2024 TESLA MODEL Y Forward Collision Avoidance Automatic Emergency Braking Problems

Tesla's full self-driving software continues to malfunction. On January 30th at 9:50 a.m. mountain standard Time I was parked at work and discovered my vehicle was left unlocked with a partial window unrolled. This was my first indicator of many faulty problems signaling the vehicle was not operating properly. The vehicle has a proximity sensor via Bluetooth that locks and unlocks the vehicle on the owners approach was not properly working. On January 31st I experience a catastrophic failure with the software of the vehicle as I was locked out of my vehicle after utilizing summons feature that stopped the vehicle in the middle of the intersection obstructing traffic. Police arrived on scene within 30 minutes and we were unable to put the car in any gear to either drive the vehicle or place it in neutral to roll it out of the way. I have detailed video of the...

Incident Description (in my own words): We were driving southbound on [XXX] from New Hampshire back to Boston in our Tesla Model Y. Traffic became increasingly congested and slowed to a crawl. My wife was driving, and I was in the front passenger seat working on my laptop. As traffic built up, my wife became momentarily distracted and did not apply the brakes in time. I noticed, in my peripheral vision, the vehicle ahead of us—a truck—suddenly filling the windshield. At that point, I heard the forward collision warning alert, but it activated extremely late, essentially at the moment of impact. The vehicle did not automatically brake or stop, despite the low speed, close proximity, and clear visibility of the truck ahead. We collided with the truck in front of us. Damage was minimal (license plate damage and a small dimple on the front of the vehicle), but the safety...

Autopilot was engaged and active at the time of the incident. The vehicle failed to detect a stationary piece of road debris (appearing to be a dropped vehicle part) located in the driving lane. The system did not provide any warning, slow the vehicle, or attempt an avoidance maneuver. The vehicle drove directly over the debris, resulting in damage to the lower exterior/body panel. Road and traffic conditions were normal. This raises concerns about Autopilot’s object detection and hazard response capabilities.

I am writing to report critical safety issues I’ve experienced while using Tesla’s Full Self-Driving (FSD) software version 14.1.3. The recent update has introduced several dangerous behaviors that were not present in previous versions. One issue occurs frequently on neighborhood roads: the vehicle performs sudden emergency braking for no valid reason. For example, during one drive, it abruptly stopped because a few leaves blew across the road. There were no pedestrians, vehicles, or real obstacles present. These false-positive braking events are alarming and could easily cause a rear-end collision. An even more concerning issue arises during highway driving. The car now repeatedly swerves partially into adjacent lanes—without signaling—and then abruptly returns to its original lane. This happens constantly, even in light traffic, and it did not occur in previous versions of the software. While driving ~40 mph with no Autopilot or cruise control active, my 2024 Model Y suddenly braked itself from 40 → 0 mph for no reason. The road ahead was clear. I did not touch the brake. My 3-year-old was in the car. We were nearly rear-ended and could have been seriously injured. When I went to Tesla Service, the manager insisted I must pay a $270 diagnostic fee unless they “find a problem,” while also saying the incident was likely caused by “shadows.” This makes me believe they will simply say “no issue found” and charge me despite this being a severe safety malfunction. When I reached out to Tesla Customer Support, they stated it was a known issue. This poses a serious risk of rear-end collisions, injuries, or fatality, and Tesla is not addressing it.

On November 8, my 2024 Model Y suddenly came to a complete stop from about 40 mph while I was driving with no cruise control or Autopilot active. This was extremely dangerous — my [XXX] was in the car, and we were lucky no one was behind us. It put myself, my child, and others in the absolute risk of collision and injury. When I contacted the service center they said it sometimes happened due to "shadows" or "sun". They refused to run a diagnostic on the car for free and wanted me to pay for it. There was a sudden unintended acceleration and the brake did not work, causing a major accident. Car caught fire and door would not open. The car has completely engulfed. There were no warning lamps, messages or other symptoms prior to the failure. Vehicle has been inspected by insurance and police, but not by manufacturer.

FSD failed to break when another vehicle was making a right turn. I had to take over is a split second. I have dashcam footage but unable to upload in this form.

On [XXX], at approximately [XXX], I was driving westbound on the [XXX], just before the [XXX] and [XXX] off-ramp, with my wife and our [XXX] baby in the car. While in traffic, I noticed the regenerative braking system was not slowing the vehicle down adequately, despite having a clear and sufficient distance from the big rig ahead of us. As the gap closed, I slammed down on the brake pedal, but it felt abnormally stiff and did not respond with the urgency or force expected. shortly after i purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. i reached out to tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. i explained to them that in both cases i had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how i turn this auto feature off. they said there was no way to turn it off. i explained to them that ALL automation features should be optional, they argued with me. came to find out some sensor in my driver's seat was malfunctioning. they...

