Phantom braking that causes the vehicle to suddenly apply the brakes for no reason. Issue occurs at least once each time I use the Autopilot. Occurs on interstates as well as highways. In my 6 hour drive today it happened 5 times. Each time the vehicle decreases in speed by 15-30mph. This is extremely dangerous for vehicles following behind and for the passengers who are suddenly jolted.

My Tesla 3 suddenly braked while driving on the road from about 40 mph to 10 mph. There was nothing obstructing my car from the front or sides. It was a scary experience. My neck was jolted forward and the seatbelt pressed very tightly against my chest. I immediately scheduled for service with Tesla. I then got a call from them and was told I should be okay to drive my car until my scheduled appointment a week from now! How is this not an emergency?!

1) Ongoing issue with car braking suddenly when no other cars are ahead of it. 2) When in auto-pilot, the car brakes late when carrying speed. This has happened when the car is less than 1 car length behind the car ahead of it forcing me to disengage and brake hard to avoid a collision. Since I purchased this car new in Sept 2018, the car has had a "front Left safety restraint system fault". It has been fixed over 3 times and it is going for the 4th time now. The car is going out of warranty this year and I am afraid I will be stuck with the bill if it happens again. Even though all the fixes have been free so far, it is an ongoing problem Also, the known phantom breaking, just add to the numbers. it is more than common. I almost see it coming when going under overpasses because of the shadow. The computer must see it as an object. I have inquired about the lane departure warning and they keep saying it is just a software issue. If it is activated at any point during the drive, then it says that "lane departure warning" is not long available...

The optional "Self Driving Option" we purchased for an additional $8,000 when we purchased this vehicle has never worked properly. The adaptive cruise control will brake for no reason, often multiple times per mile. It seems to be affected by shadows on the road and the vehicle mistakes these shadows for obstacles. The "Self Driving Option" will abruptly jerk the steering wheel in the direction of oncoming traffic and violently disengage. My wife and I find it to be unusable. Multiple service requests to Tesla have resulted in no actions.

The recent full self driving update that was received approximately one week ago has resulted numerous, substantial, and dangerous Self driving errors. The Vehicle struggles greatly to understand lanes and has on multiple occasions suddenly and without warning jerked the wheel to completely change lanes and then re-jerking the wheel to change back into the Original Lane. There has also been a shocking increase in phantom breaking while driving at highway speeds which is unsafe because other nearby drivers may slam on the brakes. On June 1, 2022, we left on a roadtrip to CA from UT. Our Model 3 kept having incidents of "phantom braking," in which the Traffic-Aware Cruise Control and Autopilot systems would suddenly, inexplicably cause the car to slam on the brakes, and sometimes even flash a "red" steering wheel and an alarm with a warning that there was an object ahead —when there was NOT! This happened at least a dozen times en route on Highway 95 in Nevada, and continued to happen throughout our roadtrip on other highways until we returned home one week later on June 8, 2022. I was driving on a one way street when the car suddenly stopped. I was not at an or close to an intersection. There was a pedestrian waiting on the side of the road, standing between cars. He was not in my way. He was not even close to the vehicle. The car sounded audible warning and came to a complete stop. I believe this was the reason the vehicle stopped. This is the 2nd time since my 3 year ownership that this has happened. I provided Tesla with time stamps of the event during my service visit. They told me this is normal function of the vehicle. Our model 3 was purchased with Tesla’s “full self driving (FSD)” software installed in 2018. Throughout our entire ownership period, whenever FSD is active, there are specific locations known to us - mostly at certain points on freeways - where the car will “phantom brake,” in other words suddenly decelerate the vehicle very rapidly and require *immediate* driver intervention. For instance, there is a freeway interchange ramp that passes under a bridge and doglegs to the left. The car will always phantom brake about 200 feet before it reaches the bridge. I was using Enhanced Autopilot on Tesla Model 3. It breaks automatically in middle of road for no reason even when there is no object in front of it. Its a life threatening situation because the vehicle behind my car will not have enough breaking distance and time at high speeds. It happens almost every month and specially under the reflection of bridges. I complained to service center and employees just ask exact time when it occurred and if exact time is not provided than Tesla employees does not do anything to resolve the issue. I received access to Tesla’s Full Self-Driving system in late 2021. In the lead-up to that access, Tesla used a series of driver scoring metrics to select which users would get access first. Consistently, my metrics for forward collision warnings would show as alarmingly high, despite never coming within remote striking distance of another vehicle. On more than one occasion, a vehicle passing in other lane or turning left but in the other lane tripped the warning. At the time this registered as a red flag but more a bug of the evaluation system than the car’s recognition software. When driving on local roads and the interstate the card suddenly brakes for no reason when using auto pilot or full self driving. Sometimes it seems to be triggered by shadows from trees or other objects on the road and other times for no reason at all. I have been going 45 mph and the car suddenly braked to 30mph. I have been on the open interstate doing 70 mph when the car will suddenly and violently brake when there is a car on your bumper and this is very dangerous. If you dont keep your foot over the gas while driving and force the car to speed up it will cause an accident bc it will brake until the car is stopped probably. Multiple episodes of phantom breaking with the car on cruise control on the highway. The car was not on autopilot during the episodes. The car abruptly drops from highway speed with full braking until you can override it with the accelerator. This will cause accidents of someone is close behind you. Twice the car in Autopilot braked for no apparent reason. First time on an Interstate traveling at 65 mph in the center lane. As I was about to pass a truck located in the right lane that was going very slowly with its blinkers on, the Tesla abruptly decelerated from 65 mph to about 40 mph. The automatic emergency braking system activated when there was no collision imminent. (7:13am, 5/22/22) Neither cruise control nor autopilot was engaged at the time of incident. I was nearly hit from behind by a driver who did not expect me to stop as there was no hazard or other vehicle. Tesla inspected the vehicle and found no fault and claimed that I needed driver education. Also on a subsequent interstate trip with cruise control engaged, the automatic emergency braking system activated unnecessarily. It appears to be related to shadows on the road or pavement changes.

