All Forward Collision Avoidance Automatic Emergency Braking Complaints
The rear camera malfunctions intermittently and causes other errors to happen, like Automatic emergency braking unavailable or forward collision warning unavailable. When the camera error happens, it says the camera is unavailable. I took my car to the Tesla dealership three times. The first time, they did a continuity check and said the harness was degraded, and they replaced the harness and the rear camera. A week later, it happened again. The second time they reset the camera calibration and it worked. The day after, it happened again. The car is at the Tesla dealership for the third time (02/20/2026).
The car, with no known or evident driver, stopped the lane awareness, cruise control, etc. All of those just stopped working and per Tesla that's expected which is surprising since it's all part of the safety systems. The car's visual cues stopped identifying lanes and as a byproduct of that, none of the dependent features work.
I drove into my neighbor’s driveway to drop her off. As I slowed down and applied the brakes to stop, the car failed to respond. It continued forward, running over the flower and plant pots at the edge of the driveway, crossing the backyard, and finally colliding with the trees and bushes at the rear of the property. All airbags deployed, and the vehicle was damaged beyond repair.
Component/System involved: Advanced Driver Assistance Systems — FSD (Supervised)/Autosteer (Lane Keeping Assistance) and Adaptive Cruise Control. Possible failure to warn/stop (FCW/AEB). Vehicle and data are available for inspection upon request. I preserved dashcam files and requested Tesla to preserve engineering logs and EDR. What happened & safety risk: On [XXX] at ~[XXX] PDT on [XXX] near East Palo Alto, CA, with FSD (Supervised) engaged, the system appeared to misinterpret an exit ramp/gore area as a continuing lane at a highway fork and maintained ~60 mph (posted ~70). As soon as I saw the trajectory was unsafe, I braked and began manual steering takeover, but the vehicle contacted a roadside sign near the gore/shoulder before I could complete the maneuver. On [Wednesday, 07/09/2025], I was driving my Tesla Model 3 in rainy conditions when the vehicle suddenly swerved and crashed into a sidewall. The road was wet but not flooded, and I was traveling at a moderate speed. As I lifted my foot off the accelerator to slow down, the car's regenerative braking system engaged abruptly, which I believe shifted weight forward and caused the rear tires to lose traction. This led to hydroplaning and complete loss of control. There were no warning lights or alerts prior to the incident. The road surface did not have visible hazards or standing water that would typically explain a spinout. The tires were moderately worn but within usable limits. While using traffic aware cruise control or autosteer, the car occasionally detects a nonexistent person in the center of the lane I’m driving in, the proceeds to slam on the brakes when no hazard exists. It seems that tire markings in the road and certain lighting conditions cause the car to think there is a person in the road. Additionally, when driving on a highway with rolling hills, the car detects a forward collision alert and hits the brakes and disengages the autosteer. Both of these could cause a rear and collision at highway speeds. The dealer has claimed that the cause is functionally normally and I already tried to get it serviced but the car continues to have these issues. I do have video evidence of the car slamming on the brakes and detecting a forward collision.
I was driving east on [XXX] in Bourne MA using the Tesla adaptive cruise control system called "Autopilot" in very light traffic conditions. The sun was low on in the sky (behind and to the right). The system automatically engaged the brakes while crossing the bridge without obvious reason, decreasing the car speed about 10 MPH in less than a second before I overrode by pressing the accelerator. I believe the car vision system mistook a shadow cast by the vertical girders on the bridge onto the road as an obstacle. The sudden deceleration was a hazard to those behind that had to brake, but there was no collision or loss of control. The manufacturer seems acknowledge and accept that the defect exists, but has not been able to resolve the problem. vehicle constantly phantom brakes while driving. i have reported 100+ incidents thru my tesla "report bug" feature. It will read the speed limit sign on the off ramp and slow the vehicle down 20mps abruptly, it will see a car on an onramp and slam the brakes on thinking there is a collision coming. it is every single day using cruise control
I started receiving messages that the cameras were occluded and that there was an error and ALL of my system safety features were disabled. No front forward collision warnings, no emergency braking available, nothing available and the system computer that showed all of the traffic and everything completely disappeared from the screen, the car has literally gone blind. All of this started happening on with the camera issues in May, but I thought that it was a software issue and would be addressed in the next update, although the critical disengagements were supposed to be sent to Tesla. Location: 35.084609,-92.414194 While driving through the roundabout north to south on Amity road, the car has twice now applied automatic emergency braking while exiting the roundabout. There is a billboard 200ft from the road with an image of a person on it. The car is confusing that as a person in the road. This has caused my dog to be thrown (thankfully buckled in) and us to almost be rear-ended both times. This is extremely dangerous issue.
