Just days after taking delivery of our first Tesla (2022 Model Y, second-hand ), we experienced a dangerous incident. While Autopilot was engaged, the vehicle failed to slow down or properly navigate a curb near a roundabout. Despite our immediate manual intervention, the front wheel was damaged, resulting in loss of steering and a repair bill exceeding $7,000. After the incident, we were surprised and alarmed to learn that Tesla’s Autopilot is widely known to be unreliable and risky in urban settings. I was driving the car on supervised self drive and going home around 9 pm. When I reached my house I tapped the break to disengage the FSD and take over so that I can pull into our driveway. After coming to a stop the car all of a sudden just took off on its own at almost 60 mph on a residential street. I was repeatedly pressing on the brake to try to control the car. But it seemed like the car had a mind of its own and was not responding to me at all. approximately 8 seconds (based on Tesla dashcam recordings) later after travelling a distance of approximately 700 feet the car hit 2 parked cars on the street, which eventually activated the brake and brought the car to a stop at the front yard of a house across the street from where the parked cars...

The issue I encountered was with Tesla's Smart Summon Feature. While using the feature, the car crossed the parking line and crashed directly into a car parked beside it. Happening so quickly I could not react in time. All of the cameras and sensors were in working order and it seems to have been a software related issue. The safety of anyone around the vehicle could have been at risk thankfully no one was hurt. The vehicle has since been inspected by insurance and damages to both cars were repaired. There were no warnings of any kind prior to the collision. I have a video from the on-board cameras of the entire incident.

Multiple, random occurrences of hard braking at highway speeds when there was no object or obstruction ahead. In the 2-1/2 years we’ve owned the car this has happened on two lane and multiple lane highways, generally at speeds over 50 mph. Also, the windshield wipers randomly deploy even when turned completely off. They generally work when it’s raining but the delay function is also randomly not in sync with the settings or actual rainfall.

The front triple camera glass was found to be dirty on the interior part of the glass. I had alerts of camera blocked/blinded and also I had several instances of emergency corrective maneuvers including braking, lane assists and forward collision warnings. Tesla cleaned the interior of part of the glass.

Tesla has failed to provide a software update to address both recalls, and has made it impossible for me to get support to have this rectified. They cancelled a service appointment I made and are no longer responding to my inquiries.

When I drive a Tesla Model Y under Self-Driving mode at night inside YellowStone NP, there is another car on the opposite side with a high beam turned on, and just near that car there is a bison on the road, the Self-Driving computer fails to detect the bison in the beginning, and too late to do emergency break, the car hit the bison after passing that car. The bison died but driver is safe, the front right head light part and right repeater camera was missing. It's very hard to see the bison in the dark when the other car turns on the high beam, for both humans and cameras, the self-driving without radar or LiDAR is not able to detect the obstacle at the blind spot. Phantom breaking problem with Tesla Y occurred on I90 between Rapid City and Sioux Falls SD, June 2nd between 1 and 2 pm when my wife and I were traveling across country. The proximity sensor alerts do not work with Tesla vision and resulted in a minor collision with adjacent vehicle while manually parking. With the removal of the ultrasonic sensors from the alert system, Tesla is not reliable anymore for distance warning and is responsible for accidents to to the false sense of confidence provided by that system

Auto acceleration(forward) caused while parking and crashed into fence and utilities. Requested TESLA to diagonalize the cause of auto acceleration. Tesla couldn't handle my request well and they canceled and delayed my request, finally, they rejected my request as the car was salvaged.

Adaptive cruise control with Auto Steer will without warning or noticable reason suddenly decelerate the vehicle. At highway speeds from 70 to sudden deceleration to 30 mph if the accelerator pedal isn't pushed immediately when it slows without warning. Sometimes with vehicles behind. There has been some close calls but no accidents.

In a 1 minute clip I captured how the Tesla constantly misses collision warnings and/or assistance while all safety features are turned on because the car misses the first collision warning and then goes off early on the second scenario with the same car and driver in front of my passenger and I. During my driving shift on Lyft my first ride was right next to me at the medical clinic when I accepted the ride but Lyft sent me 10 minutes away to park in the middle of a median facing a memorial for a car crash victim (while playing Stan by Eminem about a fan being killed in a car accident after no one gets his letters(On Tesla Radio which has been proven to be hackable and I have reported to NHTSA before)). Tesla was in "full self-driving” (FSD) mode in a quiet residential area. Vehicle chose to make a left turn on a less than perfectly symmetric T junction. The car was on the thru street at the top of the T. Street had a middle yellow turning zone with yellow lines on the outer side and dotted ones inside. Car did not stop after I parked and continued to go forward and hit another parked a car Even with my foot on the break. No warning light of any sor. This incident happened at 5:35 PM Pacific time on Monday, September 12. I made an appointment with the service department and it took the car there on September 23, 2022, . They only asked me about the time this happened and they said you’re going to send it to engineers and will get back to you. They did even Care to look at the car or inspect it. While on a cross country road trip, our 2022 Tesla Model Y has repeatedly slammed on the brakes at highway speeds with nothing in front of us. Most times have been cresting hills, but one time it was on straight road with, fortunately, no other cars near us.