I was driving at a speed of 60 miles per hour in rainy weather when the car’s emergency braking system suddenly activated. At the same time, the steering wheel locked due to the braking, and I completely lost control of the vehicle. The car spun multiple times before crashing into the center divider of the highway and finally coming to a stop. This was an extremely dangerous incident from a safety perspective. This was the first time such a braking malfunction occurred. The incident has not yet been investigated by the insurance company, and no warning or notification was displayed by the vehicle at any point.

After update 2024.38.2 they introduced a "Active Curve Assist" which can't be disabled while using the cruise control. This has caused the car to excessively brake (70mph to 45mph), a couple times while a vehicle is following resulting in a near rear end collision and has created dangerous situations

Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame....

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start on 1/12, and I scheduled the earliest appointment available for 1/30. They did not offer me a rental or any assistance during this time, and I need my vehicle to commute to work and conduct other essential activities. It has just over 1,000 miles on it and is ~3 months old now. Park assist error message when put car in reverse. No visualizations on display. Early collision not working. Several times (not in this instance) backup camera has been black and does not show an image. Sentry system error message. FSD offline. (may not be a safety issue but wanted to provide information as it would show other errors in the system affecting safety) I have a scheduled appointment with the service center in February.

Around the end of November, my 2024 Tesla Model Y's cameras stopped functioning. The sensors for blind spots and possible collision prevention stopped working. The GPS was also not working, the car believes it's been in the same spot for over a month. The lights of the car also do not work as promised, as now I have to manually turn them on on my dashboard and it goes on and off. I have been trying to get into Tesla to have this fixed but the appointment keeps getting pushed out due to the part that needs to be fixed not being in stock. All cameras stopped working. Blind spot detection not working. Blinkers stopped working. No parking assist. Auto wipers . Adas not working. Screen auto brightness not working. Clicking when letting off accelerator. We damaged our tire and rim due to this issue.

The autopilot computer failed. This has rendered all safety systems and camera on the vehicle inactive, including the rear view camera, all forward and side cameras so no active safety systems are functioning. This includes crash avoidance, lane keeping, blind spot warning, cruise control, and rear camera during reverse, the vehicle is nearly impossible to reverse as the view is terrible with no cameras.

The car lost all ADAS functions, and GPS on the onboard navigation system. In addition. it can't use regular cruise control and it does not have rear view mirror blind spot warning capability any more. This all happened when the car attempted to install the "holiday" updated from Tesla automatically around 11/29/24 .

On 12/16/2024 my vehicle received software update 2024.39.15, after installing the update i went to the grocery store and noticed nothing was working in the car except basic driving functionality. cameras were showing up as a black screen. TACC, aurosteer and FSD supervised were no longer working as well as AEB,FCW,BSM,LDW,LKAS and the gps had me in the middle of the ocean, headlights on during broad daylight. I informed a service center by my home and they simply didn’t care, i took it to another service center 50 minutes away and they took two days to do repairs and diagnostics, i was told the computer had shorted out and the technician replaced the car computer and AI4 (HW4) computer system which runs safety features. My Tesla Model Y experienced a complete computer failure, rendering all active safety systems inoperable. This included backup cameras, speedometer display brightness, and collision avoidance system. After receiving assistance from Tesla service, the issue was diagnosed as a shorted computer. A computer replacement was performed, resolving the problem. I am submitting this case as a reference for potential investigations.

Tesla shared I have an AP-4 computer short that disables all cameras (rear back up camera, blind spot camera, lane deviation detection, emergency assisted forward braking), navigation, and auto lighting features are disabled. Other non-essential features do not work as well.

All of the cameras on the vehicle are no longer working after the latest update. This means all sensors (even as simple as windshield wipers) do not work. All driver safety assistance does not work.

One day the cameras, gps, and sensors just stopped working and haven’t worked since. Been 4 weeks.

All cameras stopped working. With this, all safety features are not working. I can’t see to back up at night. No collision avoidance, no lane departure warning, high beams don’t stay on, turn signal doesn’t stay on. It has been two weeks and Tesla has no fix. There was no warning, just noticed it one day as I was driving it and was very dangerous in dark snowy conditions. I called Tesla service immediately and they gave me an appointment for following week. Then next day I got message from them that there are a large number of vehicles having the issue and they said they don’t have a fix for it and moved my appt out 7 weeks. I have gotten no help from them and I told them I don’t feel safe driving the car. They have not provided another car.

On Nov 23, 2023, I was reversing car from my driveway in the morning. Car started giving warnings with issue on all its cameras. While reversing car, it will give error that rear view camera not available. While giving left/ right turn signal, it is showing blank screen for that view. All other safety features like blind spot detection, lane assist are not working at all. I also started getting error message that emergency brake is not available. I have been contacting them for immediate repair/ provide loaner car from same date (11/23/2023). Earliest appointment date given is Dec 11, 2023. This is not acceptable at all. It is very dangerous to drive the car without those safety futures. I am also concerned on any other issues which it has not reported. I don't feel safe to drive the car.

The computer is not working which connects to the forward collision warning system, navigation, and all safety when it comes to driving. I am still waiting to hear back from Tesla about the problem and haven't had a resolution.

