Defect: ON CERTAIN SCHOOL BUSES AND TRUCK MODEL VEHICLES MANUFACTURED BETWEEN FEBRUARY 28, 1999, THROUGH APRIL 5, 2004, THE ANTI-LOCK BRAKE SYSTEM ELECTRONIC CONTROL UNIT (ECU) MAY MISINTERPRET A CORRUPT WHEEL SPEED SIGNAL.
Consequence: THE CORRUPT WHEEL SPEED SIGNALS MAY IMPROPERLY ACTIVATE THE ABS, INSTEAD OF DEACTIVATING THE ABS. THIS MAY RESULT IN THE DRIVER EXPERIENCING A HARD PEDAL FEEL AND A DECREASE IN DECELERATION AT THE END OF THE STOP, RESULTING IN EXTENDED STOPPING DISTANCES WHICH COULD CAUSE A VEHICLE CRASH, POSSIBLY RESULTING IN PROPERTY DAMAGE, PERSONAL INJURY, OR DEATH.

Defect: ON CERTAIN HEAVY DUTY TRUCKS, A BRASS FITTING CONNECTS THE DOUBLE CHECK VALVE TO THE BRAKE RELAY VALVE. WITH CERTAIN INPUTS TO THE VEHICLE, THE WEIGHT OF THE CHECK VALVE ON THE FITTING CAN CAUSE THE ASSEMBLY TO RESONATE. THE SUBJECT VIBRATION CAN CAUSE EXCESSIVE STRAIN LEVELS IN THE FITTING, WHICH COULD EVENTUALLY FATIGUE AND FAIL.
Consequence: IF THE FITTING BREAKS, THE REAR SERVICE BRAKES WILL CEASE TO OPERATE WITHOUT WARNING, RESULTING IN AN EXTENDED STOPPING DISTANCE, INCREASING THE RISK OF A CRASH.

IF SOMEONE OUTSIDE THE CAB ADJUSTS/SLIDES THE DRIVER'S SEAT TO ITS MOST FORWARD POSITION, WITH THE PARKING BRAKE SET, THE SEAT WILL CONTACT AND RELEASE THE PARKING BRAKE LEVER.
Consequence: IF THE PARKING BRAKE IS ACCIDENTALLY RELEASED, THE VEHICLE COULD MOVE UNEXPECTEDLY, INCREASING THE RISK OF A CRASH.

THE BRAKES HAVE BEEN DEFECTIVE SINCE CONSUMER PURCHASED THE VEHICLE. CONSUMER'S VEHICLE WAS INCLUDED IN A RECALL FOR THE SAME BRAKE PROBLEM, BUT PROBLEM RECURRED. ABS LIGHT WAS STILL ON AFTER BEING SERVICED 8 TIMES.*AK

I OWN A 2001 INTERNATIONAL FLAT BED TOW TRUCK,ALSO REFERRED TO AS A ROLL BACK OR CAR CARRIER.I HAVE HAD ONE INCIDENT WITH THE THE"L"ARM BRACKET THAT RETAINS THE "L"ARM FOR THE WHEEL LIFT COMING OFF MY TRUCK AND ANOTHER INCIDENT WHERE THE COMPLETE HYDRAULIC CONTROL PANEL ASSEMBLY THAT GOES FROM ONE SIDE OF THE TRUCK TO THE OPPOSITE SIDE, FALLING DOWN.THE EQUIPMENT WAS INSTALLED BY CHEVRON CARRIERS AND WRECKERS,LOCATED AT 7320 W.MARKET ST,MERCER,PA,16137.THEIR PHONE NUMBER IS 724-981-7500 OR 800-886-6400.I CONTACTED CHEVRON AND NOTIFIED THEM OF THE PROBLEM WITH THE HYDRAULIC CONTROL PANEL AND ASKED FOR REIMBURSEMENT FOR THE REPAIR TO WHICH I WAS REFUSED.I DEEM THESE TO BE SAFETY ISSUES.BOTH PROBLEMS WERE CAUSED BY THE LACK OF LOCK WASHERS OR SPACER WASHERS BEING INSTALLED UNDER THE HEADS OF THE BOLTS CAUSING THE BOLTS TO LOOSEN UP ON THE " L" ARM AND THE BOLTS THAT HOLD THE HYDRAULIC PANEL IN PLACE WORE THRU THE PLATE UNDERNEATH CAUSING THE BOLTS TO FALL THROUGH THE ELONGATED HOLES AND FALLING DOWN.IT APPEARS AT THIS TIME THAT CHEVRON HAS BRUSHED OFF MY VERBAL COMPLAINTS TO THEM.THE SERIAL NUMBER FOR REFERENCE TO CHEVRON FOR THE TOW EQUIPMENT PACKAGE IS 10164820.I WAS ADVISED BY CHEVRON THAT THE VEHICLE HAD A ONE YEAR WARRANTY ON IT AND THAT THEY WAOULD NOT BE RESPONSIBLE FOR THE REPAIRS/REIMBURSEMENT..TO WHICH I ADVISED THEM THAT IT WAS A MANUFACTURERS DEFECT THAT CAUSED BOTH PROBLEMS. RIGHT REAR HYDRAULIC BRAKE CALIPER FAILED. *MR THE RIGHT REAR BRAKE CALIPER FAILED TO RELEASE PROPERLY AND RESULTED IN A FIRE. THE CONSUMER WAS TRANSPORTING A BULLDOZER AT THE TIME. ALL REAR BRAKES AND CALIPERS WERE REPLACED. WHILE TRANSPORTING A BACKHOE, THE RIGHT FRONT CALIPER FAILED TO RELEASE PROPERLY AND RESULTED IN A NEAR LOSS OF CONTROL. THE THERE WAS AN OCCASION WHERE THE AXLE HAD BROKEN ON THE VEHICLE WHILE IN MOTION.
