| 4,506 vehicles affected

Defect: A CRACK OCCURS OUTBOARD OF THE AXLE ATTACHMENT WHICH GROWS AND RESULTS IN SEPARATION OF THE END OF THAT AXLE, INCLUDING TIRES AND WHEELS, FROM THE TRAILER.

Consequence: AXLE SEPARATION WHILE MOVING CAN INCREASE THE RISK OF A VEHICLE ACCIDENT.

Remedy: DEALERS WILL INSPECT THE TRAILERS AT ALL 8 WELDS ON THE AXLE TO AIR RIDE BEAM CONNECTIONS FOR VISIBLE CRACKS. IF A CRACK IS DETECTED, THE TRAILERS WILL BE REMOVED FROM SERVICE AND THE AXLES REPLACED.

A DUMP TRUCK WAS ON NEW YORK AVENUE SOUTHBOUND AND INSTEAD OF WAITING FOR THE GREEN LIGHT HE MADE A RIGHT TURN ON RED INTO ONCOMING TRAFFIC PULLED INTO THE LEFT LANE INSTEAD OF STAYING IN THE RIGHT LANE WHICH IN TURN CAUSED ME TO PUT ON BRAKES AND CAUSED EVERYONE BEHIND ME TO SLAM ON BRAKES WHICH ALMOST CAUSED AN ACCIDENT. I BLEW MY HORN AT THE AFRICAN AMERICAN MALE DRIVER AND HE PROCEEDED TO SLAM ON BRAKES AND THEN BEGAN DRIVING SLOW IN THE LEFT LANE.
