Defect: Forest River, Inc. (Forest River) is recalling certain model year 2016 Evo, Salem, Sandstorm, Shockwave, Stealth, and Wildwood recreational trailers. The affected vehicles were mistakenly not equipped with a carbon monoxide detector to warn occupants of elevated levels of carbon monoxide.

Consequence: If occupants are not warned elevated carbon monoxide levels, they are at an increased risk of injury.

Recall 15V359000 — SUSPENSION:REAR:AXLE:NON-POWERED AXLE ASSEMBLY

| 1,497 vehicles affected

Defect: Forest River, Inc. (Forest River) is recalling certain model year 2016 Evo, Salem, Sandstorm, Shockwave, Stealth, Wildwood, Cherokee Grey Wolf, Wildcat Maxx, Vibe, R-Pod, and Surveyor trailers. In the affected trailers, the studs that are used to attach the wheels to the axle hubs may break.

Consequence: If the wheel studs break, the wheel could detach from the vehicle, increasing the risk of a crash.

WHILE DRIVING APPROXIMATELY 55 MPH, THE TRAVEL TRAILER CAUGHT ON FIRE NEAR THE DRIVER'S SIDE WHEEL. THE CONTACT PULLED OVER AND EXTINGUISHED THE FIRE WITH BOTTLES OF WATER. THE TRAVEL TRAILER WAS TAKEN TO THE NEAREST CAMPGROUND WHERE A CERTIFIED MECHANIC INSPECTED THE VEHICLE AND STATED THAT THE FIRE WAS NEAR THE REAR LEFT DRUM AND THE AXLE NUT WAS NOT ATTACHED. THE DEALER (THE TRAILER HITCH, LOCATED AT 215 NORTH FRONTAGE RD, NIPOMO, CA 93444, 805-929-8095) WAS CONTACTED AND STATED THAT THE WHEEL BEARING FAILED BECAUSE THE NUT THAT WAS NEEDED TO HOLD THE BEARING IN PLACE WAS NOT PRESENT. TL* THE CONTACT OWNS A 2016 FOREST RIVER EVO 19RB. THE CONTACT STATED THAT HER GRANDDAUGHTER WAS OUTSIDE OF THE MOTOR CARRIER AND BURNED HER HAND (3 FINGERS BLISTERED) WHEN SHE TOUCHED THE HOT WATER HEATER GRATE. THE WATER HEATER GRATE WAS LOCATED NEXT TO THE TRAILER DOOR AND THERE WAS NO INDICATION THAT IT WAS HOT AND DANGEROUS TO TOUCH. NO INJURIES WERE SUSTAINED DUE TO THE BURN. THE DEALER AND MANUFACTURER WERE NOT NOTIFIED. THE CONTACT HAD NOT EXPERIENCED A FAILURE. THE VIN WAS UNKNOWN.
