TL* THE CONTACT OWNS A 2009 TOYOTA AVALON. WHILE DRIVING AT VARIOUS SPEEDS, THE DRIVER AND PASSENGER SIDE HEADLIGHTS FAILED TO ILLUMINATE INTERMITTENTLY. THE VEHICLE WAS TAKEN TO A DEALER WHERE IT WAS DIAGNOSED THAT THE HEAD LAMPS NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 49,920.
