drive the car without it triggering, and I use the car almost daily. It constantly thinks I am going to run into the car in front of me when coming to a stop at a light, for example, however I am never anywhere near running into the car in front of me. It also triggers when making a turn if there is a car parked on the curb. It thinks I am tracking straight, however my wheel is turned and I am simple following the curve of the road. These constant alerts cause me and my guests panic, and occasionally cause me to jerk the wheel in an unsafe manner for, again, no reason. Its embarrassing. This feature does not work as intended whatsoever.

Tesla recently rolled out an update that gives everyone in the US a trial of FSD. After this update I've had numerous situations where the car would panic and act like I was about to collide with a parked vehicle both while driving down a residential street in daytime and when coming around corners. This never happened in previous version. Also the auto windshield wipers no longer work when its raining but will turn on when its dry and cause dirt to smear on the window, mainly when the sun is facing the driver which makes it very dangerous. The last issue is pretty big, the car has been exhibiting severe phantom braking when on the highway using adaptive cruise control (FSD I guess now?)

I was driving my normal route utilizing the autosteer functionality of the Tesla that I purchased and it suddenly jerked and crashed into a parked vehicle, which resulted in a total loss of my vehicle by the insurance company. Thank god nobody was injured during the accident but it was scary, dangerous and a major inconvenience. Once the accident happened (on 12/6/2023), I submitted this claim over the phone on December 7th 2023 with Tesla and Geico, my insurance company. I spoke with the Tesla representative for over an hour giving details of the accident and have yet to hear back. The vehicle was on self driving mode and about to get into a near fatal crash. I had immediately intervened and avoided the crash. This caused damaged to front passenger side tire and wheel apart from mental trauma to myself and for my family . I contacted Tesla asap and informed about the incident. Tesla responded back two days after and asked me to pay for tire and wheel replacement which caused by Tesla’s fault. Earlier, for the same kind of incident, i visited Tesla’s service center so that they can fix the faulty full self drive system. Forward Collision Warning (FCW) throws false positive alerts every time. The sudden false warning notification takes the attention of the driver away from road even though there is no reason for the driver to be alerted. This is a safety hazard as driver can make unnecessary mistake due to the false warning chime. FCW throws warning chime even when there is no vehicle ahead, or there is a vehicle coming in from the opposite lane or when the vehicle ahead takes a turn etc. Tesla insurance is also monetising a faulty beta version of FCW (released to all users) by penalising driver with false warnings in real time driving analysis and thus increasing insurance premiums.
