While Autopilot was engaged, the vehicle failed to slow down or properly navigate a curb near a roundabout. Despite our immediate manual intervention, the front wheel was damaged, resulting in loss of steering and a repair bill exceeding $7,000. After the incident, we were surprised and alarmed to learn that Tesla’s Autopilot is widely known to be unreliable and risky in urban settings. I was driving the car on supervised self drive and going home around 9 pm. When I reached my house I tapped the break to disengage the FSD and take over so that I can pull into our driveway. After coming to a stop the car all of a sudden just took off on its own at almost 60 mph on a residential street. I was repeatedly pressing on the brake to try to control the car. The issue I encountered was with Tesla's Smart Summon Feature. While using the feature, the car crossed the parking line and crashed directly into a car parked beside it. Happening so quickly I could not react in time. All of the cameras and sensors were in working order and it seems to have been a software related issue. The safety of anyone around the vehicle could have been at risk thankfully no one was hurt. The vehicle has since been inspected by insurance and damages to both cars were repaired. There were no warnings of any kind prior to the collision. I have a video from the on-board cameras of the entire incident.

Multiple, random occurrences of hard braking at highway speeds when there was no object or obstruction ahead. In the 2-1/2 years we’ve owned the car this has happened on two lane and multiple lane highways, generally at speeds over 50 mph. Also, the windshield wipers randomly deploy even when turned completely off. They generally work when it’s raining but the delay function is also randomly not in sync with the settings or actual rainfall.

The front triple camera glass was found to be dirty on the interior part of the glass. When I drive a Tesla Model Y under Self-Driving mode at night inside YellowStone NP, there is another car on the opposite side with a high beam turned on, and just near that car there is a bison on the road, the Self-Driving computer fails to detect the bison in the beginning, and too late to do emergency break, the car hit the bison after passing that car. The bison died but driver is safe, the front right head light part and right repeater camera was missing. It's very hard to see the bison in the dark when the other car turns on the high beam, for both humans and cameras, the self-driving without radar or LiDAR is not able to detect the obstacle at the blind spot. Phantom Braking. The most recent instances of phantom braking occurred on a recent trip to Lake Tahoe. The car phantom braked on the way there on the I-5 on August 5th in the afternoon. While driving at a normal speed and turning around at an intersection in our neighborhood, the car suddenly went out of control, causing the car to hit a fire hydrant in front of a home, smashed a tree before crashing into the siding of the home. Insurance deemed the car as "totaled". We were reimbursed by insurance so this is not about monetary losses, but a report to protect other drivers and their families. Ever since Tesla removed the radar units in the Model 3 and Y phantom breaking has gotten worse and worse. I have a 2022 Model Y Performance and every time I use the Basic Autopilot, which is glorified adaptive cruise control, it will slam on its breaks for no apparent reason. The traffic-aware cruise control triggers constant forward collision warnings on my commute, and the system highlights as threats vehicles that are parked on the side of the road safely out of the path of traffic. When driving on a two lane highway today, I had 3 episodes of the car applying the brakes to suddenly decrease speed when a vehicle was in the oncoming lane. I also had another event of emergency braking when a car was crossing the highway on a perpendicular road far ahead of my car. When I reached that road, the car had already safely crossed. When driving on a two lane highway (US 131 and US 115) at speeds of 55-70mph in clear weather with adaptive cruise control engaged over a 4 day period and covering ~500 miles, this car starts emergency braking (warning chime and sudden deceleration) EVERY time the car interacts with an oncoming tractor trailer going the opposite direction. It will sometimes do this for multiple, closely following cars coming the opposite direction. It will additionally slow down when overtaking tractor trailers on the expressway (I-75) traveling at speeds of 70-85mpg when passing on the left and rounding gradual curves.
