Backup camera recall was not fixed properly or the new parts have same issue. As a result the backup camera was not functioning. When the camera is not functioning the autopilot system drives aggressively, speeding up to the max set speed when it is not safe to do so with current traffic conditions. Had to disengage autopilot otherwise it would have crashed. Also I received an alert that the emergency breaking system was not functional. When reversing could not use backup camera.

October 12, 2024. I was using the Tesla on autopilot. I was in the far right lane and the freeway was curving to the left. The car was following in the lanes, but then all of a sudden it stopped following the freeway and drove into the right guardrail. There were no alerts by the car. It did not warn to take over the vehicle or attempt to avoid the collision. The car drove directly into the guardrail without warning and was totalled.

Two incidents while testing the most recent version of Tesla's "Full Self-Driving": 1. While driving at less than 25mph in a residential neighborhood, using the "Full Self-Driving" mode, the car was entirely unable to detect either heavy steel plates in the road (being used to cover road work) or significant potholes in the area near the plates. 2. While driving on H1 (a federal "interstate" highway) on Oahu, at approximately 55mph, using the Tesla's "Full Self-Driving", the car inexplicably slammed on the brakes. There was absolutely nothing visibly in front of the car for at least 200 yards. The car behind me had to swerve to avoid rear-ending my vehicle.

I have a 2018 Tesla Model 3 that was involved in an accident on 10/12/2024. It has Enhanced Autopilot, which was the most advanced version of self driving when I bought the car. It was engaged in "Navigate on Autopilot" on the freeway. The car was following the freeway as it veered to the left, but then it stopped following the lines and crashed into the side of the freeway. The result of the accident was a total loss on the vehicle. I had shoulder and neck pain that have not required serious medical attention, but it was almost a week of discomfort and difficulty sleeping. My insurance company has deemed the vehicle a total loss and I will have to get a replacement. There were no warning alerts and the vehicle did not recognize that it was leaving the freeway lines. I was not going over the speed limit....

I was driving (~42 m/h) my Model 3 Tesla on a freeway and the traffic in front of me suddenly slowed down. The driver in front of me (she was a new driver with a new car) braked hard and came to a complete stop even though the traffic in front of her was moving. I ended up colliding with her as a result. I would have expected the Tesla Forward collision system or the Automatic Emergency Braking system to have activated and prevented this crash. Further, it seems Tesla brakes behave differently when it is fully charged as the engine braking does not engage. In fact the car seems to accelerate instead. If the Tesla Forward collision system and the AEB do not work and prevent such accidents, what is the point of having these systems?

I may have figured out the common link between Tesla Autopilot crashes. Distracted operators have the accelerator pressed while the TACC is engaged, thinking (for many reasons) that the vehicle will stop if there are any obstacles in front of them. However, when the accelerator is being pressed, the vehicle will not stop. It will happily run into a wall. One second before impact, the forward collision warning turns on, followed by the ABS and the slamming of the brakes, but by then it's too late for a distracted driver. The raw Tesla "Vehicle Data" on Column D, shows "Accelerator Pedal Position (%)." I strongly encourage you to take a look at the data for the autopilot crashes and review this column to see how many had the accelerator pressed until 1 second before the accident, even if the autopilot was engaged. Following an over-the-air software update, all audio in the vehicle stops working. This includes entertainment services, but more importantly turn signal indicator "clicks", forward collision warnings, parking assist chimes, shift confirmation and warning chimes, and various ADAS notification sounds. In some instances, the problem corrected itself within 24 hours. In another instance, it persisted for nearly a week before I discovered that unplugging the USB cable for the dashcam fixed the problem. The dashcam hard drive has been installed for over 4 years, per the instructions in the user manual, without problems until early 2024. There were no error notifications or anything else to indicate that there was a problem with the car's safety systems, or what the cause of the problem was, except for the lack of audio. I received a warning message that displayed, “Front Left Safety Restraint System Fault.” This is an engineering problem as the cables associated easily detach under the driver’s seat. Tesla has been long aware of this problem. They came out to my home and replaced the wire connection by installing sturdier connectors. I was subsequently was charged $329.40.

I have also had the same "forward collision warnings" on a flat stretch of road when there is clearly nothing in front of me in my lane. I have also been experiencing "phantom braking" episodes, noticing a pattern where they will happen as I approach a bridge overpass, as if the car thought the bridge overpass was perhaps an obstacle. This is a report of suddent unintended acceleration on a 2018 Model 3 performance with 47,800 miles on the odometer - the first known occurrence on this car. At 50% state of charge, I drove the Tesla on a 10 minute trip and pulled into an underground parking garage with 3 lanes of valet parking. At about 5-10 MPH, the forward collision warning came on as I was driving towards the cones and stop sign. I brought the vehicle to a full stop with the friction brakes (not regenerative braking, which was set to LOW). The drive mode was set to HOLD, which lets the car roll forward but not backwards and doesn't CREEP when foot is off the brake. During rush hour traffic on Highway 101 Southbound near Redwood City, I experienced an incident while utilizing AutoPilot and AutoSteer. Despite being in stop-and-go traffic, the vehicle did not respond to the car in front which suddenly came to a complete stop. Neither the Automatic Emergency Braking system nor warning sounds, which has been known to activate erroneously, were triggered. As a result, I was forced to manually brake at the last moment, but there was not enough time for manual intervention and the vehicle collided with the car in front. It is worth noting that every other instance in which I have utilized AutoPilot and AutoSteer, which occurs on nearly every trip I take, the vehicle has been able to respond and brake, albeit with slow reaction times and close distances.