When driving on undivided highways, the car will phantom / panic brake due to oncoming cars approaching in the opposite lane. At times, the car phantom brakes for no apparent reason at all as there are no cars nearby. At night, it gets much worse, with the car phantom/panic braking at nearly every oncoming car. My 2018 Model 3 is equipped with radar-assisted Autopilot, but newer Tesla vehicles don't have the radar hardware at all and instead rely on a vision-only system that uses the cameras alone. When I opted into Tesla's Full Self Driving beta, the new software disabled my car's radar and my car became a "vision-only" vehicle, like newer Tesla vehicles. Driving while on cruise control, the car will phantom brake. That is, it will apply the brakes: - when a vehicle is way way out there in my lane and not a potential problem. - when two vehicle are close on each left and right side lanes, but not in my lane. For instance, when on 3 lane road with all lanes going the same way, vehicles on my left and right sides out in front of me but not in front of me, the car will suddenly apply the brakes. These are just the most common instances. I get lots on incidents of the vehicle unnecessarily applying the brakes when on cruise control.

I intermittently get an on-screen message “automatic emergency braking is disabled.” Adaptive cruise control and auto-steering are disabled and I am unable to engage either one. Randomly the problem will resolve itself with no intervention from me - usually a day or two later. I have taken the car to Tesla service twice and they are unable to resolve the issue. On the last visit the service technician said that it is a firmware issue.

My car is equipped with forward-facing radar, but ever since Tesla turned it off in favor of "Tesla Vision" my Autopilot/TACC experience has been greatly worsened. The car suddenly (and very unsettlingly) slams on the brakes on the highway or city traffic (I have FSD Beta) for anything from shadows, to hills/inclines, to curves, or minor harmless debris (shopping bags) in the road. This has not yet caused an accident but has almost come close. Other issues: - FSD Beta, when engaged, forces the computer to have control over the high-beams and windshield wipers. The car has repeatedly (but not often) initiated emergency braking while traveling on a freeway. It has done this in literally the same place on California highway 85 several times, and occasionally on other freeways. It has always been a clear, sunny day, and it seems to occur when the vehicle travels from deep shade into bright sun (i.e. as when going under an overpass). The software on the car is and has always been updated to the latest standard. Driving on east and westbound on US 60 near Phoenix and Mesa AZ the car will emergency brake from time to time. This condition has continued for years off and on and is still happening as of this week. Generally speaking it appears to be caused by shadows from the overpasses and is far more prevalent at low sun angles that create significant shadows leading up to those overpasses.. I don't recall it happening after dark. On several occasions I have submitted "bug" reports via Tesla's online automated system.

