I am submitting this complaint to formally report a serious and widespread safety defect involving the front-facing camera system on my 2021 Honda Pilot. This defect has resulted in multiple safety system failures and has placed myself, my passengers, and other motorists at unacceptable risk. The front camera on my vehicle does not function as designed, which has caused the complete loss or repeated failure of critical safety features, including adaptive cruise control, lane keeping assist, lane departure mitigation, collision mitigation braking, and other forward-facing driver assistance and alert systems. These systems are not optional or convenience features. They are advertised and sold as essential safety technology intended to reduce accidents, mitigate driver error, and protect human life. Their failure substantially impairs the vehicle’s safety performance and creates a foreseeable risk of serious accidents, injuries, or fatalities, particularly during highway driving or emergency situations. The front camera has malfunctioned after locking down the brakes on my vehicle. Without the camera, the automatic braking system, forward collision, lane departure, adaptive cruise control, and high beam are no longer functioning. While the high beams are not as big of a deal, the safety features such as the braking system and forward collision are of more significance.
Multiple safety features are failing on this 2021 Honda Pilot. At startup, I get the following errors: Collision Mitigation System Problem, Road Departure Mitigation System Problem, Adaptive Cruise Control Problem, Lane Keeping Assist Problem. Dealership says it is an infotainment system problem and it will cost about $1,200.
On multiple occasions the hazard mitigation system has picked up on hazards that aren't there causing rapid braking AND steering over correction. Currently the button on my vehicle that has an image representing the ability to turn the system off doesn't appear to be connected or working. I have always left the system on as it felt "safer" when I bought the vehicle new. I haven't even tried to turn it off until recently when the system seemed to start malfunctioning. Today (11/19/2025) while traveling the interstate for a routine doctor appointment the adaptive cruise control randomly shut off as I was driving multiple times. If I changed lanes and a vehicle was in front of me the adaptive cruise control would just shut off with no warning or alert rather than adjust the speed as the system is designed to do. Vehicle is setting multiple faults for ADAS system. LKAS, ACC, collision mitigation system, auto high beam lights inoperable. Forward facing camera in the top of the windshield has failed.
Upon startup, LKAS, ACC, FCW and brake lights are illuminated and the corresponding systems are non-functional. This failed with less than 50k miles and the car was ~3 years old. This is making the car less safe to drive because some of the safety systems are non functional. The basic cruise control also does not work.
Component inspected by Honda dealer It’s the forward camera it has already malfunctioned and no longer works I only have 62,000 miles on my pilot. This seems excessive there is definitely a defect in these vehicles. The incident is the warning lights that came up and no longer worked
I have two issues. First issue is the car will hit an intermediately not start once shut off and will take a few times of making sure the car is completely shut off opening your doors letting it power down before being able to restart. The next issue is the forward collision notification will randomly beep at every car that goes by specifically white cars. And also the adaptive cruise control brakes a little bit harder than expected.
Dash warning lights come on for Lane keeping assist, collision mitigation, ACC, brake system, auto high beam.
The 2021 Honda Pilot would randomly flash brake, which would lock up the steering wheel, electronic warnings going off when nothing was there, and then all of the CMBS warnings came on. Dealer charged for diagnosis, and requires $2,300.00 to fix the sensor. The vehicle was never in an accident, and various ghost electronic issues would be present over the last few years before the system failed with multiple warnings.
I have photos, but unable to up load at this time. Suddenly, 9 different consecutive warning messages with bell associated with each, appeared on the instrument panel. The barrage of these 9 messages continued until I shut the engine off. I was in a very congested parking lot when this began. The Pilot was subsequently checked for codes and battery condition. The evidence suggests the VSA Modulator. The Messages continue when powered up with or without the engine running. Lane keep assist problem. LKAS All wheel drive system problem. VSA problem. ACC Adaptive cruise control problem problem. Road departure mitigation system problem. Collision mitigation system problem. Collision mitigation system problem. TSA trailer stability assist problem. Hill start assist problem. Power steering system (EPS) problem. Plus: AWD, Brake System and steering caution lights remain on continuously. I am very glad we were not towing our travel trailer this week.
Recently , I stated seeing CMBC failure in my 2021 Honda Pilot 2wd. My car has 41500 miles in it. I took car to dealer ship and dealer ship says they are no recalls i need to change my camera. i am not understanding it's like brand new car and not driven many miles. i research on google and there are so many pilot owners having same problems. Can this be recall?
