I recently purchased this vehicle less than a month ago and i have more than 15 Forward Warning collisions that come from nothing. Vehicles will be 100 yards out and my tesla thinks i am about to collide prompting the forward warning signal. I am driving passed the mailman and it thinks i will hit, my neighbors cars parked on the side of the road also prompts the message. This has caused my tesla insurance to rocket 30% more. I reached out to tesla, the car had a problem with the cameras pitch, alignment, and angle. However, they will not adjust my insurance despite the service center agreeing that the issue was with the vehicle.
Problem: unintended acceleration On 05/30/23, the Tesla Model Y was parked in a parking spot, southbound facing a building. The car suddenly began to drive forward on its own and struck the curb, an AC unit, and a business office wall. No injuries, but the driver and people in the building (a dental clinic) panicked.
I was using autopilot on the car. A truck with a long truck bed was in front of me. I was getting really close to the truck bed and realize the my Tesla Model Y was not identifying the truck bed. There were no collision warning so I braked 3-4 feet away from the truck bed when I saw that the car was not braking on its own. I submit some pictures that shows the Tesla identifying that the truck is a car that is further away.
To whom it may concern I was driving in a 2023 Model Y around 7:30 in the evening at less than 30mph. There was light rain in the area. As I was driving down a street in the metro Boston area, I noticed the car ahead of me had slowed and was stopping behind another car at a crosswalk. I had taken my foot off the accelerator and noticed that the car did not slow down at all and the regenerative braking did not function properly. Even as I attempted to brake, it seemed like the car would not stop in time. While driving down I-5 South between Bay Area and LA, the vehicle applied brakes and sounded a collision warning when there was no apparent vehicle/obstruction in front of the car. Tesla's Traffic Aware Cruise Control (TACC) was engaged at the time and the max speed was set to 75MPH. The speed reduced to ~60MPH before I intervened and pushed on the accelerator pedal.
I was driving using "auto-pilot" for speed control while going 70mph my vehicle randomly braked hard for no reason (maybe a cloud shadow) causing the vehicle behind me to swerve into the shoulder. I just bought the car 3 weeks ago and am very disappointed that the vehicle doesn't come standard with cruise control that won't cause accidents. This is a recurring issue with using autopilot in my vehicle. I don't feel safe and will be contacting tesla next. Although I doubt they will be of much help.
On every trip in my 2023 Model Y there is phantom braking for no reason with no cars in front of or around me. It happens on the highway on perfectly clear days, with no hills, no signs, no shadows or bridges, with no cars in front of me or behind me on completely straight roads in addition to happening when there are other vehicles around. Tesla's forward collision system gave another false warning: on June 15, 2023 at 10:00am on Reston Pkwy at Reston, VA while I was traveling around 25mph for a car in front of me, with more than plenty of space between cars. This false warning caused a lot of confusion on my side, I tried to figure out what was wrong, the false warning itself distracted from the road. I can see the Tesla's false warning causing accidents in some cases. This needs immediate attention.
My car keeps giving false alarm which itself causes a lot of confusion and safety hazards. This incident took place on June 13, 2023 at 6:13pm on Rte 602/Reston Highway at Reston, VA. While I was driving on the right lane slowly, Tesla fired a false alarm for the car traveling in front of me, at that time my speed was about 15mph and there was plenty space between the cars. I filed bug report with the manufacturer.
I am filing the following two incidents regarding false and faulty warnings of the forward collision system. My car's forward collision warning system, gave another false warning: 1. May 31, 2023 at 9:04am 2. While driving on Rte. 50 at Fairfax, VA 3. I had more then enough safe distance between my car and the car in front of me The forward collision system provided sound, and visual alarms for the unwarranted situation that I described above. This cause a great deal of confusion, shock and distracted me from the road.
Driving directly north on SR281, about an hour before sunset. Perfectly clear dry sunny day. No traffic for at least 500ft in front of me. A car immediately behind me (about 2 car lengths).. I have my car serviced 4(four) times, most recently on May 26, 2023 at Tesla Sterling, VA Service Center. While traveling with family on May 27, 2023, we had three false warning incidents, each caused serious confusion and distress: 1. On May 27, at 12:47pm on Alder Woods Dr. at Fairfax VA - my car issued a false warning for a legally parked car on the street. 2. On May 27, at 6:51pm on I-495South - my car issued another false alarm for a slow downed traffic, for the car travelling in front of me - going well below the speed limit, with a lot of space between two cars. 3. This incident took place on May 26, 2023 at 3:43pm on Rt 608 (West Ox Rd) at Fairfax, VA. I had another false alarm warning for a car traveling in front of me with below the speed limit and having more than plenty space between us. My car put a false warning with extreme sounds and visuals, as a result I had to take off my eyes from the road and look at the screen to see what may be going on. I was distracted due to this warning, and I had to 'hard brake' as a result. Tesla's false forward collision warning almost caused an accident as a result. False forward collision took place on May 24, 2023 at 6:18pm on Rt. 50 Arlington, VA in the following form: I was driving on my way home, while driving around 35mph (limit is 45mph), a car in front of was taking the left lane dedicated to turn left, which slowed down. I was going slow (35mph) and there was more than enough space between me and the car in front of me. At this point Tesla's forward collision gave me another false warning.