The contact owns a 2021 Tesla Model 3. The contact stated while driving approximately 35 MPH, the vehicle experienced phantom braking, causing the contact to lose control of the vehicle. The contact was driving in a rainstorm during the failure and stated that the vehicle had not hydroplaned nor had the accelerator pedal been released. The contact stated there was no warning light illuminated. The contact stated that the failure occurred another time during a rainstorm. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 74,000.
While using the Full Self Driving Mode (FSD) the vehicle will randomly and without warning slam on the brakes at highway speeds and decelerate to 30-50mph on an active highway. If the highway is multilane, the car will slow and attempt to change lanes. It appears the vehicle thinks that something is in the road, even if the road is clear. This happens multiple times on my longer road trips, and is very concerning. It did this yesterday with a semi-truck closely behind me. The vehicle has gone into service and I did bring up the issue, but the service team was unable to assist on this issue (screenshots attached). The software currently installed in my vehicle is 2024.3.15, the latest available from Tesla. This issues has been ongoing from every software update I have had in the car however.
When using cruise control, auto lane assist and auto pilot, the vehicle will brake or slow down without apparent reason. This can happen suddenly and dramatically. This can happen when there are other cars nearby, but also when there is no other traffic on the road. It is distressing enough when there are no other cars nearby, but can be a hazard when there is other traffic behind me and they have no warning this is happening.
This car, like many Teslas, is experiencing what has been come to be known as phantom braking. When in the autopilot or full self driving, this car will occasionally to frequently, depending on the day, suddenly break where it will decelerate from say 65 mph to 45 or 50 in a very short period of time. This can happen in very busy traffic conditions. It seems to happen when I'm going faster rather than slower. For example, I haven't experienced it so much on City Street. It seems much more prominent on highways. This is an issue that has many YouTube videos about. It has been reported by many Tesla owners as well as the media.
My vehicle is 3 years old and has roughly 52000 on it. After a recent software update (within the past month or so) all of the automatic safety features such as automatic braking, blind spot monitoring, cruise control stopped working on the vehicle. I brought it to Tesla and they confirmed the issue and said the entire ECU needs to be replaced, a $2700 expense on a 3 year old car. There is a warning message every time I start the car that says the automatic breaking is not working.
While driving [XXX] on [XXX] in Texas, the Tesla autopilot abruptly and forcibly applied the brakes while I was driving approximately 77 mph. I was almost rear ended by a semi truck due to this issue. This happened again about 20 miles down the road and I disengaged the cruise control and autopilot altogether at that point. Full Self Driving and cruise control brakes randomly and violently with no warning at highway speeds. This has happened no less than 20 times on one trip on 8 March 2024. This has put myself and other vehicles in extreme danger. The model has had this issue known by the manufacturer and nothing has been done. This braking occurs with no warning other than the vehicle slamming on the brakes. This needs to be fixed asap, its a hazard to me as the driver and the general public
My 2021 Tesla Model 3 has various issues with the auto pilot feature. When engaged the car makes sudden and abrupt stops while driving. This has happened various times while driving on the freeway when there were no obstructions present. The autopilot feature also prompts to change to the right lane, when dismissed the prompt appears again repeatedly. This is repeated throughout the drive and poses a major distraction. There was supposed to be an “over-the-air” correction but my vehicle has never received the software update.
I leased a Tesla Model 3 on [XXX] in Tampa, Florida On [XXX] the car went into Sudden Unintended Acceleration as I was coming out of a "Hold" at a Yield sign where I was at a complete stop waiting for traffic to fully clear for a right hand turn onto a two lane street. FSD Computer module (AP3 module) when starting to fail causes all active safety systems to top functioning, including but not limited to Lane Assistance, Automatic Emergency Breaking, Adaptive Cruise Control, Lane Departure Warning, Lane Keeping Assistance, Blind Spot Warning, Pre-collision systems, etc. This is due to the ECU that processes the video feeds from the cameras needed for these function begins to fail, and is not surprising. What is surprising is there are absolutely zero customer or service facing alerts for this issue. Customers simply are unable to manually activate some features, vehicle display has difficulty recognizing objects and lane markings, and the service menu (meant for vehicle service/repair) shows part of the ECU as critical in diagnostics, but does not provide an alert. Driver assisst function for cruise control/autosteer failed/malfunctioned. Applied Emergency braking systems while autosteer/driver control active. Multiple times on freeway. OTA software Updates require additional data downloading and are not required to install. Never received update and safety issues resolved via OTA are already being exposed and they are not allowed to do anything to prevent any damage or loss to the vehicle. Wifi at all government EV charging stations is a basic requirement.
