While the media screen and rearview camera functioned, the Autopilot Board (APB) was "MIA." Tesla Randolph (Invoice #3000S0015357298) confirmed the internal failure and replaced the unit. Safety Risk: Failure disabled safety-critical ADAS: Automatic Emergency Braking (AEB) and Forward Collision Warning. Driving visualizations were severely compromised; the vehicle failed to detect pedestrians or lane markings while in motion, creating an immediate collision risk. The car’s active safety "intelligence" was essentially blind despite the camera feed being visible on the screen. Inspection & Contradictions: Tesla Service confirmed the "internal failure" (12/19-12/23). On [XXX], while using Full Self-Driving (FSD) to drive to a hospital in Oak Park, Illinois, the vehicle experienced sudden unintended acceleration during the disengagement of FSD. As the vehicle approached the hospital area, I initiated disengagement of FSD to manually enter the parking area. During—or immediately after—the disengagement, the vehicle suddenly accelerated at full power without warning, causing an immediate loss of control and a collision with a nearby fixed structure. No system warnings or alerts were issued prior to the event, and the acceleration was so abrupt that manual intervention was not possible. This occurred in a low-speed, street-level environment where no acceleration was intended or expected and does not correspond to any normal driver input. While driving at approximately 25 mph on a local street in Seattle, WA, my Tesla suddenly engaged the brakes without any apparent reason. The vehicle was not in Autopilot or Full Self-Driving mode, and my foot was actively pressing the accelerator pedal at the time. The braking was abrupt and forceful, causing a sudden deceleration. The weather was clear with good lighting. There were no obstacles, vehicles, or road hazards in my path that could have triggered the braking. The unexpected stop was shocking and could have led to a rear-end collision, as there were vehicles behind me.

On Friday 10/11 at 10:45 a.m I was in traffic on Day Street in Moreno Valley, California. This was the third time I experienced a sudden unintended acceleration with my Tesla. I would have hit the car in front of me, but I was able to swerve into an island on my left to avoid the car. The car went over the curve and stopped. The front triple camera glass was found to be dirty on the interior part of the glass. I had alerts of camera blocked/blinded and also I had several instances of emergency corrective maneuvers including braking, lane assists and forward collision warnings. Tesla cleaned the interior of part of the glass.
The FSD is not fully matured and I have tried multiple times and all times and the experience is weird and very hard braking and no control on parking lots. So not sure why this is called Fully self driving capability by Tesla? I have paid 12k for this capability in my car and unfortunately it's not fully functional. Two incidents: 1) Non-accident: My wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) Accident: Our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. I was performing a 3-point turn on a residential street. While reversing during the turn the Automatic Emergency Braking enabled and slammed on the brakes, bringing the car to a complete halt instantly. This caused my head to hit the headrest with enough force that I have headaches and brain fog the next day, indicating a concussion. I was 10 feet from the nearest parked vehicle when reversing; the AEB kicked in erroneously. This is not the first time that Tesla's AEB has erroneously and dangerously engaged while I was driving.
Autopilot will suddenly start to slow down on highway and freeway speeds with no warning, this has happened more than 4 times. The last time that this happened me and my partner almost got rear-ended.
Tesla is using camera-only detection for Forward Collision Warning system and this leads to numerous false positive alerts. This has happened from simply driving next to parked cars or slowly approaching vehicles at a traffic light. When these faulty alerts occur, a loud audible beeping is heard and red alerts are displayed on the screen. These faulty alerts are dangerous distractions, causing panic, fear, and uncertainty in the driver, causing them to take their attention off of the road while they assess what just occurred. This is a critical flaw in the functionality of Tesla vehicles that the company has refused to address.

The contact rented a 2022 Tesla Model Y. The contact stated that while driving 70 MPH with the Adaptive Cruise Control activated, the vehicle experienced Phantom Braking and abruptly decelerated. The contact stated that the failure had occurred twice during the trip; after de-activating the Adaptive Cruise Control, the vehicle operated normally. During the failure there were no other vehicles or objects nearby. The cause of the failure was not yet determined. The manufacturer, the local dealer, and the Rental Car Company were not yet notified of the failure. The failure mileage was 32,000. The VIN was not available.