Incidents of phantom braking: We took our brand new Tesla model Y on a road-trip from Saint Helena CA to Scottsdale AZ and back. We had several incidents of the car slamming on the brakes while in cruise control due to mistakes in recognizing an actual reason to brake. May 11, in Arizona on highway 60, single lane road, we were in the right lane heading east when a large white pickup approached in the left lane. Our car mistook the approaching vehicle as a potential collision and slammed the brakes. This occurred a second time with another approaching vehicle at which point we ceased using cruise control. May 17 return trip on highway 10 in the morning, as I was driving with cruise control on, two lane highway, I approached a swale in the road with a change in pavement color. This incident took place on May 20, 2023 at Judicial Dr, Fairfax VA at 1:12pm. I was driving with my family, I changed from left lane to the right lane and the forward collision warning went off for no good reason: I didn't have any cars in front of me, or at my right hand side. Forward collision warning is producing false/phantom warnings.

My vehicle has issues correctly identifying forward collisions. There are moments the car produces "phantom forward collision warnings" of objects or people not in the same lane (such as a pedestrian on a sidewalk or a car far outside of a safe stopping distance. However, there are other instances where I would expect a forward collision warning and receive none at all. This is exasperated by environmental factors and road conditions. "Tesla Vision" with camera-only detection seems to be highly unreliable based on the environmental factors (such as amount of daylight) or road conditions (speed bumps, construction areas, etc).
This incident, which is a false forward collision warning, took place on May 10, 2023 at 8:56am ET on Route 50 at Arlington, VA: 1. I was travelling on Rt. 50 at the left lane 2. Due to morning traffic jam, I, and all the other cars, were moving way below the maximum speed limit 3. On my right, there was another car traveling below the speed line with left signal on 4. The Model Y with AutoPilot Plus and Autopark enabled lurched forward and crashed into a wall during parking. Just as important a concern is that the car never made a beep, chirp or any other indication or warning that it ever even realized it was driving into an obstacle. We are certain the collision would not have happened had Tesla not removed ultrasonic sensors from the car because the driver has driven many teslas and even owned a tesla before but all had USS sensors in them.
I was going to park in front of the store at the shopping mall. before I reached the parking spot car was not slowing down automatically after releasing the acceleration pedal. I decided to use the brake (which I rarely use because the car stops automatically when you release acceleration pedal) and I really don't remember how everything happened and I might accidentally step on the acceleration pedal instead of brake, but the car was not able to detect any obstacles or objects and I ended up braking heavy metal bench in front of the store and broke into the store. I ended up smashing entire front window and drove into the store as deep as 1.5 feet breaking the liquor boxes. After this heavy impact the airbags were not deployed. My concern is into two safety issue: Airbags not deploying and car safety sensors not functioning.
I had taken my car to the Tesla Service center twice in the recent past for the same problem: My car is engaging forward collision warnings for the legally parked cars on the street. I had more than several incidents of the same problem. It creates a big safety situation by showing a moving car in front of me, and indicating that there will be a forward collision. This creates a lot of stress while driving the car, and so much confusion so that I need to give my attention to my surrounding to see where the (false) problem may be. On May 4, 2023 at 11:51am, at 602/Reston Pkwy in the area of Reston,VA Tesla Forward Collision Warning falsely engaged for a moving car in front of me at a legal speed, and having more than abundant distance between two cars. I requested a Tesla service appointment for the third time regarding faulty forward collision warning. I've experienced "phantom braking" about 5 times in the last 3 months, where the vehicle automatically braked aggressively when on Autopilot without another nearby object in front of it. Some of the cases are re-producible, especially going West on Route 66 near coordinate 38.84779147045726, -77.42673470607882, where there are overlapping bridges above the highway. The aggressive braking of my vehicle caused vehicles behind me to swerve. Also, I've experienced false forward collision warnings multiple times. The most re-producible one is going down a little hill and its bumps near 38.88456462883165, -77.49153173786168 going west. The others happened when going around a circle or semicircle curves, with cars parked on the side or another car farther ahead in the circle.