Driving on I-5 North from Los Angeles to San Francisco with adaptive cruise control activated. Suddenly and without warning car activates emergency braking as if it sees an obstruction on the road but obviously road was clear of any obstructions. This happened 4 times then I finally decided to no longer use cruise control. This happened on a clear sunny day around noon time 9/8/23.
Recently on a trip between Wichita Falls and Quanah Texas, my Tesla Model 3 while on autopilot repeatedly braked hard while traveling at 75 mph in moderate traffic. While this is a frequent occurrence with my model 3, this is the first time I felt at risk of an accident due to how hard the vehicle was breaking.

Major phantom Braking at 70mph on interstate 5, 4 times the brakes slamed on as if yo had done an emergency stop from 70mph to 20mph fortunately we were not rear ended as I had my son and grandchildren in the car. The car was in cruise control on each occurrence I finally had to stop using it for fear of a major accident
The vehicle was traveling in excess of 75 mph with only the autopilot system enabled. The vehicle brakes engaged by itself enough to squeal the tires and the rear end momentarily broke free from the pavement. I disengaged autopilot and drove for approximately 5 minutes. I reengaged autopilot and approximately 5 minutes later the brakes engaged again. I left autopilot disengaged for the remainder of the trip (~ 4 hrs) There were no other vehicles nearby but the breaking was severe enough that if another vehicle had been behind me, it could have caused a collision. The dealer acknowledges that this unintended breaking does occur on other vehicles. There was no warning that the vehicle would severely brake. The weather was clear with bright sun on route I25 south of Denver.
Just completed a 2week trip and the car attempted to stop on its own over 37 times (stopped counting) on the highway. 2times I had a semi within a block of my rear of the car. Since this occurrence is not anticipated it worries me that it's going to cause an accident. Reported to Tesla and it's days the feature is beta, I don't believe that for a second . This my 3td car with adaptive cruise control. I was not using the auto guidence feature.
When cruise control is activated at highways speeds, vehicle sometimes brakes abruptly for no reason whatsoever. Issue is phantom braking. No other cars in sight, no changes in speed limits. Happened repeatedly on CA Hwy 46 between Interstate 5 and US 101 on 7/31/23 and 8/2/23. Similar instances observed on other highways, too.
“Phantom Braking”. Driving on the interstate approximately 75 mph using cruise control, auto steer, not auto pilot. Within a span of three hours today we experienced at least six episodes of the car emergency braking for apparently no reason. Immediately going from 75 to 40 mph. Thankfully no one was close behind us. We finally stopped using the cruise control. Now afraid to use it at all.
Constant phantom breaking
When vehicle is in cruise control or autopilot the car will suddenly brake without warning. It will do this numerous times and could cause an accident if a vehicle is following close behind.
Multiple phantom breaking events while driving at highway speed. Luckily there were no cars behind us, otherwise there could have been a collision. Caused driver and passenger whiplash and items in car to be forcibly displaced. There were no advance warning. Tried to report to manufacturer but they seem to be closed for the holiday, will try again to report after holiday.
Car “Phantom brakes” constantly when going over a hill when it does not have vision. The braking is aggressive at freeway speeds and poses a risk for rear end collisions or road rage incidents if people think they are being brake checked.

The incident first happend (and has been happening ever since) on I-8, I-10, I-20 east bound 2 days ago. Driving on EB I-8 with speed limits 75MPH, engaged Tesla's well touted onboard Enhanced Auto Pilot (EAP) with Auto Steer control. There was no one ahead of us for over a mile. The weather was clear and sunny, time of incident around noon. Max speed was set to 75 on EAP. All of a sudden, we felt a jerky motion, and realized the EAP has activated brakes, dropping speed from set 75 to about 50mph. No visual or audible alarms, the EAP did not disengage. The driver took over to speed up to avoid being rear ended by vehicles coming behind us. It happened several times more during our trip and then we decided to manually drive. We have video documentation available.

While using the autopilot or cruise control feature The vehicle slams on the brakes without driver intervention. I found this appears to be a common problem called “ Phantom Braking” I opened ticket with the local Tesla facility but they required an accurate time storm of the incident to be able to troubleshoot the issue. This is a major safety issue and I request it be investigated as soon as possible. I read some news reports that Tesla Phantom Braking has caused major accidents and I don’t want to be in one.
While on Autopilot, the car suddenly phantom brakes when there is no risk of any collision ahead. This has happened multiple times on high speed highways. Within 1 hour, this happened 5 times and caused near-collisions with the car following my car.