The camera in the windshield used for collision mitigation, cruise control, lane keep assist, auto high beams, etc. has been found to have gone bad and need replaced by the Honda service center near me. The dashboard indicates all of these issues and disables the use of these safety features. The cost to repair is $1900 which includes camera, installation, and calibration.
While driving the CMBS (crash mitigation braking system) turns on at random timed intervals causing sudden breaking and jerking of the steering wheel towards center. This causes severe safety issues including increasing the chances of a wreck due to wheel jerk movements during turning and sudden breaking. The CMBS error message pops up on the touchscreen and during the episodes of sudden onset the entire digital display both on the gage cluster and entertainment center go black and an audible clicking is heard in the steering column.
Here it is over two years later and passenger seat switch has not been replaced I was At Gunn Honda in 2/2024 and 6/2024 requesting an update of replacement answer was no parts. This seems to be an on going problem for many Honda owners according to the forums. My Adaptive Cruise Control, lane keeping assistant, collision mitigation, road departure mitigation, and auto high beams have all gone out after going to the car wash. I have cleaned the windshield but I find it crazy that this is a common problem and there is no recall.
The vehicle will provide "BRAKE" warnings on the dash display or display the warning and auto-brake when on a 2 lane road, with only a painted divided line, when on a curve and an oncoming vehicle is coming wards my vehicle, but there is no apparent oncoming collision with the oncoming vehicle and with no vehicle in front of me. This has been intermittent and ongoing since vehicle purchase.
None of the safety sensors were working.
Normal driving of vehicle when suddenly several warning lights began to appear and show up every time when driving (LKAS, ACC, Mitigation, etc) despite these features working. Honda dealership claims it is due to a faulty front camera. Safety at risk because if these systems do fail, there will be no way of indicating.
Car slammed on breaks for no reason. Good thing I was paying attention and was able to keep my vehicle under control. My neck hurt for the rest of the day and is Sore today. Now my cars system says the system isn’t working correctly after my car slammed on the breaks for basically no reason. I know this is an ongoing problem certain Honda models already. I believe every Honda with this system should be recalled.
My car started stating that 6 systems were having problems. The adaptive cruise control system, automatic high beam system, blind spot information system, road departure mitigation system, collision mitigation system, and lane keep assist system. I took the car to the Honda dealer it was purchased from and was informed that the technician found that the multipurpose camera was not working and needed to be replaced. Car in front of me stopped. My vehicle did not causing a rear end collision. I do not recall the brake warning flashing to alert me to brake. The automatic braking system did not seem to engage. Even though I pressed the brake pedal the vehicle did not seem to want to stop. It reminded me of a vehicle without power brakes. Nor did the wheels lock up or leave any skid marks from hard braking.
I was traveling with my daughters [XXX] , and [XXX] , in the back seat. It started raining heavy just as I passed a vehicle and returned to the right lane. I reduced my speed, then felt the vehicle hydroplane slightly and felt the traction control system engaged. As soon as the traction control icon displayed, I began to feel the traction slipping and noticed a slight unresponsiveness in the steering. My speed suddenly dropped 15-20 mph, without braking, or any change in pressure on the accelerator. I then tried to accelerate, but was unable to possibly because of loss of traction I noticed the traction light was flashing. The vehicle I had just passed moments before was quickly approaching at roughly 65 mph (speed limit was 70 mph) and had no warning of the issue with my vehicle decelerating, luckily they were paying attention and able to pass safely....
VSA modulator failed at approximately 69,000 miles. Safety features were inoperable putting my family at risk. Dash said to contact a dealership and to not drive the vehicle. The dealer informed us that the car was safe to drive as long as it wasn't making any funny noises. I paid the dealer to replace the VSA modulator per their diagnosis but then upon the car being returned to my possession, I was informed all the alarms are present on the car again. The dealer is instructing that I now need to replace the forward camera / radar sensor.
Frequently, my 2021 Honda will apply the brakes without warning or very abruptly. While using the adaptive cruise control, the car will brake suddenly when there is not a car anywhere near me. Also, when driving on two lane roads, the car slams on the brakes when there is not a car in front of me.
