2020 TESLA MODEL 3 Forward Collision Avoidance Automatic Emergency Braking Problems

While driving my Tesla Model 3 with self-driving engaged in excellent conditions, the car suddenly braked to a full stop. There was no apparent reason to do so and the driver did nothing. The road ahead was free of traffic, and no obstacles, pedestrians or animals were visible. It was possible to resume driving by turning self-driving off. Tesla forums online show many instances of others reporting similar problems in various Tesla models over several years. A rear end collision will ultimately be the result of this flaw if it hasn’t yet occurred.

My Model 3 failed at under 50,000 miles and gave out a series of failure warnings. It was unable to drive anymore and steering became stiff and unresponsive. In researching this I see it is a common issue with hundreds of thousands of cars affected. And the NHTSA has an open investigation into it. Tesla as usual is denying the problem exists.

In a private parking lot, within line of sight and ample time to respond to problem I used Tesla Smart Summon. Instead of taking an obvious route through the parking lot, it narrowly hit a parked car, then proceeded to drive over a parking barrier and into a wooded field towards me and a crowded food truck area. I was unable to cancel the feature in time to stop the car from the collision as it accelerated too fast and without any initial signs of an issue. I am reporting a critical safety incident involving my vehicle, which occurred during rush hour on a busy highway with no traffic lights or stop signs. While waiting in the left turn lane at a center divide, my emergency autopilot braking system activated unexpectedly, causing my car's left front side to encroach into oncoming traffic. Despite my attempts to move the car, it became immobile. I attempted to reset the car by turning it off and on, but the vehicle’s computer system malfunctioned entirely. With oncoming traffic traveling at speeds of 70 mph, I had no choice but to exit the vehicle for safety. I left the car in the center divide. This incident was extremely frightening, and I am deeply concerned about the potential dangers posed by the malfunctioning autobraking system. Thank you for your attention to this serious matter.

I have taken my vehicle into Tesla to have the issue fixed twice. They don't know what's happening or how to fix it. My car is randomly taking the wheel and steering me into other cars / off the road. It happens once a month or so, I have a newborn baby I drive in the car and I don't feel safe driving. I've recorded the incident and Tesla Service said "that should not be happening unless I'm using auto pilot" which I never do. I've looked into lemon law but I did not purchase the car from tesla directly, I bought it from a used car dealership. I'm not sure what to do, I can't sell the car because I would still owe $10,000 after the sale. Please help anyway you can, I'm desperate for anything.

While driving on cruise control at highway speeds (75-80mph) on I-80 the car braked VERY hard and unexpectedly MANY times. There were no other cars around me, weather clear, road straight. No obstacles noticed to cause this reaction. If a car had been behind me I would have caused him to rear-end me because there was no apparent reason for the severe braking. I reported four incidents of this type to the Tesla during the drive which I believe were date/time stamped so they could diagnose (using the "report" function). I matched these with my own observations which I also provided to the service department. I have had the car diagnosed by Tesla and they claim they could not duplicate the problem and/or fix the problem.

Driving at highway speed (75 mph) using Autopilot & car rapidly applied hard braking. There were no obstacles, shadows or anything visible that should have caused this. It occurred twice over two days in different locations. If anyone had been behind me too close they would have collided with me. No other vehicle I’ve owned have done this with their implementation of adaptive cruise & lane assist. My car used to use radar and I never had this issue until vision based detection was implemented. It’s occurred numerous times over the years, but no this rapid of braking.

I was traveling on autopilot in left lane with autopilot engaged on 06/13/2024 on HW 101 at around 9:40am my car all of the sudden slammed on brakes and dropped speed from around 65mph to 30mph before I was able to react and recover speed. Reached out to Tesla on their app about this and they are sending me estimate of $275 to check it out

While operating TACC with lane keeping assist, I experienced multiple iterations of phantom braking when the road and weather was clear ahead. This phantom braking could potentially cause other drivers to collide with the vehicle that unexpectedly braked in the middle of the highway. This problem has been a common issue with multiple people taking their cars to the service center to get checked out with no problem identified. There are no warning lights or indicators that the Tesla is going to phantom brake on the highway. Recommend having Tesla provide information regarding this phantom braking.