While using Autopilot adaptive cruise control at speeds of 50-70 mph, the car suddenly applies the brakes even though there is no car or other hazard anywhere near the car. This phantom braking has been happening since I bought the car in 2018. I imagine it's a software problem. I read recently that NHTSA is investigating a similar problem on 2021 and 2022 Tesla Model 3s, so wanted to let you know that this problem is also affecting my 2018 model. Tesla FSD beta/ autopilot has frequent phantom braking. Sometimes for oncoming traffic in opposing lane or because of parked cars or even a shadow or nothing at all. It’s dangerous especially when it slams on the brakes so hard items in car will fly to the front. Could cause rear end collision or startle driver and cause them to react poorly. I have been doing bug reports and sending emails to Tesla FSD beta team since 11/2021 after each new update and it has never been resolved even remotely to an acceptable safe feeling product for use.

On multiple occasions while my car was in Autopilot mode on highways and at highway speeds, I have experienced rapid braking (phantom braking) when there was no apparent reason for such. All of the phantom braking events have occurred on sunny days, usually around mid-day. Most often, the events have occurred when approaching a bridge or overpass. My supposition is that the system is mistaking dark shadows under bridges as obstacles that need to be avoided. As a result of the frequency of these events, I no longer use the system on sunny days.

The contact owns a 2018 Tesla Model 3. The contact stated that while driving at highway speeds with the vehicle in auto pilot mode, the vehicle would erroneously reduce speed and on occasions nearly come to a complete stop. On one occasion while driving approximately 70 MPH, the speed was suddenly reduced to 10 MPH. The cause of the failure was not determined. The manufacturer was notified of the failure but no assistance was offered. The local dealer was not contacted. The failure mileage was 5,000.

On several occasions, with Autopilot engaged my 2018 Tesla Model 3 has strongly applied the brakes while traveling at highway speeds, despite the fact that there were no obstacles up ahead. This has occurred throughout my ownership of the vehicle, and in each instance, it has been extremely unsettling. It required me taking manual control of the vehicle in order to return to highway speed. I do believe that on these occasions my safety and that of my passengers and other motorists has been at risk. It is worth noting that each time this has occurred there has been an overpass up ahead. I have hypothesized that the Autopilot interprets either the overpass or the shadow cast by the overpass as an obstruction. As others have reported, please add my 2018 Model 3 to the list of reports of the vehicle having constant phantom breaking issues that Telsa needs to address. I'm glad you are conducting this investigation. Most times phantom breaking occurs I submit a "Bug Report" using the voice button in the car that sends car logs directly to Tesla so they will have records of these phantom breaking issues at the exact time and location my car experienced it. While it is random it also is very predicable at certain spots on highways I drive in the Bay Area. Namely 680 South in the HOV lane right before the Mission Blvd. exit. While driving to work, in 2021, I had experienced sudden deceleration and disengagement of the autopilot. It happened a few times. I have ignored it thinking it to be an electronic glitch. I did not know how to report to Tesla. But since I was sharing my data with them, I thought they will pick up some issue and alert me if there is really something malfunctioning. When driving on autopilot on highways phantom breaking occurs for no reason. There are no warnings, the car just slams on the brakes. Elon Musk has state they were removing radars from cars to eliminate this issue and disabling it in cars with that hardware, but since my car have gone to all vision it is much worse. While following vehicles should always follow at a safe distance to prevent collisions, the reality is most drivers follow too closely and this could very easily result in a rear end collision. It could also trigger road rage from following drivers assuming the driver is doing a brake check on them. This has occurred numerous times over various software versions, but its much worse with the pure vision version of autopilot. Current software is 2021.44.30.5

I drive twice a week from Washington DC to Charlottesville, VA, a distance of 117 miles each way. 35 miles are on an interstate and 82 miles are on a divided 4-lane state road. Since I restarted this commute in September 2021 after Covid shutdowns and summer break, I have experienced recurring, frequent problems with the car's vehicle warning system, adaptive cruise control, and forward collision notification system. When I drive on the interstate (where there is currently a great deal of construction), the adaptive cruise control keeps automatically adjusting my speed when there is no speed limit change.