This incident is related to the previous incident that I filed with NHTSA issue is 11514358 on March 28, 2023. I had my car serviced at Tesla service on April 14, 2023. I shared all the data and my complaint with the service center located at 22400 Davis Dr., Sterling, VA. I was told the issue is fixed. But, I had exactly the same problem: AI/Deep learning technology based Tesla Forward Collision System mistaken a 'parked on the street car' for a moving car, and it gives me crash warning, thinking that I will be crashing to a moving car. This incident happened at April 21, 2023 at 10:25am at Alder Woods Dr, at Fairfax, VA. I have other forward collision warning related mishaps, which I will be filing separately.
Around 3:54 PM EST, I was traveling on US 1 Route 1 North Bound, and I tried to turn right into Target Complex (4196 US-1, Monmouth Junction, NJ 08852) from Route 1. I slowed down to turn into the complex, and my vehicle had Sudden Unintended Acceleration while I am turning into a parking location. It jumped over the curbs and hit a tree. Airbags got deployed, and the car was damaged badly. Insurance deemed it a total loss. I saved videos from the crash from the USB. I can forward or upload them.

On March 25, 2023 on Alder Woods Dr., Fairfax VA Forward Collision Warned activated automatically for yet another parked car on the street. When forward collision warning activates the screen turns red, there are high sounding alarms inside the car, it distracts the passenger by creating an unsafe conditions for the wrong reasons.
On February, Friday 17th, 2023 around 10:15pm on 287 exit ramp towards rte 78 west , a truck with a flatbed suddenly went from right to left but the car failed to detect that and we almost collide into the back of it. Collision was avoided only by driver reaction. All collision warning systems failed to detect the situation. Still this is the same behavior we have seen since we bought the car. There is no awareness of close proximity objects. Tesla is acknowledging that critical ultrasound sensors were removed from the car.
Twice while driving in early afternoon on very bright days in the direction of the sun I've received forward collision alerts for no apparent reason. Due to adrenaline I don't remember if I slammed on my brakes or not, but I hope I did. Given that there was nothing in front of me for a substantial distance sudden braking for the alert would have been like a brake check had another vehicle been tailgating me. Date is for most recent occurrence, first occurrence was a few weeks ago.
When I purchased the car it was sold with certain safety features and those features are not available. It was also a surprise that the sensors were removed and Tesla Vision was the replacement. As of today Tesla Vision software update has not occurred and many features have been disabled. I cannot understand why Tesla would eliminate the sensors / radar and not have the upgrade to Tesla Vision tested and active in my new vehicle. Pretty much all of the Advanced Driver Assistance Systems (ADAS) you list above are not functional. I have written three letters to Tesla since I picked up the "Y" model and have not received a response. In those letters I requested the car be returned for a full refund because it was knowingly sold with these features missing!
Brand new 2023 Tesla Model Y LR phantom breaks on shadows. Issue happens with just auto speed Adaptive cruise control and the more advanced lane departure "Autopilot" feature on. Happens on 45mph roads and on highways at 65-75mph. Most serious instance was on US 540 in Apex NC where car hard braked when it was about to go under an overpass with a strong shadow. Sunny day, 10AM in the morning, no other cars near by. Would have likely caused an accident if someone had been behind me.
On January 13th, 2023 we were waiting for green light on an intersection, 2nd in place. The car in front of us started moving on green light for a left turn, and suddenly it slowed down as a result of another car coming in the opposite direction. Our Tesla Model Y didn't recognized the situation and crashed into it. The automatic emergency braking didn't actuate and also the Forward collision warning didn't alert. The low speed collision avoidance also failed to act. A police report was filed because of the accident. Previously, On January 6th, 2023, a service was requested because of the vision system is not properly detecting objects / situations that can result in collision. Tesla canceled the service appointment. After contacting the dealer on January 13th, they acknowledged verbally that the car is missing sensors that were previously installed in older models.
The car advertising having USS when place the pre-order. But when pick up the vehicle, USS is not there
Car applied brakes suddenly multiple times when no vehicles or objects were remotely close (i.e. the car had phantom braking). This occurred while auto-pilot mode was on as well as while cruise control was on and with auto-pilot off. This happened several times on wide open highway lanes including when no other vehicles or other objects were anywhere in sight.
When I purchased this vehicle, I was told that all ADAS features would be available once the cameras calibrated (approximately 50 miles). After 100 miles my wife bumped into a wall in our garage. I opened a ticket and Tesla said the ADAS features are unavailable until a software update is sent. The sales person who I spoke to were not aware of this. This is EXTREMELY dangerous. Drivers are reliant on warnings from features like this and to have the features off without a warning could get someone run over.
