The contact owns a 2025 Honda Civic Hatchback Hybrid. The contact stated that while driving 50 MPH and approaching another vehicle, the adaptive cruise control mode failed to operate as needed. The message " Brake" was displayed. The contact stated that the brake pedal was depressed, but the vehicle did not have sufficient time to stop and crashed into a truck. The front end of the vehicle went underneath the high bumper truck. Several unknown warning lights were illuminated. The air bags did not deploy. Neither the contact nor the other driver sustained any injuries. The vehicle was towed to an autobody repair shop. A police report was filed. The contact stated that he was afraid to disclose to the police officer that the incident occurred due to the adaptive cruise control mode failure. The contact advised the police officer that he failed to depress the brake pedal in time. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 11,300.

On 5/20/25 while pulling out of my house's driveway I saw a warning on my dashboard that the brakes were malfunctioning, that it was unsafe to continue driving, and to stop as soon as possible. The brakes were soft and I could feel and hear a grinding noise when I stepped on the brake. I returned the car to the driveway. The vehicle was towed the same day to the dealer for service. The car was repaired on 5/23/25. The repair invoice states: "Electric brake control internal circuit malfunction...brake hold simulator not pluging...replaced simulator set (part#:01469-T61-A02). Car was tested after repairs." (Purchased on 4/4/25.)

I've previously reported the CMBS (Collision Mitigation Braking System) and RDMS (Road Departure Mitigation System) warning messages that have been appearing too frequently. Today is the day it finally happened, my wife was driving on the highway when auto cruise control with lane keep assist set at 66 on the passing lane, with a truck to the right. Halfway passing the truck on auto cruise control with lane keep assist, our Honda suddenly automatically braked (went from 66mph to ~56mph, there were no vehicles in front of her). She urgently took back control and sped up as the warning lights flashed (collision mitigation braking system problem - 60mph and road departure mitigation system - 58mph, pictures attached) and the driving assist features that were originally on all became disabled. All of this happened at about the same time in a manner of ~3 seconds. She was lucky that no one was behind her at that moment, and she did not get into an accident or worse! Honda has done nothing to fix this issue, and we have already taken the vehicle to the dealership's service 3 times already. Please expand the investigation to ALL Honda vehicles that uses their Driver Assistance System including models of newer years! This is really dangerous!! I've included photos of many of the previous times the error/warning message has appeared, and I've included the photo of the error/warning message that appeared right after incident occurred today. These error/warning messages has come up at least over 50+ times now since I've owned this car.