"Phantom breaking" issue. When the vehicle is placed in cruise control the vehicle just randomly slams on the breaks; this typically happens when on the freeway/highway at speeds of 55 mph or greater. My wife and I have experienced this extremely dangerous issue from day 1 of taking possession of our Tesla over a year ago. I've reported it to Tesla and also spoken to the repair technicians that work on the Tesla vehicles. They're aware of the issue and haven't done anything to help or even offered to fix the issue. The technicians advised me that they are fully aware of the issue, and Tesla has no fix at this time. This is totally unacceptable and extremely dangerous. We've nearly gotten into a couple of major accidents because of this uncorrected issue.
I was driving south bound on hwy90 at about 78mph using navigate on autopilot. Without warning the car swerved into a left turn lane and then slammed on the brakes when the turn lane came to an end. This caused a number of personal belongings to be damaged. Thus happened as I was on my way home from a service center visit that stated the autopilot systems were all functioning correctly. When I drive a Tesla Model Y under Self-Driving mode at night inside YellowStone NP, there is another car on the opposite side with a high beam turned on, and just near that car there is a bison on the road, the Self-Driving computer fails to detect the bison in the beginning, and too late to do emergency break, the car hit the bison after passing that car. The bison died but driver is safe, the front right head light part and right repeater camera was missing. It's very hard to see the bison in the dark when the other car turns on the high beam, for both humans and cameras, the self-driving without radar or LiDAR is not able to detect the obstacle at the blind spot. Phantom breaking problem with Tesla Y occurred on I90 between Rapid City and Sioux Falls SD, June 2nd between 1 and 2 pm when my wife and I were traveling across country. The vehicle has suddenly applied automatic emergency braking on numerous occasions when there is no obstruction in the road or potential emergency situation. This often occurs multiple times per trip on longer trips (multiple hours of driving). It seems to happen regardless of whether Autopilot is activated or just cruise control. It seems much more common on hot, sunny days when highway mirages are present but it occasionally happens due to some shadow conditions. None of these situations have resulted in compromised safety but that might have been different if there had been a vehicles immediately behind us. The brakes are applied without warning and the vehicle decelerates rapidly.
There was an update last night on my Tesla model y. I've noticed two episodes of phantom breaking after this update this morning when I was driving to work. There was a truck in the next Lane but the car hit the brakes all of a sudden on the highway when the autopilot was on active. It was at a curve on the highway. I'm assuming the car thought that the truck was moving into my lane leading to sudden activation of brakes. This could have been dangerous. Fortunately there was nobody behind me and we were safe.
Many occurrences of phantom braking have occurred since I purchased the vehicle several months ago. I was driving on I-5 in California with the cruise control set at 70mph, then changed lanes from left lane to right lane with about 5-6 car links from the semi truck to the rear in the right lane. The emergency braking feature engaged with no vehicles in front of me in either lane for at least 500 yards. The semi truck locked up its breaks to avoid rear ending me. I applied the accelerator as quickly as possible and when I did the semi was approximately 3 feet from crashing into the rear of my vehicle.

While traveling at highway speeds from Colorado to Texas and from Texas back to Colorado my car continuously slammed on the brakes without warning while the autopilot(cruise control) was engaged. There was no visible evidence or object in common for each event. What made this dangerous is any untied objects in the car would shift forward and the vehicles behind me would have to slam on their brakes as well. On a few occasions it changed lanes with no cars in front of me.

