I hope this letter finds you well. I am writing to express my deep disappointment and frustration regarding the malfunction of the Teammate™ Advanced Drive package in my recent purchase of a 2022 Mirai Limited edition, with the Vehicle Identification Number (VIN): [XXX]. The faulty performance of the system resulted in an accident and subsequent damages to my vehicle. On November 5, 2022, while operating the vehicle with the Teammate™ Advanced Drive package engaged, the car failed to stop as intended and collided with a parked car, causing significant damage. Understanding the gravity of the situation, I immediately contacted my insurance provider and followed their recommended course of action. What component or system failed or malfunctioned, and is it available for inspection upon request? Car suppposed to stop itself as it has all sensors. Still car rear ended other car. How was your safety or the safety of others put at risk? Jeopardy. Since accident occured in residential area my family was okay. What if it was in Free way? Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet, they are not providing me alternate transportation during inspection so not ready yet from my side. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Vehicle was inspected by Insurance as normal accident. Not sure if there were further investigation. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings
