2023 TESLA MODEL Y Forward Collision Avoidance Warnings Problems

While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake application, the vehicle did not decelerate as expected and continued to move forward. While driving a Tesla Model Y on a freeway in Arkansas, the vehicle was operating in Autopilot mode (Tesla's driver-assistance system). The vehicle in front of us came to a sudden stop, but my Tesla failed to issue any alerts or activate automatic braking. When we attempted to brake manually, the car did not allow full braking and continued moving forward, ultimately causing a collision. As a result, I’ve suffered significant financial losses due to the vehicle being totaled and damage caused to other vehicles/other persons injuries involved. My car is equipped with autopilot feature which I am unable to use due to safety score system designed by Tesla which is used for calculating insurance premiums. This safety score system doesn’t count the miles driven on autopilot and only counts the miles driven without autopilot to decide safety score which impacts insurance premium. This policy forces consumers to drive car without autopilot to increase number of miles driven to compensate for any mistakes made during driving without autopilot. This not only compromises safety of the vehicle and passengers but also deprives me to use features of my car that I paid for. Since autopilot is a supervised driving and requires driver active engagement the miles driven using autopilot should also be counted to decide safety score. Harsh braking while using cruise control. Known as "PHANTOM BRAKING". This is causing extremely dangerous situations. The vehicle slams on the brakes while at high speeds for no apparent reason. Perfect weather and road identification markers. Happens with oncoming vehicles in normal path of travel as well as when there is a break in painted lines on the roadway. This creates a serious potential for an accident injury or death. Happens consistently.

I [XXX] was driving at around 50mph with 100% visibility and in few seconds I got in to fog ( see pic shows clear condition ). First time when it happened, i was able to control and It went to back to normal. I did not pay attention after. Second time I took right turn and the car drove in left lane but this time I felt steering was rigid and was unable to control the Steering and hit a tree. Airbags (any) failed to deploy in a hard impact t-bone collision with another vehicle, endangering occupants. Vehicle is being inspected by insurer.

Tesla met with an head on collision with another Toyota. Tesla’s airbags didn’t deploy and i had my 6 year old in car as well. The other car which was 2007 toyota, its airbag still worked.

While driving on the highway at 75mph, with "Traffic Aware Cruise Control" (aka, "Adaptive Cruise Control") enabled, a white napkin had blown into the lane ahead. This caused the car to slam on the brakes, and rapidly reduce speed to 20 mph, before I was able to re-accelerate back up to speed. I consider this a dangerous malfunction of the Adaptive Cruise Control system (and possibly the Automatic Emergency Braking system as well) on a high speed lane on a highway, which easily could have resulted in a high speed rear end collision and injury. (Fortunately, there wasn't any traffic close behind in the the lane.)

Ever since the first few miles on the car, the forward collision warning (FCW) system triggers all the time for no reason. I cannot drive the car without it triggering, and I use the car almost daily. It constantly thinks I am going to run into the car in front of me when coming to a stop at a light, for example, however I am never anywhere near running into the car in front of me. It also triggers when making a turn if there is a car parked on the curb. It thinks I am tracking straight, however my wheel is turned and I am simple following the curve of the road. These constant alerts cause me and my guests panic, and occasionally cause me to jerk the wheel in an unsafe manner for, again, no reason. Its embarrassing. This feature does not work as intended whatsoever.

Tesla recently rolled out an update that gives everyone in the US a trial of FSD. After this update I've had numerous situations where the car would panic and act like I was about to collide with a parked vehicle both while driving down a residential street in daytime and when coming around corners. This never happened in previous version. Also the auto windshield wipers no longer work when its raining but will turn on when its dry and cause dirt to smear on the window, mainly when the sun is facing the driver which makes it very dangerous. The last issue is pretty big, the car has been exhibiting severe phantom braking when on the highway using adaptive cruise control (FSD I guess now?)

When on Autopilot, the car engages in "ghost braking" for no valid reason. This has happened a couple of time. The last incidence happened today, 02/16/2024, at about 10:38 a.m. while on a freeway. I was driving my normal route utilizing the autosteer functionality of the Tesla that I purchased and it suddenly jerked and crashed into a parked vehicle, which resulted in a total loss of my vehicle by the insurance company. Thank god nobody was injured during the accident but it was scary, dangerous and a major inconvenience. Once the accident happened (on 12/6/2023), I submitted this claim over the phone on December 7th 2023 with Tesla and Geico, my insurance company. I spoke with the Tesla representative for over an hour giving details of the accident and have yet to hear back. My insurance premium has gone up significantly and I need Tesla to agree that it was not the driver but the software's mistake that caused the accident.

I received a false forward collision warning on the intersection of Alder Woods Dr. and Sweat Leaf Te. intersection for a parked that is legally parked on the street. Tesla's forward collision mistook the parked car, for a moving car. Driving conditions were clear with no snow, no ice or rain. None of the car cameras were blocked either. I took a picture or the street and the car that is parked on the street.