During a month-long road trip in the US, I experienced frequent "phantom braking" incidents while using adaptive cruise control. In some cases, Autosteer was also active. By "phantom braking" I refer to sudden hard braking by the car itself in clear daylight conditions on interstate highways with no vehicles visible ahead of me for at least 1/2 mile. By "frequent" I mean that this behavior occurred multiple times per hour over multiple days. I decided to cease using cruise control for the remainder of the trip after the following episode occurred. At lease several times in the past 3 months, when using AUTOPILOT and when passing semi-trucks on my right my Model 3 has automatically given a very loud, what I believe to be either a Forward Collision Warning or a Lane Departure Warning, then braked and seemingly veered to the left! I've had the car since December 2019. The car will "phantom brake" when the autopilot adaptive cruise control is on. This happens intermittently, ~1 time per month on my normal commute. I will be going along, cruise set to 75 and out of nowhere it'll slam on the brakes and lose 10 MPH in a second before I can take over. This occurs with absolutely no traffic or reason, it'll be an empty highway and it does it. I drive with my foot over the accelerator so I can floor it to override the car. This is the exact opposite reaction I would think that someone should have to do with a safety feature. Tesla tells me the system is working fine, but random brake checks on the highway should never be normal. I don't understand how this system was approved to operated by the government.

On September 9, 2023, we were driving from Winter Park, CO to Omaha, NE. Three or four hours into our trip, we experienced what is described on the internet as "phantom breaking" four or five times in an hour. I have had multiple incidents of sudden speed reduction on major hi ways. (us 287 and I 25). Usually these incidents occur when I am traveling north (increasing elevation and later in the day), but that is not always the case. The events appear to be totally random, but result in a speed reduction of 70mph to 40mph rapidly enough that loose items (including pets) fly. on occasion I have had to be very quick to accelerate to avoid being run over by a tailgater. I scheduled a service appointment, but was discouraged from doing so by Tesla Service people who said “the car is still learning”. The fault has not improved. If anything it happens more frequently. At this point, I would say we travel “on edge”.

My car cameras and audio will randomly stop working. Come to find out I took a recording with a timestamp of 1151 pm where I am on the highway and driving. Have witnesses of me driving and the issue. My model 3 logs, as stated by service technician, say my vehicle is "Parked and not being operated". This means I am driving with potentially dangerous and absolutely unreliable software in my car. It shows the car isn't being operated as I am going 60~ on the highway in traffic. My car cameras will go completely black and the audio warnings will go out including the backup sounds which are a required feature. Phantom Braking occurred almost 40 times on our recent trip from Northern NV to California and return. We left on July 16 and returned on July 21. Our route was from Minden NV to Las Vegas on Highway 95 and then onto Temecula CA on Highway 15 and 215. The return trip was on Highway 15 to CA 138 to Highway 14 to Highway 395. All travel was during daylight hours. Sometimes phantom braking was so severe, the deceleration cause us to be thrown forward towards the dashboard. We wore seat belts to prevent hitting the dashboard. This also happened on a similar trip in 2022 and was reported to Tesla and were told that nothing could be done. The current software version is 2023.20.9

Unexpected Phantom Braking was experienced in this vehicle on a sunny day at mid day on I-30 going East through Arlington. The car was operating on Traffic Aware Cruise Control. Travelling at approx 70mph the vehicle suddenly slowed very quickly below 50 mph before I interceded to resume control of the accelerator to resume my speed before I was rear ended. This occured as I was approaching an overpass in my middle lane.

I was driving from indio,ca to Orinda,ca on interstate 10-210-5 north and the vehicle had at least 6 phantom braking events when the car was in cruise control and one event the auto pilot was on. There was clearly nothing front of me. This is so dangerous and there will be a bad accident if it isn’t fixed. Please have Tesla fix this.

