The brake pressure switch assembly may leak brake fluid into the brake pressure switch's electrical components and cause an electrical short-circuit.

Consequence: An electrical short in the brake pressure switch can overheat the circuit and increase the risk of a fire while driving or parked.

The sealing washer may not seat correctly in the pilot bore holes, allowing the high pressure fuel rail assembly to leak.

Consequence: A fuel leak in the presence of an ignition source increases the risk of a fire.

The contact stated that she was a Medical Doctor who was calling to report an incident involving a 2019 International MV607. The contact stated while the tow truck driver was loading an abandoned vehicle onto the flatbed, the flatbed moved forward independently, and the driver's pelvis was pinned against the flatbed while the driver's back was against the oil reservoir that appeared to be mounted on the truck cabin. The contact stated that the tow truck driver had been pinned for over an hour before he was found and pronounced deceased at the scene. The contact stated that the deputy sheriff operated the controls and moved the flatbed backward to release the deceased; however, the flatbed would not stop moving. The Deputies turned off the Truck, and the flatbed stopped moving backward. Additionally, the contact stated that the deputy sheriff noticed an oil leak and a burning oil odor. The tow truck remained at the scene. No further information was provided. The failure mileage was unknown.