Full Self Driving was engaged, my hands were on the steering wheel, and vehicle made very unexpected and wrong turn sharply to the right directly into a barrier with front bumper. When driving on the highway and using the "autopilot" lane assist feature the care will automatically change the set speed limit and hard brake which puts my passengers and I in danger of being rear ended due to the abrupt stop. For example I have been driving on the highway with the autopilot set to 65mph and all of a sudden the speed will change to 35 and execute a hard braking maneuver. To compensate for this I have learned to hover my foot over the accelerator to counteract this phantom braking. When this happens there are no alerts that there is something in my path that I need to slow for. Other times there are forward avoidance alerts, but there is nothing in the path. The manufacturer has done nothing to remedy this issue.

Very recently Tesla had a standard software update for the car. They happen all the time. There was no warning with this update (or any update I’ve seen) about removing paid for features if you install the update. This one disabled my radar and turned my radar based autopilot I specifically bought this car for into vision based autopilot. I paid $15k for full self driving and never requested the beta to use it because I specifically did not want to disable my radar because of safety concerns. They did it anyway now, and without warning. It phantom brakes now all the time and it never used to and the general autopilot performance is worse and unsafe. Constant "phantom braking" occurrences despite great conditions (sunny, no wind, good visibility, low traffic) during a road trip that was two hours both ways. Typically happened when coming to the crest of small hills, or when "road mirages" due to light refraction, or when a car in the distance just became noticed by the vehicle. Happened both on divided highway and two lane highways. Several braking occurrences were pronounced enough to be likened to not quite slamming on the brakes, but definitely applying them firmly. regularly when autopilot or cruise control are engaged, the emergency braking will abruptly slow the vehicle when no object is on the road, highway, or interstate. This creates an unsafe environment when vehicles behind you are not following at a safe distance. I have the issue recorded on video. The computer display totally froze for several minutes while driving on the highway. The turn signals would not work while the computer was frozen and the system needed to reboot after several minutes before the turn signals would start working again. On June 1, 2022, we left on a roadtrip to CA from UT. I received access to Tesla’s Full Self-Driving system in late 2021. In the lead-up to that access, Tesla used a series of driver scoring metrics to select which users would get access first. Consistently, my metrics for forward collision warnings would show as alarmingly high, despite never coming within remote striking distance of another vehicle. On more than one occasion, a vehicle passing in other lane or turning left but in the other lane tripped the warning. My car is equipped with forward-facing radar, but ever since Tesla turned it off in favor of "Tesla Vision" my Autopilot/TACC experience has been greatly worsened. The car suddenly (and very unsettlingly) slams on the brakes on the highway or city traffic (I have FSD Beta) for anything from shadows, to hills/inclines, to curves, or minor harmless debris (shopping bags) in the road. This has not yet caused an accident but has almost come close. Other issues: - FSD Beta, when engaged, forces the computer to have control over the high-beams and windshield wipers. The software on the car is and has always been updated to the latest standard. Driving on east and westbound on US 60 near Phoenix and Mesa AZ the car will emergency brake from time to time. This condition has continued for years off and on and is still happening as of this week. Generally speaking it appears to be caused by shadows from the overpasses and is far more prevalent at low sun angles that create significant shadows leading up to those overpasses.. I don't recall it happening after dark. On several occasions I have submitted "bug" reports via Tesla's online automated system.

Tesla FSD beta/ autopilot has frequent phantom braking. Sometimes for oncoming traffic in opposing lane or because of parked cars or even a shadow or nothing at all. It’s dangerous especially when it slams on the brakes so hard items in car will fly to the front. Could cause rear end collision or startle driver and cause them to react poorly. I have been doing bug reports and sending emails to Tesla FSD beta team since 11/2021 after each new update and it has never been resolved even remotely to an acceptable safe feeling product for use.

Many times when I am driving using the adaptive speed control the vehicle will suddenly brake very hard, slamming on the brake, scaring the driver and all passengers in the car. I bought the standard autopilot for 2 model 3sI find it highly useful. It keeps me in the middle of my lane. Warns me with loud signal and bright red car marking of cars in my blind spot if it is dangerous to change lanes. Allows me to see clearly all cars around me. It will not allow me to drive without my hands putting active pressure on the steering wheel. I feel that it makes driving much much safer. Driver still needs to pay attention with active pressure on steering wheel.I drive 30,000 miles per year. I paid $3000 extra to buy the autopilot after not having it for the first 6 months. It was well worth the cost.Should be a standard feature on all new cars!!! My wife who drives my other model 3 has had the same experience.