When using “Auto Pilot” (Teslas adaptive cruise control) the vehicle experiences violent phantom braking. The system slams on the brakes in some cases going from 65 to 20 MPH almost instantly. This happens on a fairly regular basis and is a massive safety issue. We have nearly been rear ended on several occasions.

I was driving on a highway using the adaptive cruise control. Several times the car braked for no reason at all. There were no cars in front of us, it was phantom breaking for no apparent reason. While driving in FSD, autosteer, traffic aware cruise control, and with all turned off, I have experienced 5-10 episodes of “phantom braking” on the highway at high speeds over the past 48 hours. I have driven in 4 states total so far; in sunny, cloudy, straight road ahead conditions, sometimes with cars around and sometimes on totally empty stretches of road. The first episode took my car from 80mph to 60mph extremely suddenly, with no warning, lurching me and the things in my car forward. I was shocked as I was unfamiliar with this issue. Luckily the car behind me at the time was about 1/8 mile away. However, this could have very easily caused someone to rear end me. I was on a 3000 mile road trip over multiple interstates (including I-35, I-90, and I-80), and while the adaptive cruise control was engaged, the car would abruptly and aggressively brake when no other cars or obstacles were in the area. This would often be a 30mph deceleration before I could disengage cruise control and take over. This happened repeatedly, in multiple states, and in different weather conditions. At times, it was happening every 10 minutes. The system would sometimes, but not usually, make an alarm noise. This problem has become noticeably worse in later software updates. No accidents occurred because I would not use cruise control if other cars were in the area, and I eventually stopped using it altogether due to how frequently it was braking.

Emergency braking is activated on highway when no obstacle is visible on clear sunny days. This has occurred over the last year but has become more frequent over time.

My car was running in auto-steering mode. Last year it did phanthom breaking 3 times in freeway ( Arizona I-10 from casa Grande to tempe direction). Same happened on 2/14/22, it did phantom breaking for no reason. Speed dropped from 80MPH to 55mph. Luckily no one was ahead or behind me. This is high risk for drivers behind me.

Using Autopilot on an empty highway at appx 65mph, the auto pilot system began to brake aggressively without any obstacles in front of me. As it approached 40 or 30 mph, I pressed on the brake to cancel the autopilot before I was hit from behind. Scary.

On a recent trip, while using the adaptive cruise control on a divided, straight, flat four lane highway, the vehicle would randomly slam on the brakes for absolutly no reason, endangering us and other vehicles behind us, who were not expecting a random slam of the brakes. There were no oncoming or road-crossing vehicles and no vehicles in front of us on the divided highway to trigger the reaction. This is the first and only time this has occurred as of today, 1/1/23. Around 11:45am EST, I was driving down a side street with no traffic coming towards me, behind me, or in front of me (and with no objects in the road). Suddenly, my brakes were aggressively applied and a warning came up on my screen saying it was the Automatic Emergency Braking that had been applied. It then released a few seconds after. I do not have the Autopilot option on my car, and I did not have any type of cruise control engaged at the time. If this had occurred in traffic for no reason it easily could have caused an accident.