The contact stated while driving 74 MPH with the cruise control activated, the vehicle experienced phantom braking and decelerated to 20 MPH before returning to normal operation. The contact stated that the failure recurred three times. The contact stated that moments later with the cruise control still activated, the vehicle seemed to enter self-driving mode independently, briefly taking control of the steering wheel. The contact stated that the vehicle jerked erratically as if attempting to avoid a crash however, there was no other vehicles or objects nearby. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 14,600.
While in simple autopilot mode (not full self driving) the vehicle had two episodes of phantom braking at freeway speeds of 80 mph. On one occasion it applied the brakes when a car passed me going in the same direction. On the other occasion there were no vehicles anywhere in the vicinity and it applied the brakes for no reason. I took the car to the Tesla service center and they said they are aware of the phantom braking issue but have no fix for it. They recommended I use the autopilot again and file a report at exactly the time it occurs again. So, basically risk getting killed so I can file a report for them for a problem they can't fix. When using Tesla's Cruise-Control, Traffic Aware Cruise Control or Enhanced Auto-Pilot the vehicle severely brakes without any reason. This happens every time the vehicle is taken on the highway. It is sometimes reproducible because it happens every single time at specific geographic locations. Most often it is not reproducible. On the highways it has put me in danger more than once. The vehicle would brake while going at 75mph and slow down rapidly all the way down to 45mph. While driving between Lincoln Nebraska and Denver using the driver assist system in my Tesla the car braked for no apparent reason. This occurred 3 times in my 8 hour drive. I would be driving at 80 miles an hour with nothing around on a sunny day and all of a sudden the vehicle would hard brake.
Adaptive cruise control with Auto Steer will without warning or noticable reason suddenly decelerate the vehicle. At highway speeds from 70 to sudden deceleration to 30 mph if the accelerator pedal isn't pushed immediately when it slows without warning. Sometimes with vehicles behind. There has been some close calls but no accidents.
Since I received this awful vehicle it has never properly identified emergency vehicles. Sometimes it confuses neon/led signs with emergency lights and smashes on the brake for emergency vehicles that aren't there. Other times it slams on the brakes on the highway when behind an emergency vehicle and the reflective tape is lit up from my own headlights bouncing on the awfully hard and cheap suspension on this 'performance' vehicle. Since the day I got it the Tesla hasn't been able to identify emergency vehicles with their lights on. I started video taping after like the 20th time because my dashcam cant show you the screen also. I did this strictly to display that the Tesla failed to identify a State Trooper with his lights on for several hundred feet with 'Full Self Driving' enabled. Just to be clear the car is on the current software update and has ALWAYS been up to date. For the last 8 months I've been reporting this FATAL error where my Tesla in FSD with location set will slow down all the way to 0mph in a 70mph zone on the expressway for a stop sign and round about under the highway that is not visible by ANY of the cameras the car is supposedly using to 'read the environment' well that's a load of crap because its stopping for non existent stop signs and driving 57mph in a 70mph zone and misses several speed signs in the video. Within the last week, while driving cross-country on Interstate highways at speeds of 65-75 MPH, car repeatedly (estimate 10-15 times over 1800 miles) experienced “phantom braking,” automatically engaging the brakes in the absence of any sort of hazard whatsoever (no cars or other objects nearby, fortunately; clear, sunny weather). This occurred when Full Self-Driving was engaged and also when cruise control without FSD was engaged.

While using adaptive cruise control on a highway about every 30 miles or so the car would suddenly slam on the brakes. The speed limits on these highways were between 65-75 miles per hour. There was not anything in the road or vehicles in front of our car. It was very scary and distracting for the driver and passengers. If a car or truck had been following closer to our vehicle we could have been in a bad accident. The day was sunny and clear so there wasn’t any weather to have caused these issues or cause the cameras to be blocked.

In a 1 minute clip I captured how the Tesla constantly misses collision warnings and/or assistance while all safety features are turned on because the car misses the first collision warning and then goes off early on the second scenario with the same car and driver in front of my passenger and I. It's dangerously inconsistent and this is the third passenger on Lyft this week this has happened to and the passenger verified as a witness to the event in each example without any sort of coercion. During my driving shift on Lyft my first ride was right next to me at the medical clinic when I accepted the ride but Lyft sent me 10 minutes away to park in the middle of a median facing a memorial for a car crash victim (while playing Stan by Eminem about a fan being killed in a car accident after no one gets his letters(On Tesla Radio which has been proven to be hackable and I have reported to NHTSA before)). She gives me directions; I accept the ride then drive 10 minutes back to where I accepted the ride in the first place.