Phantom Braking. The most recent instances of phantom braking occurred on a recent trip to Lake Tahoe. The car phantom braked on the way there on the I-5 on August 5th in the afternoon. I don’t have the exact time, the braking surprised us so much that we didn’t think to enter a “bug report”. The I-5 at that section is a 4 lane divided highway. I think the cause of the phantom braking may have been a bridge that had a slight rise, causing the cameras to think there was oncoming traffic. While driving at a normal speed and turning around at an intersection in our neighborhood, the car suddenly went out of control, causing the car to hit a fire hydrant in front of a home, smashed a tree before crashing into the siding of the home. Insurance deemed the car as "totaled". We were reimbursed by insurance so this is not about monetary losses, but a report to protect other drivers and their families. The Tesla database records mentioned user acceleration and error, but this doesn't sound like a reasonable cause. The airbags didn't open and neither did the automatic emergency braking or forward collision warning work. I was unable to brake and control the car to stop. Ever since Tesla removed the radar units in the Model 3 and Y phantom breaking has gotten worse and worse. I have a 2022 Model Y Performance and every time I use the Basic Autopilot, which is glorified adaptive cruise control, it will slam on its breaks for no apparent reason. It happens at night, in the day, and without any warning. I have almost been rear ended multiple times now. Tesla said they were working on a fix but that was over 6 months ago now and it's only getting worse. I am no longer comfortable using that feature with how unsafe it is.

The traffic-aware cruise control triggers constant forward collision warnings on my commute, and the system highlights as threats vehicles that are parked on the side of the road safely out of the path of traffic. The warning activates automatic emergency braking, which results in a collision hazard to vehicles behind me due to unexpected and unpredictable heavy braking. I stopped using the feature after the fifth such incident within the same approximate 7 mile trip. I have read about this safety issue previously and am aware that it is under investigation by the NHTSA, but did not realize it was this bad.

When driving on a two lane highway today, I had 3 episodes of the car applying the brakes to suddenly decrease speed when a vehicle was in the oncoming lane. I also had another event of emergency braking when a car was crossing the highway on a perpendicular road far ahead of my car. When I reached that road, the car had already safely crossed. Cruise control is unusable on the Tesla because of these events.

Tesla Y has a special setting for carwashes, that driver has to enable, which puts the car in neutral, closses all the windows and plus some more. Unfortunately system failed; car accelerated on its own and I hit a car in front of me, so it failed the second time and car didn't stop on its own to avoid collision. I called Tesla and this incident is under their investigation.

While driving from San Francisco to Salt lake city and back on highway 80 this past week, my vehicle frequently experienced “phantom braking” while using “autopilot”. At least 15 times on the drive, my car, while using “autopilot”, would forcefully apply the brakes when no hazard was present. Using autopilot (adaptive cruise control) on a two lane highway. Vehicle slammed on brakes when a semi truck was passing on the other lane. Happened three times on same trip. False braking was not consistent as it happened once as semi passed beside us, once when the other vehicle was at least 100 yards away and once again when another truck was about 25 yards away. Lane in front of us was clear with no traffic in front of us. This did not happen on divided highways. Only on two lane highways at speeds above 55mph. When the ADS is engaged you can not reliably drive the vehicle due to phantom braking events which puts us at risk of being rear ended at highway speeds. Has occurred on numerous dates.

Driving down the road on Cruise control and car will slam on the brakes and you almost get rear ended every time. Scary to say the least.

When driving on a two lane highway (US 131 and US 115) at speeds of 55-70mph in clear weather with adaptive cruise control engaged over a 4 day period and covering ~500 miles, this car starts emergency braking (warning chime and sudden deceleration) EVERY time the car interacts with an oncoming tractor trailer going the opposite direction. It will sometimes do this for multiple, closely following cars coming the opposite direction. It will additionally slow down when overtaking tractor trailers on the expressway (I-75) traveling at speeds of 70-85mpg when passing on the left and rounding gradual curves.