Vehicle cameras stopped working. Navigation stopped working. Screen also stopped working for a time. Was able to get it back on but it went off multiple times since. If I can't see the screen, I can't tell how fast I'm going to do anything else. Cameras control many of not all of the safety systems. So those are all inoperable. Issue is being reported across multiple forums all over the internet as "short in ap4 module."

While driving between 50-55 mph the Tesla applied emergency breaking even though there was nothing in front of us and no risk of accident. The car went from 55 to about 5-10mph in the matter of a second. We are lucky no one was directly behind us or it would have led to a high speed rear end accident.

I have had multiple incidents of “phantom braking” when using traffic aware cruise control and auto steer. It usually happens on one lane state highways with opposite direction traffic, sometimes with turns or hills. The vehicle has slammed on the brakes without warning, sometimes the warning tone also sounds. Each time there was absolutely no danger, nothing abnormal. On a couple occasions there was traffic behind me, and it must have scared them. They probably thought it was a hard brake check. If I wasn’t able to quickly over ride the braking with the accelerator it very easily could have caused an accident. On two occasions my pregnant wife was in the car and it scared her very much. In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.

I was driving on [XXX] across Kansas and my car suddenly braked hard. I was in cruise control when it occurred. I was not in automated driving mode as I don’t have that feature. This happen once before earlier this year on a two lane highway when a truck was coming toward me in the opposite lane. While using adaptive cruise control the vehicle regularly decelerates for no reason and without warning "phantom braking". I experience this about once a week, but experienced it multiple times on a longer drive this weekend. The unexplained abrupt braking could result in being rear-ended by another vehicle.

On multiple ocassions when using adaptive cruise control the vehicles slams on the brakes without warning and for no reason and brings car to a near standstill in the middle of traffic unless I slam the accelerator down and fight the car. Engineers from Tesla inspected the vehicle and said everything was working normally, but it continues to malfunction and almost causes accidents with cars behind in traffic having to slam on their brakes as well.

I was driving on a street at the speed limit there were no cars in front of me and behind me thankfully the car just braked very hard suddenly for no reason and I lost complete control over the vehicle after the car came to a complete stop I was able to continue driving. Thankfully there were no cars behind me because if there were I don’t even want to imagine the accident I could’ve caused aside from this the breaking was so hard that my whole body hurts and I am afraid to drive the vehicle as I do not know when to expect this breaking since it can happen so suddenly at any time.

I was parking the Tesla Model Y at very low speed, when there was a sudden unintended acceleration. The car banged against the wall of a building and was severely damaged. My family - wife and two children were in the car.

I was driving and going about 50mph and there was no other car near me. Suddenly the car slammed on its brakes. Dropped to probably 10mph. My mom and I were both wearing seatbelts but our heads were thrown forward. My neck and shoulder are sore and stiff today now. There were dark skid marks in the road ahead of us and I believe the car saw those and broke because of them. If there had been a car behind us, we would have be rear ended. There were no warnings beforehand. When the car broke, lights flashed and there was also an alarm. The vehicle has not been inspected by police or manufacturer.

The incident occurred on [XXX] at [XXX] PST in Sammamish WA, where my car (2024 Tesla Model Y) incorrectly enabled Automatic Emergency Braking , and unexpectedly applied brakes when I was traveling at 45 mph on an empty road. The sudden braking reduced the speed of my car to less than 15 mph in a span of 2 seconds and eventually brought the car to a halt . I strongly believe this was done in error due to a system malfunction, as there was no potential cause of a collision given there were no incoming vehicles or any vehicles ahead. During my trip, traveling at speeds ranging from ~30 to ~75mph my car suddenly and with no advance warning braked to a full stop, four separate times. I was not using cruise control or any driver assisted features. There were no obstacles in the road. I did not get in an accident because there were no other cars near me on the two different roads I traveled on.

My family and I were on [XXX] heading toward Sacramento when the Tesla Y suddenly braked. We were using autopilot and going around 70 mph. The car was trying to pull a full stop in the middle of the freeway! I quickly disengaged the autopilot and stepped on the pedal to speed back up. I'm guessing the Tesla saw a hazard on the road and put on the brakes. The problem is there were no hazards. Our safety was put at risk because we could've been reared, and it could've caused a pileup. Luckily, it was early Sunday, and there weren't a lot of cars on the road. This is my fifth Tesla, so i.am very familiar with the Self Driving feature which is mostly good. However, recently I got a Model Y and with in 24 hours I was rear-ended on the freeway! I had the car in Full Self Driving mode (FSD)! For some reason it just braked hard in the middle of my driving however, traffic was backing up. I injured my lower back and neck and I'm now having to go in for therapy and schedule a CAT scan. Luckily the person behind me was fine. After the incident I downloaded the zip disk which records incidences. The car had documented pre and post accident! But the actual incident there is no record... almost like it got wiped out!?!

The vehicle would not stop and ran into the back of a friend. I am not safe in the vehicle as we were in a parking lot no way I could’ve reached top speed.
