Incident Description: While traveling at 45 mph with Adaptive Cruise Control (ACC) active, the vehicle experienced a "phantom braking" event triggered by high-contrast shadows on the roadway. The Collision Mitigation Braking System (CMBS) incorrectly identified a shadow as a physical obstacle and abruptly applied the brakes in a clear path of travel. I was forced to manually override the system by applying the accelerator to prevent a potential rear-end collision. Safety Risk & Technical Context: This defect is repeatable under specific lighting conditions and occurs across multiple speed intervals (35–45 mph). I believe this incident is directly related to the ongoing NHTSA Engineering Analysis (EA24002) regarding inadvertent AEB activations. Furthermore, recent Service Bulletins (such as MC-11024251-0001) regarding Front View Camera and Radar Sensor updates suggest a known failure in the system's ability to process high-contrast environmental data. The contact owns a 2024 Honda Pilot. The contact stated that while driving approximately 75 MPH, the brakes inadvertently activated after passing a truck on the highway. The contact stated that the vehicle unexpectedly slowed down to 40 MPH. The contact stated that there was no warning light illuminated. The contact stated that while the accelerator pedal was depressed, there was an abnormal banging sound coming from under the floorboard. The contact pulled over and checked the vehicle, and saw a puddle of transmission fluid on the ground. My Honda is having strange, intermittent issues with a plethora of things. I have taken it to a diagnostic specialist who worked with a phone mechanic and noone can tell me what is going on. My car will fully shut off at stop lights randomly, so I have to turn off the auto-idle feature every time I drive. Sometimes my power steering cuts out and my wheel locks and unlocks and my computer screen shuts off when this happens. I have to shut off and start my car for it to work again. Other times, all of my automated safety features stop and I get a huge slew of messages telling me that my lane assist, automatic hill braking, and a ton of other automatic safety features don't work. While driving the car flashes a BRAKE warning on the dash and then automatically begins emergency style braking attempting to bring the car to a STOP, but this happens while driving at highway speeds which is dangerous for rear approaching traffic. I was driving down a residential road at about 40 mph. There was no car or other obstacle in front of me, but my car suddenly slammed on the brakes. It brought the car to an abrupt stop for no reason. If a car had been behind me, they would have slammed into me. It was so abrupt, I felt like I had whiplash. It was terrifying. I had taken the car into the dealership to due to the brake pin recall just 3 days ago. They inspected and said the pin was fine. When I called them to report this experience, they said it was unrelated to the recall. I am bringing my car into them tomorrow to inspect what happened with the automatic braking system.

The automatic emergency braking system in my vehicle has repeatedly activated without any visible reason. The brakes engage suddenly even when there are no cars, curves, reflective surfaces, or obstacles nearby. This has occurred on straight, empty roads and in clear conditions. We have previously taken the car to the dealership for issues related to sensors and wiring, and were even told at one point to turn off certain safety features. Most recently, we went through the dealership and escalated the issue to Honda corporate, but they closed our case and claimed there is nothing wrong with the car. I am very concerned that this unresolved issue could lead to the brakes locking up in a dangerous situation, potentially causing an accident.

Automatic emergency breaking applied at 50mph on a perfectly clear road with not a single car around. Caused panic in the vehicle and would have absolutely caused severe collision if there were any cars behind. Dealer said cannot do anything without reproducible issue. Police was not called. Thanks God nobody got hurt. Not the first time this happens, it happened crossing railroads before. But this time there was absolutely nothing on the road.

Sudden automatic brake was applied for no apparent reason. There was no collision situation at all. Car suddenly applied strong brake when we were driving at 40 MPH. The car came to a complete stop on a busy road, luckily for us the vehicle behind was bit far back. It was so scary.

The wheel speed sensor has malfunctioned. Warning lights, messages, and alarms displayed that multiple safety features were not working and that the dealer needed to be contacted. Some of the messages included: Parking Brake System Failure, Cruise Control System Failure, Vehicle Stability Failure, Road Departure Mitigation System, Lane Keeping Assist System, Auto High-Beam Problem, etc... The warnings first appeared on January 8, 2024. The Pilot was taken into the Honda dealership on January 9, 2024. It remains there with no definite time for the problem to be resolved.

Today, the same problem with my car reemerged immediately after I collected it from the dealership, where it was supposedly repaired for this issue. Upon driving away, making three stops, and using both the Brake Hold and Engine Idle-Stop features, I encountered the same malfunction, leading to the flashing of all warning indicators. Alarmingly, the parking brake engaged on its own at traffic signals, and the car wouldn't turn off even when this issue was occurring. Previously, I had provided the dealership with a detailed description and photos of this issue. They identified the cause as an improperly installed rear wheel speed sensor, which they claimed to have fixed and reset the error codes.
