The contact owns a 2020 Honda SXS 1000. The contact stated that while accelerating at an undisclosed speed, the vehicle inadvertently accelerated, the contact then stated that he depressed the brake pedal however, the vehicle failed to stop. As a result, the vehicle crashed into a tree. The contact sustained an injury to the head that required medical attention at the ER. At the time of the crash, the contacts two children were in the vehicle. A police report was not filed. The vehicle was towed to the contact’s residence by the contact’s tractor. The vehicle was not diagnosed or repaired. The dealer was not notified of the failure. The manufacturer was notified of the failure, but the contact had not received a response. The failure mileage was approximately 169.

The contact owns a 2020 Honda Motorcycle Talon 1000X. The contact stated that while driving at a speed unknown, when it was placed into low gear and drive it would disengage from low gear causing the side-by-side to flip downhill. The contact hit another motorist and came to a stop. The contact received injuries to their hands however due to them being harnessed in they did not receive many injuries. Medical attention was not needed. There was no need for police or police reports. The side-by-side was able to be moved. The side-by-side was taken to the dealer who diagnosed that the sub-transmission failed and needed to be rebuilt. The side-by-side had not been repaired as of yet. The manufacturer was made aware of the failure. The approximate failure mileage was 100.

The contact owns a 2020 Honda Motorcycle SXS1000 (PIONEER 1000). The contact stated that while crossing a creek at 5 MPH, he hit a hole which caused the motorcycle to roll over into the creek. Upon landing in the water, the contact attempted to exit the motorcycle but the seatbelt would not detach. As the contact struggled to release himself, his girlfriend who was also on the motorcycle, made three separate attempts to release the seat belt and was finally successful on the third attempt. As his girlfriend was able to pull him from out of the water, the contact was blue and unresponsive after being underwater for nearly 5 minutes. The contact's girlfriend was able to perform CPR on the contact and he was able to regain consciousness. EMS arrived on the scene and checked his vitals; the contact refused to be taken to the local hospital and was taken home by a sheriff. Later that night, the contact's girlfriend had taken him to his local hospital where they discovered that his vitals were low and he was immediately placed into ICU. The contact was released from the ICU the next day and was discharged from the hospital after two days. The motorcycle was initially towed to an independent mechanic and was later towed to another location by his insurance company where it remained in their possession awaiting an investigation. The manufacturer nor the dealer have yet to be notified of the failure. The contact was informed by his insurance that after the investigation concluded, the vehicle would be destroyed. The failure mileage was approximately 800.