While driving our new Tesla MYLR in heavy rain, with Traffic Aware Cruise Control (TACC) on, we experienced two Front Collision Warnings (FCWs) in quick succession, causing the car to massively reduce speed, causing a car behind us to nearly hit us. I believe this is called "phantom braking" in the common parlance. I believe this is also an issue with the wiper system. We were driving about 55mph on a two lane road. False Forward Collision Warning took place on Jan 4, 2024 at 6:40pm in Herndon, VA while I was driving on Reston Pkwy: I was driving around 30mph on the rightmost lane, Tesla issued a false forward collision warning for a car that was in front of me, right before Dulles Access Rd exit. Driving conditions were clear with no snow, ice or rain. None of the cameras were blocked either, there was no daylight. False warnings (both sound, and red screen flash) created a safety risk by startling me, distracting me and creating unnecessary confusion.

I was driving in left lane of highway at night with cruise control engaged, traveling 70mph. Autopilot was NOT engaged - only the Traffic-Aware Cruise Control. I turned on my right turn signal to change lanes. At the same time a car came up behind me in the left lane and was tailgating me. I am reporting a false/phantom Tesla forward collision warning took place on Dec 13, 2023 at midday at 12:22 PM ET, clear road conditions with no rain, snow or ice, clear visibility without any fog, while I was driving on Alder Woods Dr. at Fairfax, VA. Tesla's forward collision warning system issued a warning for a legally parked car on the street which didn't pose any safety threat. The system mistaken the parked car with a moving car on the road and issued a warning which caused a lot of anxiety and distraction on me and my passengers traveling with me in the car. I passed the parked car without any problem, so did many other drivers ahead and behind me. This incident clearly shows that Tesla's forward collision system is not accurate and it is creating a lot of problems.

The vehicle’ Full Self driving control malfunctioned two times. This vehicle delivered to me on Aug 31, 2023. The recent full self driving malfunctioned on Dec, 16th, 2023. The vehicle was on self driving mode and about to get into a near fatal crash. I had immediately intervened and avoided the crash. This caused damaged to front passenger side tire and wheel apart from mental trauma to myself and for my family . I contacted Tesla asap and informed about the incident. Tesla responded back two days after and asked me to pay for tire and wheel replacement which caused by Tesla’s fault. Earlier, for the same kind of incident, i visited Tesla’s service center so that they can fix the faulty full self drive system. The Tesla service center kept my vehicle for ten days and took no action to fix the issues.

This has only happened in our 2023 model Y. We also own a 2016 & 2017 Model X and we never had the car applied the brakes while in auto pilot and nothing is in front of the car for at least a mile. We are worry about being rear ended because of the sudden braking to a almost stop. My car randomly brakes while using autopilot/cruise control on the highway. It will brake and move the car toward one side of the line. This happens sometimes every 20 minutes on a roadtrip. On one occasion, it threw the car to the far left side of the line and then reacted to the on the left. It threw the car back to the right side of the lane, near the car on the right side. Tesla forward collision system activated inaccurately twice consecutively at very low speed: on October 28, 2023 at 1:09PM ET on US-50 E. 1. Traveling on the left lane US-50 E at 1:09pm, for a slowed done traffic, around 10mph Tesla issued a false forward collision for the car traveling ahead of me with plenty of space between cars. 2. Right after this incident, Tesla falsely had another false forward collision warning issued on US-50 E at 1:09 pm for the same conditions that I described above (very slow traffic 10mph, left lane, for the same car, plenty of space between cars) In both of these incidents due to Tesla's false forward collision warning a great deal of confusion, and distraction is created by the very same system which is supposed to protect drivers and its passengers.

Two times in one week with two different drivers driving the same vehicle the vehicle experienced sudden unintended acceleration while parking. On August 12, 2023, I went to the Plano TX Tesla Delivery Center to pick up my new TeslaY model as scheduled at about 11 am. CST . The vehicle had 3 miles on it. This faulty and false forward collision warning incident took place on US Route 50 from Arlington to Fairfax direction, in slowed down traffic for a vehicle that is in front of me having plenty safe distance on September 27, 2023 at 2:27 PM. Notice that there is no hard breaking recorded by the car right after this incident.

False and faulty forward warning collision incident took place on July 5, 2023 at Alder Woods Drive. This happened for a legally parked card on the street, the system labeled the car moving car infront as opposed to a parked and stopped car on the side of the street near the curbside. As a result it issued a false and faulty warning.

Brand new vehicle, the vehicle cameras do not seem to be conveying the correct information to the control systems resulting in all related safety features like forward collision, parking assist, lane departure, automatic wipers, adaptive cruise control, blind spot warning, green light chime, etc are not working. In the last few months these features have not been operational for more than a few days. This is a major safety flaw with this vehicle and the local dealer is unable or unwilling to help.

I was pulling up to a ticket machine in a parking garage. I had my foot on the accelerator and was using regenerative braking and was essentially stopped. I was short a few inches from the button to get the ticket so applied almost no pressure to the accelerator – more like a tap so I could go forward a few inches. Forward Collision Warning (FCW) throws false positive alerts every time. The sudden false warning notification takes the attention of the driver away from road even though there is no reason for the driver to be alerted. This is a safety hazard as driver can make unnecessary mistake due to the false warning chime. FCW throws warning chime even when there is no vehicle ahead, or there is a vehicle coming in from the opposite lane or when the vehicle ahead takes a turn etc. Tesla insurance is also monetising a faulty beta version of FCW (released to all users) by penalising driver with false warnings in real time driving analysis and thus increasing insurance premiums.