Phantom braking issue. Cruise control set at 68mph. Medium freeway traffic at 7pm. Noticed disabled car ahead on the shoulder. No danger. As we approached the car, my car went into a panic stop as if it thought a collision was ahead. Car braked and skidded. I was able to regain control and accelerate and avoid a rear end collision. "Phantom braking" issue. When car is in autopilot or enhanced autopilot mode (an upgrade I purchased after I bought car), will brake suddenly and firmly at high speed for no apparent reason. Until September 2022 car would very occassionally brake too easily in autopilot mode (perhaps 3 times in two years), but it was not sufficiently forceful or frequent to cause alarm. However, on 9/26/22 and 9/30/22 during daytime driving car violently braked on highway at least 6 times over approx 500 miles of driving. When using autopilot cruise control, the car will brake hard for no apparent reason. I only use cruise control on limited access highways. When this happens, I immediately disengage cruise control and drive manually. This can happen at any time. I have driven the car cross country several times, driving all day. Some days it won’t happen at all, and on some days it can happen several times. I’m concerned that if someone is close behind me, they could hit me from behind. Ever since a recent software update was downloaded and installed, my Model 3's incidence of "phantom braking" events while using traffic aware cruise control/autopilot has gone up by a very noticeable degree. It has always been a known issue, however since the redundant safety systems (radar and ultrasonic sensors) on my model year vehicle were disabled by one of the recent updates, phantom breaking events that can possibly CAUSE a collision event or loss of control of the vehicle happens far more frequently. Instead of occurring once every 1000 or so miles, it now occurs two or three times every 250 or so miles, judging from performance on recent cross-state road trips. My car does not use the "Full Self Driving" package that Tesla offers.

While driving on an open highway using the Tesla Auto Pilot (Adaptive Cruise Control, NOT Full Self Driving), we experienced 3 separate instances of unexpected braking. In each case, we were on flat terrain with no vehicles in front of us for hundreds of yards. The weather was sunny and warm with few clouds in the sky. We were driving westbound between 10am and 1pm at the time of the incidents. On or about June 15, while driving my TeslaI received a notification from my Tesla that the automatic Emergency Braking was disabled. There were also notifications that Autosteer was unavailable, and so was cruise control. I made an appointment with a Tesla repair center for repairs to the car under warranty. My initial appointment was postponed to enable remote diagnosis. On July 6 I brought my car to a Tesla repair center. After looking at my car, Tesla informed me that the car had no then current alerts or faults. They "were able to confirm this is known firmware characteristic. Tesla is working to to resolve this in a future firmware update." It's not clear to me what they are saying and whether this was a false positive from the firmware error reporting system or a transient fault that they could not reproduce. Phantom braking: While driving down the highway for no apparent reason the car brakes and one time we had a car behind us that almost hit us. This is very dangerous and Tesla need to fix this asap. This has happened multiple times since around March 2022. The passengers in the car are slammed forward without any warning. Today was the scariest because we were almost rear ended and my 6 year old granddaughter was slammed forward. This is unacceptable.

On 6/18/2022, nearly all visual based safety and convenience features of my car ceased to function randomly. I was given errors/warnings on the screen that cruise ("autopilot") control was unavailable and that automatic emergency braking was unavailable. I drove the vehicle manually to my destination, and after sitting overnight, it magically all worked again. Software on the car at the time was 2022.16.1.1. I am filing this compliant because since obtaining this vehicle in March of 2022, the "autopilot" system has been a constant area of concern. Tesla really needs to get this system working, or perhaps be forced to reduce its functionality (like just have a normal speed based cruise?) until its ready.

My car when passing under signs or on two lane undivided roads will hit the brakes very aggressively. It’s difficult to reproduce and catches me and other drivers by surprise. The car didn’t do this during my first year of ownership, but now it happens several times a week. This will happen when using cruise control and autopilot. To me it’s rendered both features unusable.

While traveling on highway with Full self driving engaged car would occasionally brake and had to quickly press on the gas pedal for fear a car would hit me from behind. This seemed to happen when I approach an overpass but not most overpasses and typically happens once or twice during highway trips of about 80 miles on NJ garden state parkway. I had not reported this to Tesla or had the car checked for this issue and report it now seeing that there were formal reports filed on this issue to NHTSA. I do not recall whether any warning lights or audible alerts occurred during these incidents. First occurred after I took possession of car a couple of years back.

I was driving on highway 205 near Tracy CA. I was in the left lane approaching a left curve in the road. The right lane to me was occupied by a semi with "wide load" on the back. It was in its lane and just ahead of me. As we both approached the curve in the road, my car experienced a "phantom brake" episode thinking it was going to hit the truck. Again both our vehicles were in their respective land and the truck was not crossing over the line.