On 3/18/24 I started my car on a routine after school pickup trip but my notification system started blinking with different sensor problems. I took pictures of the display and tried to drive my car and saw at the time it did not affect drivability. I made an appointment with my dealer and brought it in 3/25/24. I started researching if there might be a pattern with other Honda Pilots and did find some discussion boards about 2020/2021 Honda Pilot that had a similar event—sensor problems displaying for no evident triggering event. I brought my car in for the appointment and the advisor said they would run a diagnostic as well as address 2 active recalls on my Pilot and take care of the maintenance. Twice now at red light my pilot has not started due to stop start feature. It was stuck in park / neutral . Car wouldn’t restart or go into gear. Almost everyday at least once car is hesitant to start after engine cuts out . Then dash lights like system collision isn’t working. Extremely dangerous was at a light on major interstate road. On Friday, August 18th, 2023, I started up my 2021 Honda Pilot EX and all safety system warning lights (ACC, LKAS, CBWS, etc) came on. I called Metro Honda Montclair and a service advisor asked me to bring the car in to them as soon as possible. I asked if it was safe to drive, and he said I honestly do not know, but if you can drive it, I advise you to take the slow lane and avoid the freeway. They kept my car over the weekend, and on Monday August 21st, 2023, Dealer diagnosed the problem as a defective multi-purpose camera. These cameras are on back order until October. They said multiple codes came in in the diagnostic but they will have to start with this. Vehicle shut off while at a busy intersection light. Unable to restart the vehicle for several minutes. All safety systems (crash mitigation.....) were disabled. Unable to find a current recall through Honda and the dealership is closed since it is sunday.
On 2 land roads especially with no other vehicles in view, the car will suddenly brake, flash the dash for emergency braking and violently shake the steering wheel back and forth and almost take it out of your hands. It was done this since new. Taken to dealer 11/15/21 for this issue, they "aligned it" and made it somewhat better but still did the braking and steering wheel shake on way home from dealer. We just put up with it as the vehicle cost so much. The dealer also replaced the windshield as it cracked within 3 months of new sitting in a parking lot. This again required another paid alignment of this system. Once when driving down a two lane road with a car coming in the opposite direction in their lane and I was in mine, the car braked. Then once getting on a road nothing coming nor any other vehicle around the vehicle did a hard brake. I just purchased the car last month and it just started happening.

I’ve purchased my Honda pilot 2021 Brand New within the last two months. The CMBS regularly activates with the brake warning lights, audible, and steering wheel shaking. There has never been anything in front of the vehicle when the CMBS activates. Most recently, the CMBS activated and applied the brakes when driving approximately 40 mph for no reason. Obviously, the safety of the driver and all passengers are at a risk of an accident caused by the CMBS' faulty activation, as well as any other people and/or property involved. We have been complaining to the dealership regularly since we purchased the vehicle over 2 years ago, to no avail. The dealership states they have not been able to confirm or reproduce the CMBS' faulty activation. No one other than the dealership has inspected the vehicle for this problem. The car braked automatically without any obstruction or car in front. I lost control of the brakes causing a severe Jerk and car to slip on freeway. I had a 2 month and 5 yr old in the car. We nearly escaped a serious accident as the truck was behind us in full speed. The whole warning/stopping event occurred in a few seconds. There were no obstacles in the road," the driver wrote. Auto braking applies randomly cause brakes to lock up happens when driving in any conditions. This problem has already caused a minor accident with my vehicle due to brakes locking up and no throttle
Forward Collision Warning system has many times flashed the large ‘BRAKE’ and loud beeping warning when collision was not imminent. It never actually applied the brakes but worries us that it could slam on the brakes unnecessarily and cause an accident by getting rear ended or causeing the car to lose control and skid cross lanes. Most of the time this happens on an undivided road, sometimes on a curve, when the car may think the oncoming traffic may be coming at us.
There have been multiple instances where the Automatic Braking system has engaged preventing me as the operator from making adjustments. I have almost been in 2 collisions due to this feature. The automatic braking system prevents me from making a correction and will automatically lock up the brakes and steering. Both times I have almost been rear ended due to this feature. There has been an instance where the braking feature engaged where there was no car in from of me but a car coming from the other lane and my Honda Pilot engaged the automatic braking system. Luckily there was no car behind me. This feature while could be useful is not in my opinion a safety feature but a safety hazard due to these multiple instances.