My full self driving computer has crashed for the 3rd time since owning the vehicle which controls all of the cars safety features.. Tesla says they won’t do anything because the car is past the warranty by 9,000 miles.. so I’m basically stuck with a 1980’s Tesla.
During the time we have owned this car, it exhibits random and sudden deceleration events when operating AutoPilot (adaptive cruise control) on highways and freeways. The events are not explainable by presence of any notable present or potential hazard. The risk presented to and by surrounding (following) vehicles is so significant that it renders this function unusable. Saturday, April 1, was the last time this occurred before we decided we could no longer safely use this function, instead going to full manual control of the throttle and brake for all highway driving.
there were 2 phantom braking incidents today traveling between pasadena and berkeley onhighway 5. it was getting cloudy. the first incident occurred just after i passed under an overpass, and thought it might have been due to the lighting. the braking was mild . there were other cars passing me on the left. i had the cruise control set to around 74mph. i probably slowed to 65mph the second phantom braking incident was about an hour later, and it was HARD. fortunately the cars behind me were far enough away. the cruise control was also set around 74mph. interestingly, no audio alerts were issued with either incident. normally, when driving without cruise control, the car will issue audio alerts (loud, urgent beeping) if a car gets too close. in both of my cases, there were no cars in front of me as i was in the slow lane.
The car will apply the brakes suddenly when in control when there is no obstacle to be avoided.
My model 3 on crusing speeds on high ways, automatically applies brakes and makes sudden,abrupt, jolted slowing down which causes extreme pressure on persons body
While driving on the interstate I was using Autopilot (cruise control). I started into a slight curve when the vehicle suddenly emergency braked. The roadway was clear and thankfully no vehicle was following close behind me. Also there was no warning alarms, just a sudden, aggressive braking. This has happened several times before but the first time with no alarms beforehand.
My operating my Model 3 in autopilot mode, there is one particular location that my car "phantom brakes" every time I pass. I travel this location an average of 4 times per week. I have been reporting the phantom braking to Tesla for almost two (2) years and they still have not fixed the problem. They claim the car is doing what it is supposed to do. The phantom braking occurs on a divided highway on an overpass that passes over a highway with a lower speed limit ( 37° 4'54.40"N 94°18'48.09"W). It only occurs in the east bound direction. Tesla says their mapping information is provided by Google and until Google updates or corrects their mapping data, the problem will persist. I was driving with the Autopilot on, traveling about 65 mph on the freeway when the automatic emergency braking system kicked in for no reason. There was absolutely nothing in front of the car when it aggressively braked, very suddenly. I was very lucky no one was behind me. I have not reported this directly to Tesla, no one has inspected or confirmed this event.
This incident happened last summer, 2022 but I recently read that it is happening to many other people and thought I should report it. I was driving on Hwy 71 south in the Cincinnati area with the ADAS with the cruise control and auto steering in operation. All of a sudden the brakes slammed on with me not doing anything to cause it. There were no cars behind me and I touched and released the brake pedal and it went back to a normal driving mode. I don’t believe I hit a pothole or bump on the highway. I did not report it to Tesla.
While using the base level of "autopilot," remaining in a lane and maintaining speed, the car will randomly slam on the brakes even when there are no vehicles or objects in front of it. The first incident occurred while driving late at night on an interstate. There were no vehicles in front of the car, but it rapidly reduced speed from 75mph to 30mph, before immediately speeding up again. The camera based "autopilot" is essentially useless and I have decided not to use it when driving.
On 1-14-2023, while driving westbound on I-64, around sunset (5:20ish pm) in a straight portion of the highway, the vehicle suddenly braked for no reason. There was nothing in the road and no cross traffic. There was an all white semi in the right lane that I was approaching in the passing lane. I immediately disengaged the cruise and continued my trip with no further incidents.
While driving on an otherwise unobstructed road, with no nearby obstacles on a Rural South Dakota Interstate, the vehicle suddenly slammed on breaks in an unsafe fashion, forcing the vehicle behind us to swerve to evade our car. We were told by Tesla service that sometimes, "road shimmering" can cause this behavior, and there is no remedy available. In response to this, I gunned the accelerator to try and remedy the issue, hitting unsafe speeds before being able to fully return control to the vehicle. This was NOT while using the vehicle's autopilot system for FSD (which was available on the car), but adaptive cruise control, though the same issue happens with FSD regularly. This is not the first instance of this happening (I have stopped using cruise control and FSD on the vehicle since), but the first time it seriously endangered myself and another driver.
