2016 TESLA MODEL S Forward Collision Avoidance Warnings Problems

The infotainment unit in the car that controls functionality, I.e. navigation used to monitor battery availability is malfunctioning because of glue seeping from the unit and causing the screen to go black intermittently while driving creating a safety hazard for the driver and others due to a distraction. When this happens the driver loses visibility to navigation and other controls central to the vehicle functionality. Tesla is aware of this manufacturing defect that is a safety concern because it requires the driver to not focus on the road.

Dear NHTSA, I am writing to file a complaint regarding a SUA incident that occurred on October 20, 2023, involving my 2016 Tesla Model S. On the day of the incident, I pulled into my garage and began to park my car. When I pressed the brake pedal, the car suddenly accelerated forward, crashing into the garage wall and totaling the vehicle. The garage wall was severely damaged and will need to be rebuilt. This is not the first time that my Tesla has experienced SUA. In 2017, I filed a complaint with NHTSA regarding a similar incident. In my previous complaint (issue # 11000097 dated June 19, 2017), I described a similar incident that occurred in December 2016. A newer Tesla will come to a complete stop with the one pedal operation, however my older car does not. It will roll through a stop sign or red light. This has been fixed on full self driving cars but should be fixed on single pedal operation cars.

TOUCH SCREEN STOPS WORKING, REMOVES BACKUP CAMERA FEED AND ALL AUDIO ALERTS ARE SILENCED. THIS IS A DIRECT RESULT OF THE EMMC FAILURE IN THE MCU TOUCH SCREEN.

THE MAIN COMPUTER IN THE CAR GOES BLANK. THIS AFFECTS ALL PARTS OF THE CAR. IT HAS HAPPENED WHILE STATIONARY AND IN MOTION. WHEN IT SHUTS OFF YOU LOSE CONTROL OF ALL SYSTEMS INCLUDING: NAVIGATION, EMERGENCY BRAKING, LANE ASSISTANCE, USE OF CAMERAS, ALL ACCESS TO SAFETY FEATURES. I HAVE READ THERE IS A RECALL BUT WHEN I CONTACTED THE MANUFACTURER THEY REFUSED TO FIX THE ISSUE.

THE MCU (MASTER CONTROL UNIT) IS THE 17" TOUCH SCREEN DISPLAY THAT CONTROLS MOST OF THE FUNCTIONS OF THE VEHICLE. VEHICLE HAS BEEN PLAGUED OVER IT'S LIVE WITH FREEZE UP'S AND REBOOTS. APRIL 2019, SCREEN LOCKED UP AND WOULD NOT RESTART, WHICH CAUSED VEHICLE TO LOOSE PROPULTION, POWER ASSISTED BRAKES, POWER STEERING, WINDOW CONTROLS (HAD TO USE EMERGENCY MANUAL RELEASE TO EXIT VEHICLE, THEN GO BACK IN AND SQUEEZE BETWEEN THE SEATS TO GET MY DISABLED SON OUT OF THE BACK AS THE BACK DOOR HANDLES DID NOT OPERATE EITHER AND HAD TO TRY AND FIND THE WELL HIDDEN MANUAL RELEASE HIDDEN UNDER THE CARPET WITH NO MARKINGS TO GET THE REAR DOOR OPEN AND HIM OUT OF HIS CAR SEAT). VEHICLE ALSO WOULD NOT GO IN TO PARK, I HAD TO USE MY SHOES (WITH STEEL TOE) UNDER THE WHEELS TO KEEP THE VEHICLE FROM ROLLING DOWN AN INCLINE....
